[Obstructive Sleep Apnea Syndrome].

Obstructive Sleep Apnea Syndrome Abstract. The obstructive sleep apnea syndrome (OSAS) is a disorder of the control of breathing in which repetitive collapse of the upper airway during sleep leads to frequent apneas/hypopneas with cyclic oxygen desaturation and arousals. Sleep is fragmented and unrefreshing. The affected patients suffer from an increased tendency to fall asleep, impaired concentration and a reduced quality of life. The consequences of OSAS also include an increased risk of accidents caused by falling asleep as well as cardiovascular diseases. The diagnosis of OSAS is based on …

[Sleep and immune system].

Sleep is a process that occupies one third part of the life of the human being, and it is essential in order for the individual to be able to maintain body homeostasis. It emerges as an important regulator of the immune system since, during sleep, the necessary functions to maintain its balance are carried out. On the other hand, decreased sleep has deleterious effects that alter the metabolism and produce an increase in the secretion of C-reactive protein, interleukin (IL)-6 and tumor necrosis factor (TNF). These cytokines activate NF-κB; therefore, sleep disturbance can be a risk factor for …

REM Sleep Behavior Disorder.

Rapid eye movement sleep behavior disorder (RBD) is a brain disorder, characterized by the dream enactment during rapid eye movement (REM) sleep due to a lack of physiologic muscle atonia and increased muscle twitching. Schenk was the first to describe this disorder in 1986; however, few authors reported in the 1970-1980s loss of physiological muscle atonia combined with dream enactment in the course of brainstem disorders and as a consequence of alcoholism and antidepressant treatment. RBD affects less than 1% of the adult population, but can be found in up to 25-50% of neurodegenerative diso…

Sleep and Parkinson Disease.

Sleep disorders are prevalent in Parkinson disease (PD), a disease with well recognized motor dysfunction. Sleep related problems received little attention until the last three decades. Sleep disorders seen in PD patients include insomnia, excessive sleepiness, restless legs syndrome, REM sleep behavior disorder. Some of these can have significant impact and lower the quality of life in these patients. An understanding of sleep issues in PD can help identify them early and result in optimal management.

[Central sleep apnoea syndrome].

In central sleep apnoea syndrome (SAS), there is a dysfunction in central ventilatory command. Patients with central SAS can exhibit symptoms and complications linked to unstable sleep caused by apnoea and certain specific mechanisms (chronic and intermittent hypoxia). Central SAS is linked to several diseases which should be investigated before treatment. Treatment depends on underlying conditions and should focus on alleviating symptoms. Possible treatments include nocturnal non invasive ventilation and/or oxygen, and oral medications. Chronic heart failure with LVEF<45% is a contraindicatio…

[Sleep bruxism in children].

Bruxism is defined as repetitive activity of the masticatory muscles, characterized by clenching the teeth or teeth grinding and/or by tapping and swaying. This study investigated sleep bruxism. The etiology is multifactorial: mainly central (neuropathic disorder, anxiety) but also genetic and local (posture, mouth breathing). The diagnosis is based primarily on the anamnesis and examination of dental wear and progression over time (photos and dental castings). A diagnostic guide is proposed in this article. Frequently found in children, bruxism is not always considered pathological. The sever…

Sleep and Eating Disorders.

Insomnia is related to an increased risk of eating disorders, while eating disorders are related to more disrupted sleep. Insomnia is also linked to poorer treatment outcomes for eating disorders. However, over the last decade, studies examining sleep and eating disorders have relied on surveys, with no objective measures of sleep for anorexia nervosa or bulimia nervosa, and only actigraphy data for binge eating disorder. Sleep disturbance is better defined for night eating syndrome, where sleep efficiency is reduced and melatonin release is delayed. Studies that include objectively measured s…

[Sleep apnea in pregnancy].

Snoring, chronic rhinitis and fragmentation of sleep are common symptoms in pregnancy, especially in the third trimester. The prevalence of obstructive sleep apnea is also increased in pregnant women and can be associated with pregnancy induced hypertension, preeclampsia and diabetes mellitus. The fetus is as well exposed to a higher risk of prematurity and intra-uterine growth restriction. Given the lack of adequate screening tool, the diagnosis of obstructive sleep apnea is a challenge for the physician and a high degree of clinical suspicion is necessary in order to identify the patients at…

Obstructive sleep apnoea syndrome.

Obstructive sleep apnoea syndrome (OSAS) is a common clinical condition in which the throat narrows or collapses repeatedly during sleep, causing obstructive sleep apnoea events. The syndrome is particularly prevalent in middle-aged and older adults. The mechanism by which the upper airway collapses is not fully understood but is multifactorial and includes obesity, craniofacial changes, alteration in upper airway muscle function, pharyngeal neuropathy and fluid shift towards the neck. The direct consequences of the collapse are intermittent hypoxia and hypercapnia, recurrent arousals and incr…

Unihemispheric sleep in crocodilians?

