Reliability of a new radiological method for assessment of the postoperative immobilization of the first metatarsophalangeal joint.

BACKGROUND: The success of the operative treatment of mild to moderate hallux valgus (HV) relies greatly on the osseous union of the osteotomies at the desired position. Full weight-bearing is often allowed immediately postoperatively with special forefoot off-loading shoes. No precise methodology exists for the estimation of foot immobilization inside those shoes. The aim of this study was to assess the reliability of a new radiological measurement method to assess the immobilization of the first metatarsophalangeal (M1P1) joint inside a postoperative forefoot off-loading rocker shoe. METHOD…

Offloading effect of therapeutic footwear in patients with diabetic neuropathy at high risk for plantar foot ulceration.

AIMS: Custom-made therapeutic footwear is often prescribed to patients with diabetic neuropathy, foot deformity and a healed plantar foot ulcer. Offloading these feet is important to prevent ulcer recurrence. The aim was to evaluate the offloading effect of custom-made footwear in these patients. METHODS: In 171 patients with diabetic neuropathy (336 feet) with foot deformity and a recently healed plantar foot ulcer, plantar pressures walking barefoot and inside new custom-made footwear were measured. At the previous ulcer location and at locations of highest barefoot pressure attributable to

Biomechanical determinants of running economy.

Running economy (RE) defined as the submaximal rate of oxygen consumption during running, has been correlated with better performance. Biomechanical factors such as spatiotemporal parameters and lower extremity mechanics play a significant role in influencing RE. However, the relationship between RE and biomechanical variables in middle-distance running shoes is still unclear. In this study, twenty-one healthy male runners (age: 32.5 ± 5.6 years, height: 1.70 ± 0.10 m, body mass: 69.9 ± 7.3 kg) performed running trials on the treadmill. Physiological and biomechanical variables, including join…

Mechanisms, Economy, and Performance of Advanced Footwear Technology in Endurance Running-A Review.

Since the introduction of advanced footwear technology (AFT) in 2017, numerous world records from 5 km to the marathon have been broken. Among these innovations, carbon-plated shoes have received particular attention. Previous research indicates improvements of 2-4% in running economy (RE), which translates into an approximate 1-2% improvement in running performance when running in these shoes. The rapid progression of performance has generated significant scientific interest; however, a clear understanding of the mechanisms driving the effectiveness of AFT remains limited. Despite widespread …

Clustering Runners' Response to Different Midsole Stack Heights: A Field Study.

Advanced footwear technology featuring stack heights higher than 30 mm has been proven to improve running economy in elite and recreational runners. While it is understood that the physiological benefit is highly individual, the individual biomechanical response to different stack heights remains unclear. Thirty-one runners performed running trials with three different shoe conditions of 25 mm, 35 mm, and 45 mm stack height on an outdoor running course wearing a STRYD sensor. The STRYD running variables for each participant were normalized to the 25 mm shoe condition and used to cluster partic

Feet injuries in rock climbers.

While injuries of the upper extremity are widely discussed in rock climbers, reports about the lower extremity are rare. Nevertheless almost … autologous chondrocyte transplantation are given in this review. The chronic use of tight climbing shoes leads to overstrain injuries also. As the tight fit of the shoes changes the biomechanics of the foot an increased stress load is applied to the fore-foot. Thus chronic conditions as subungual hematoma, callosity and pain resolve. Also a high incide …

Foot and ankle injuries in the barefoot sports.

Playing sports barefoot has been contested since the very beginnings of athletic competition. Even today, some data suggest that shoes may limit the adaptive pronation that occurs after footstrike during running gait. This pronation likely protects runners from injury. Boardsport participants who perform their sports barefoot on the water seem to be at risk for foot and ankle injuries. The high-impact forces in gymnastics place participants at risk for foot and ankle injuries, as well. Swimming and diving have a low rate of foot and ankle injuries. The risk of ankle sprain in beach volleyball,…

Foot deformities in infants and children.

Foot deformities may reflect a generalized disorder, especially a neurologic problem; thus, the child should have a brief general examination. Many infantile foot deformities, such as calcaneovalgus, are postural and self-correcting. Metatarsus varus is not referred for treatment until age 2 months and then only if the deformity is moderate or severe. Fixed forefoot equinus and heel varus characterize a clubfoot, which requires immediate treatment. Corrective shoes are not advised as the primary treatment for metatarsus varus or clubfoot but often are prescribed to maintain the corrected posit…

Hallux valgus.

