INTRODUCTION: About 4-5% of all fractures are localised to the proximal end of the humerus. During the last 30 years a two-time increase has been observed. The cause was ageing of population and a higher incidence of fractures. Treatment improved owing to proper classification based on radiological diagnostics.
OBJECTIVE: The principles of diagnostic and classification were presented on the basis of the literature and analysis of the clinical material.
MATERIAL: We analysed the ways of the diagnosis and classification of the proximal humerus fractures in 166 patients treated in Traumatologic

Anterior shoulder dislocations following an epileptic event are considered rare. An extremely rare case of a 41 year old female suffering from bilateral anterior shoulder dislocation with concomitant greater tuberosities fractures after an epileptic seizure is presented. The patient presented to the out-patient orthopaedic clinic due to persistent pain and restriction of shoulders movement, 4 weeks after an epileptic seizure. Clinical examination and radiological evaluation established the diagnosis of bilateral anterior shoulder dislocation with concomitant greater tuberosities fractures. Clo

Pathological fractures as a consequence of convulsive seizures without direct trauma occur in 0.3% of cases. Some lesions such as bilateral posterior dislocation of the shoulder, fracture-dislocation of the shoulder or fracture and/or dislocation of the hip strongly suggest a seizure as the aetiology. We report on a patient with simultaneous central dislocation of one hip with fracture of the contralateral femoral neck provoked by spontaneous seizures. Unrecognized associated injuries following a convulsion may result in long-term functional disability and legal consequences.

IntroductionClavicular fractures, particularly comminuted fractures, are common injuries that can affect shoulder function and upper limb mobility. The management of these fractures, especially using surgical approaches and fixation methods, remains a key area of clinical focus. Preserving the periosteum during surgery may improve fracture healing and reduce complications. This study explored the clinical efficacy of a periosteum-preserving surgical technique combined with a Nice knot-assisted locking plate for treating comminuted clavicular fractures.MethodsA total of 85 patients with comminu

OBJECTIVES: To evaluate the methods and the outcomes of complex intra-articular glenoid fractures, treated by open reduction and internal fixations.
METHODS: The outcomes of 11 cases of complex intra-articular glenoid scapular fractures were retrospectively analyzed. The fractures were classified as type IV in five cases, type Va in two and Vb in four cases, according to Ideberg classification system. The mean step or gap between the main articular fragments was 6.3 ± 6.2 (4-25) mm. The fractures were openly reduced through a Judet approach and fixed with reconstructive plates or bands placed

Imaging evaluation of shoulder disorders should begin with radiographs. Several radiographic projections have been developed to best show areas affected by particular clinical disorders. This paper reviews the radiographic examinations that are used at our hospital for evaluating arthritis, impingement, trauma and instability. The techniques used to obtain each of these radiographs are briefly described to better understand the resulting images. An organized approach to assessment of these radiographs is delineated including evaluation of the ABCs (alignment, bone density, cartilage spaces and

INTRODUCTION: in the elderly patient population, where fracture comminution, osteoporotic fractures, and associated arthritis or rotator cuff pathologies dominate, metadiaphyseal proximal humeral fracture is a challenging subset of fractures to treat. This study reports on cementless long-stem reverse total shoulder arthroplasty (RTSA) as primary treatment of metadiaphyseal proximal humeral fractures in elderly patients.
MATERIALS & METHODS: Between January 2018 and October 2021, 22 consecutive patients sustained proximal humerus fractures with metadiaphyseal extension and underwent surgery w

BACKGROUND: Brachial plexus injury (BPI) following shoulder trauma is an uncommon but potentially disabling complication that is often underrecognized, leading to delayed diagnosis. This study aimed to define the institutional incidence, nerve distribution, and recovery patterns of BPI after shoulder trauma and to identify factors associated with injury severity and recovery.
METHODS: We performed a retrospective review of adult patients diagnosed with BPI following isolated shoulder trauma at a single academic center from January 2016 to July 2023. Patients with BPI were compared with a rand

BACKGROUND: Decision making in the management of proximal humerus fractures can be difficult in situations in which the surgeon is uncertain of the ideal treatment.
METHODS: Two shoulder surgeons operatively treated 476 proximal humerus fractures from 1998-2014 with open reduction-internal fixation (ORIF), hemiarthroplasty, or reverse shoulder arthroplasty. Operative treatment was stratified by year to determine the evolution of technological influences on treatment over time. To evaluate the effect of uncertainty, 274 clinical vignettes were created for all patients with 1 year of follow-up

Diagnosing traumatic diaphragmatic rupture (TDR) due to penetrating rib fractures is challenging because the lesions are often too small to be detected and may present years after injury. Patients with delays in diagnosis of TDR rarely present with orthopaedic-related complaints of pain. We report the case of a 52-year-old female who presented with chronic left shoulder pain following a motor vehicle accident (MVA). In addition to left-side lower rib fractures, she also sustained a left-sided splenic laceration, pneumothorax, and two-part upper humerus fracture. Fracture treatment was percutan

BACKGROUND Coracoid process fractures are uncommon and are often complicated by many types of shoulder girdle injuries. Previous reports have shown that osteosynthesis for isolated coracoid process fractures results in favorable outcomes and high bone union rates. However, owing to the rarity of coracoid process fractures combined with distal clavicle fractures, there are limited data to guide treatment strategies. We report a case of symptomatic nonunion despite performing osteosynthesis for fractures of the coracoid process and distal clavicle. CASE REPORT A 60-year-old man presented with le

Fractures of the lesser tuberosity of the humerus are typically traumatic in nature, most commonly occurring in association with multi-part fractures of the humeral head, often in the setting of a posterior shoulder dislocation. Isolated fractures of the lesser tuberosity are considerably more rare and are difficult to diagnose on standard shoulder radiographs without an axillary view. These fractures have been associated with 3 main types of injury: acute abduction/external rotation injury, acute injury or repetitive stress injury in adolescent overhead or throwing athletes, and rarely as a s

BACKGROUND: Following blunt trauma, diagnosis of shoulder dislocation based on physical examination alone is difficult due to possible concurrent proximal humeral fractures. X-rays are therefore used to confirm diagnosis. Results from recent observational studies comparing diagnostic accuracy of point-of-care ultrasound (PoCUS) with X-rays for shoulder dislocation have been encouraging. The aim of this study was to determine whether PoCUS improves diagnostic accuracy when used with physical examination for the diagnosis of shoulder dislocation, proximal humeral fracture and ascertaining succes

Periprosthetic humeral fractures represent a relevant complication of shoulder arthroplasty and the incidence of 0.5-3% is expected to increase given current demographic trends. Biomechanical aspects are considered in therapeutic decision-making and planning, including fracture location and morphology as well as patient-specific factors. These include among others osteoporosis and age, but also other comorbidities of the patient, such as nicotine abuse or obesity. The fracture classification follows the internationally recognized system developed by Sanchez-Sotelo und Athwal. Conservative trea

BACKGROUND: In our department, axillary views of the shoulder in trauma patients are not performed on a routine base, but sometimes, they are ordered by the trauma surgeon. We usually perform an anteroposterior view of the shoulder, combined with a posterior and an anterior oblique view of the shoulder in trauma patients. Because the classical described axillary view of the shoulder is sometimes very painful for the patient, especially in patients with humeral fractures, we perform a less painful modified axillary view.
METHODS: We now perform the axillary view with the patient standing uprig