Reduced vigilance is the conspicuous cost of sleep in most animals. To mitigate against this cost, some birds and aquatic mammals have evolved the ability to sleep with one-half of their brain at a time, a phenomenon known as unihemispheric sleep. During unihemispheric sleep the eye neurologically connected to the 'awake' hemisphere remains open while the other eye is closed. Such unilateral eye closure (UEC) has been observed across avian and non-avian reptiles, but has received little attention in the latter. Here, we explored the use of UEC in juvenile saltwater crocodiles (1) under baselin…

Sleep and Heart Failure.

Sleep deprivation occurs for many reasons but, when chronic in nature, has many consequences for optimal health and performance. Despite its high prevalence, sleep-disordered breathing is underrecognized and undertreated. This is especially true in the setting of heart failure, where sleep-disordered breathing affects more than 50% of patients. Although the optimal strategy to best identify patients is currently unknown, concerted and consistent efforts to support early recognition, diagnosis, and subsequent treatment should be encouraged. Optimization of guideline-directed medical therapy and…

Stress, arousal, and sleep.

Stress is considered to be an important cause of disrupted sleep and insomnia. However, controlled and experimental studies in rodents indicate that effects of stress on sleep-wake regulation are complex and may strongly depend on the nature of the stressor. While most stressors are associated with at least a brief period of arousal and wakefulness, the subsequent amount and architecture of recovery sleep can vary dramatically across conditions even though classical markers of acute stress such as corticosterone are virtually the same. Sleep after stress appears to be highly influenced by situ…

Genetics of Sleep Disorders.

Sleep disorders are, in part, attributable to genetic variability across individuals. There has been considerable progress in understanding the role of genes for some sleep disorders, such as the identification of a human leukocyte antigen gene for narcolepsy. For other sleep disorders, such as insomnia, little work has been done. Optimizing phenotyping strategies is critical, as is the case for sleep apnea, for which intermediate traits such as obesity and craniofacial features may prove to be more tractable for genetic studies. Rapid advances in genotyping and statistical genetics are likely…

[Sleep apnoea and glaucoma].

Several lines of evidence suggest an association between (normal-tension) glaucoma and sleep apnoea syndrome (SAS). It is hypothesised that the glaucomatous damage may result from impaired perfusion of the optic nerve head secondary to repetitive prolonged apnoeas. Therefore, screening for SAS in suspectible glaucoma patients is recommended.

[Obstructive sleep apnoea syndrome].

Obstructive sleep apnoea (OSA) and other sleep disordered breathing are common disorders causing significant morbidity, mortality and societal burden. A significant proportion of the patients are undiagnosed and consequently untreated. Due to the wide disease distribution we recommend increased disease awareness, especially among high-risk groups: patients with obesity, metabolic syndrome and cardio- and cerebrovascular diseases. Continuous positive airway pressure (CPAP) is the first-line of treatment together with weight reduction, whereas oral devices may be used for less severe OSA or in c…

Pharmacotherapy for sleep bruxism.

BACKGROUND: Sleep bruxism is an oral activity characterized by involuntary teeth grinding or clenching during sleep. Several forms of treatment have been proposed for this disorder, including behavioural, dental and pharmacological strategies. OBJECTIVES: To evaluate the effectiveness and safety of pharmacological therapy for the treatment of sleep bruxism compared with other drugs, no treatment or placebo. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (Issue 8, 2014), MEDLINE (1966 to August 2014), EMBASE (1980 to August 2013) and LILACS (1982 to A…

Sleep and Alzheimer's disease.

Sleep disorders are frequent in Alzheimer's disease (AD), with a significant impact on patients and caregivers and a major risk factor for early institutionalization. Micro-architectural sleep alterations, nocturnal sleep fragmentation, decrease in nocturnal sleep duration, diurnal napping and even inversion of the sleep-wake cycle are the main disorders observed in patients with AD. Experimental and epidemiological evidence for a close reciprocal interaction between cognitive decline and sleep alterations is growing. Management of sleep disorders in AD is pre-eminently behavioral. Association…

[Sleep disorders and epilepsy].

It has been reported that patients with epilepsy often have insomnia and/or daytime sleepiness; the symptomatologic features differ in seizure types. Not only the administration of anti-epileptics, but also inappropriate sleep hygiene cause daytime sleepiness. In subjective assessment of sleepiness, we need to pay attention if it can correctly assess or not. The prevalence of obstructive sleep apnea in patients with epilepsy is approximately 10-30%. Sleep apnea deteriorates the seizure control because of worsen sleep condition by sleep apnea, especially in elderly patients. Some researchers re…