Hallux valgus is a complex deformity of the forefoot. Beside the lateral deviation of the first metatarsophalangeal joint angle exceeding 15-20 degrees and intermetatarsalvarus exceeding 8-9 degrees, it is also characterized by extensive changes of the soft tissue arch, sesamoid mechanism and metatarsocuneiform joint. It occurs almost exclusively in humans wearing shoes. Although in some measure it can be treated non-operatively, by corrective separators and inserts, operative treatment is predominant. There are numerous operative procedures aimed at the correction of the deformity and inducin…

Polydactyly of the foot.

Polydactyly may be preaxial or tibial (hallux-side), postaxial or ulnar (side of the little toe) and central (middle toes). The duplication may appear at the distal and medial phalanges or at the whole digit. The metatarsal bone may be part wise or completely duplicated, the accessory toes may share only one metatarsal. Surgical intervention may be indicated in shoe problems, for esthetic reasons or, especially in duplication of the metatarsales, because of secondary deviation of the toes and therefore shoe problems or plantar callosities. Preoperative analysis including x-ray is of great impo

Patient-Based Long-Term Results of the Surgical Treatment of Congenital Talipes Equinovarus.

UNLABELLED: Long-term results of surgical treatment for congenital talipes equinovarus are documented in the literature but mainly in correlation with clinical and radiographic objective criteria. We present a retrospective study of 48 patients who underwent surgical correction (extended Cincinnati type "a la carte" approach) in our department with an average of 17 years' follow-up. Patients were interviewed and completed a validated disease specific instrument, which evaluates foot function as well as patients' satisfaction, the use of orthotics, shoe size, and the impact of this operation in

Dermatitis after contact with Pheropsophus sp (Coleoptera, Carabidae, Brachininae) in the Pará State, Brazilian Amazon.

Dermatitis caused by bombardier beetles is rarely reported. Here, we report a case of beetle-caused dermatitis that occurred in Pará State, the Brazilian Amazon. A female patient while putting on her shoes, felt a burning pain in the right hallux and noticed a beetle inside the shoe. During medical consultation, the physician observed that the injury site was brown and discolored. The patient complained of localized burning pain and blurred vision. The beetle responsible for the injury was identified as Pheropsophus sp. Administered treatment included cleaning with saline, analgesics, and topi…

Management of neuropathic injury to the tarsal navicular.

The goal of treatment is to have a foot that will fit in a shoe or brace without likelihood of skin pressure and ultimate ulceration. Surgery is indicated in the acute and chronic phases of neuroarthropathy related to navicular fracture of dislocation. For patients who have acute injury with deformity that threatens the skin or cannot be maintained in a stable position without manipulation in a boot or cast, surgery with arthrodesis is recommended. Patients who subsequently develop deformities that cannot be managed successfully with extra-depth custom shoes or braces, and who are not candidat…

Is the One Ray Normal or Are the Rest Shorter?: Brachymetapody-A Case Report.

In this case report, we present the case of a 20-year-old male patient who suffered from pain in walking clinically, and in whom an extremely rare type of brachymetatarsia was diagnosed. Although distorted body image is the main reason for consulting a specialist, the patient presented because he was unable to find a proper shoe. The patient had no familial history of brachydactyly, trauma, or a genetic disorder. In this extremely rare case, the decision was made to perform shortening of the normal foot ray with a double osteotomy to the metatarsal and proximal phalanx. At the end of the follo

Bilateral recurrent discloation of the patella associated with below knee amputation: a case report.

BACKGROUND: Recurrent dislocation of the patella in patients with below knee amputation is a known entity. Abnormally high-riding patella (patella alta) and medial patellofemoral ligament insufficiency in these patients predisposes them to patellar instability. The established treatment of this problem is surgical realignment. CASE PRESENTATION: A 25 year old male patient with bilateral below knee amputation presented with bilateral recurrent dislocation of the patella while walking on knees on uneven ground. Clinical and radiographic studies showed patella alta. A simple shoe modification wa
