Mechanisms and Management of Obstructive Sleep Apnea: A Translational Overview.

Obstructive sleep apnea (OSA) is a common disorder characterized by recurrent episodes of upper airway obstruction during sleep, resulting in apnea and hypopnea. While anatomical factors, such as increased upper airway collapsibility, significantly contribute to OSA, emerging evidence highlights the importance of non-anatomical mechanisms, including impaired pharyngeal muscle responsiveness, low arousal threshold, and elevated ventilatory loop gain. The interplay of these factors leads to respiratory instability, fragmented sleep, and systemic complications. Additional pathophysiological influ

Chin Tuck Exercise and Obstructive Sleep Apnea: A Case Report.

This case report describes a patient with neck pain who was recommended by his gym coach to perform chin tuck exercises to manage his pain. After a period of 3 weeks of daily chin tuck exercise, each session consisting of 10 repetitions of a 20-second hold, performed three times per day, the patient started to experience symptoms of snoring, witnessed gasping, morning headache, and excessive daytime sleepiness. Polysomnography performed by a sleep physician confirmed the diagnosis of mild obstructive sleep apnea with an apnea-hypopnea index of 10 times/h and a significant desaturation of oxyge

Adenotonsillar Microbiome Shifts in Children With Obstructive Sleep Apnea Syndrome.

OBJECTIVE: To characterize the microbiomes of adenoids and tonsils in pediatric Obstructive Sleep Apnea Syndrome (OSAS) patients compared to healthy controls. We hypothesized that the microbiome composition of the tonsils and adenoids in OSAS patients differs significantly from that of healthy controls. METHODS: Thirty OSAS patients and 30 healthy controls were included. Samples from adenoids and tonsils were analyzed using 16S rRNA amplicon sequencing to characterize bacterial communities. Differential abundance and alpha and beta diversity were used to compare the microbiome compositions be

[Optimal treatment of obstructive sleep apnea guided by physiological characteristics].

Obstructive sleep apnea (OSA) is a common and heterogeneous disorder. Most patients can be successfully treated, but some have poor treatment outcomes. This review first presented four physiological OSA characteristics, including upper airway collapsibility, loop gain, airway muscle responsiveness, and arousal threshold, which are associated with the efficacy of OSA therapy. Then, we presented how to select the most effective therapy modality for each individual based on OSA characteristics, with the purpose of providing a basis for individualized treatment of OSA.

Screening moderate to severe obstructive sleep apnea with wearable device.

BACKGROUND: Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder usually diagnosed through polysomnography (PSG). Moderate to severe OSA can significantly increase morbidity and mortality. Existing screening tools have limited accuracy. This study aimed to evaluate the Wellue O2 ring, a commercial pulse oximeter ring, for screening moderate to severe OSA. METHODS: A cross-sectional study included adults aged 18 and older suspected of having OSA who underwent PSG while wearing the Wellue O2 ring on their thumb. The oxygen desaturation index (ODI) from both the O

Symptom subtype progression in obstructive sleep apnea over 5 years.

STUDY OBJECTIVES: There is limited knowledge regarding the progression or consistency of symptoms in obstructive sleep apnea (OSA) over time. Our objective was to examine the changes in symptom subtypes and identify predictors over a span of 5 years. METHODS: Data of 2,643 participants of the Sleep Heart Health Study with complete baseline and 5-year follow-up visits were analyzed. Latent class analysis on 14 symptoms at baseline and follow-up determined symptom subtypes. Individuals without OSA (apnea-hypopnea index < 5) were incorporated as a known class at each time point. Decreased structural pathways mediating functional connectivity in obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a common sleep breathing disorder that is often accompanied by changes in structural connectivity (SC) and functional connectivity (FC). However, the current understanding of the interaction between SC and FC in OSA is still limited. METHODS: The aim of this study is to integrate complementary neuroimaging modalities into a unified framework using multi-layer network analysis methods and to reveal their complex interrelationships. We introduce a new graph metric called SC-FC bandwidth, which measures the throughput of SC mediating FC in a multi-lay

Oral appliance influence on jaw function in obstructive sleep apnea.

INTRODUCTION: Oral appliance (OA) therapy in obstructive sleep apnea (OSA) could be a risk factor for normal jaw function, given the prolonged effect of an OA in keeping the mandible in a protruded position away from a normal position. This study aimed to assess changes in symptoms and clinical findings related to jaw function after 1 year of treating OSA with an OA. METHODS: In this follow-up clinical trial, 302 patients with OSA were assigned to treatment with either monobloc or bibloc OA. Baseline and 1-year follow-up assessment included using the Jaw Functional Limitation Scale, self-repo

Obstructive sleep apnea and nocturnal attacks of paroxysmal atrial fibrillation.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is commonly seen in patients with atrial fibrillation (AF), but it is unclear to what extent this relationship is one of causation or association. We examined a cohort of patients with paroxysmal AF to determine whether the presence of OSA (apnea-hypopnea index ≥ 15 events/h) affects the time of onset of symptomatic AF episodes. METHODS: Patients with a recent emergency department visit for a symptomatic episode of paroxysmal AF were recruited from an AF clinic. The time of onset of the AF attack was classified as occurring in "sleeping hours" o

Management of Obstructive Sleep Apnea in Patients With Heart Failure.

Sleep apnea is traditionally classified as obstructive sleep apnea (OSA), which occurs when the upper airway collapses due to the relaxation of oropharyngeal musculature, and central sleep apnea occurs when the brainstem cannot stimulate breathing. Most sleep apnea in patients with heart failure (HF) results from coexisting OSA and central sleep apnea (CSA), or complex sleep apnea syndrome. OSA and CSA are common in HF and can be involved in its progression by exposure to the heart to intermittent hypoxia, increased preload and afterload, activating sympathetic, and decreased vascular endothel
