In 2010 the European Medicines Agency withdrew the indication of modafinil for the treatment of obstructive sleep apnea, shift work sleep disorder and for idiopathic hypersomnia (IH). In uncontrolled studies, modafinil has been reported to be efficacious in the treatment of sleep disorders. We therefore performed a randomized, placebo-controlled study with the aim of proving the efficacy of modafinil treatment in these patients. Drug-free IH patients without long sleep according to ICSD2 criteria, age >18 years and disease duration >2 years were included. After a washout phase, patients at bas

Sleep disorders have become a significant health concern in modern society. To investigate and diagnose sleep disorders, sleep analysis has emerged as the primary research method. Conventional polysomnography primarily relies on cerebral electroencephalography (EEG) and electromyography (EMG) for sleep stage scoring, but manual scoring is time-consuming and subjective. This study investigated the potential application of cerebellar EEG combined with machine learning in automatic sleep stage classification. Twenty-five male mice underwent 24-h cerebral EEG/cerebellar EEG/EMG recording, and manu

REM sleep behaviour disorder (RBD) is a REM sleep-related parasomnia which may be considered a "dissociated state of wakefulness and sleep", given that conflicting elements of REM sleep (dreaming) and of wakefulness (sustained muscle tone and movements) coexist during the episodes, leading to motor and behavioural manifestations reminiscent of an enacted dream. RBD has been reported in association with α-synucleinopathies: around a third of patients with Parkinson's disease (PD) have full-blown RBD. Recent data indicate that PD patients with RBD are more prone to hallucinations than PD patient

Having nightmares can contribute to poor sleep quality, and having sleep problems can be accompanied by increased nightmare frequency. The etiological models of insomnia and nightmare disorder also show some parallels. The present cross-sectional study included 654 participants (509 women, 144 men, 1 non-binary) with mild to moderate sleep problems (mean age: 41.19 ± 12.95 years). As expected, nightmare frequency was elevated compared to previously published findings of a representative sample (large effect size). Moreover, waking-life stress level moderated-at least partially-the relationship

INTRODUCTION: Narcolepsy is a debilitating sleep disorder characterized by excessive daytime sleepiness, cataplexy and intrusive REM sleep. Deficits in endogenous orexins are a major pathogenic component of the disease. This disorder is also associated with the gene marker, HLADQB1*0602. An increased prevalence of sleep apnea in narcolepsy suggested interactions among ventilatory chemosensitivity, narcolepsy-cataplexy, and sleep apnea.
RESULTS: Evidence from animal studies using orexin knockout mice and focal microdialysis of an orexin receptor antagonist demostrated that orexins are also con

STUDY OBJECTIVES: Sleep state misperception is common in various sleep disorders, especially in chronic insomnia with a prevalence ranging between 9-50%. Most prior studies used nocturnal polysomnography (PSG) for the identification of sleep state misperception during nighttime. Our objective was to assess sleep state misperception during daytime in people with sleep disorders with excessive daytime sleepiness (EDS).
METHODS: In this prospective observational study, we assessed the occurrence of, and factors influencing sleep state misperception in consecutive patients undergoing a routine mu

OBJECTIVE: Insomnia and disrupted sleep are associated with increased risk of suicide. The N-methyl-d-aspartate antagonist ketamine has been associated with reduced suicidal thoughts, but the mechanism of action is unknown. This study sought to evaluate differences in nocturnal wakefulness in depressed individuals who did and did not have an antisuicidal response to ketamine.
METHODS: Thirty-four participants with baseline suicidal ideation diagnosed with either DSM-IV major depressive disorder (n = 23) or bipolar depression (n = 11) between 2006 and 2013 completed nighttime electroencephalog

The aim of this study was to investigate sleep architecture in stroke patients, and correlate possible disturbances with the topography, severity and outcome of stroke and the presence of sleep-disordered breathing (SDB). In total, 62 acute stroke patients and 16 age- and gender-matched hospitalised controls underwent polysomnographic studies. Sleep architecture was analysed according to the topography of lesion, severity (National Institutes of Health Stroke Scale) and outcome (Barthel Index) of stroke. We found that sleep architecture is disturbed in stroke patients, regardless of SDB. Strok

Physiological hyperarousal is manifested acutely by increased heart rate, decreased respiratory sinus arrhythmia, and increased skin conductance level and variability. Yet it is uncertain to what extent such activation occurs with the symptomatic hyperarousal of posttraumatic stress disorder (PTSD). We compared 56 male veterans with current PTSD to 54 males who never had PTSD. Subjects wore ambulatory devices that recorded electrocardiograms, finger skin conductance, and wrist movement while in their normal environments. Wrist movement was monitored to estimate sleep and waking periods. Heart

INTRODUCTION: The Odds Ratio Product (ORP) is a validated EEG-based measure of sleep depth, more sensitive than traditional metrics. While it has been studied in healthy individuals and those with sleep-wake disorders, its relevance in psychiatric conditions remains unclear. This study examined ORP during sleep and its association with depressive symptoms in a large cohort referred to multiple U.S. sleep centers.
METHODS: We retrospectively analyzed data from 829 adults (48.85% female; mean age 43.49 ± 13.74 years) enrolled in two multicenter studies. Each participant completed the Patient He

OBJECTIVE: Previous neuroimaging studies have shown subtle structural changes of the brain in various sleep disorders, suggesting detrimental effects of disrupted sleep-wake cycle on brain structures. We aimed to identify structural changes in shift workers relative to day workers. We hypothesized that brain structures belonging to sleep-wake modulation may be altered in shift workers.
METHODS: Nineteen male shift workers (median age, 21 years) and 19 male day workers (median age, 21 years) voluntarily participated in the current study. Sleep questionnaires were obtained from all participants

Modulation of TASK-3 (Kcnk9) potassium channels affect neurotransmitter release in thalamocortical centers and other sleep-related nuclei having the capacity to regulate arousal cycles and REM sleep changes associated with mood disorders and antidepressant action. Circumstantial evidence from this and previous studies suggest the potential for TASK-3 to be a novel antidepressant therapeutic target; TASK-3 knock-out mice display augmented circadian amplitude and exhibit sleep architecture characterized by suppressed REM activity. Detailed analysis of locomotor activity indicates that the amplit

BACKGROUND: Sleep disturbances are highly prevalent in movement disorders, potentially due to the malfunctioning of basal ganglia structures. Pallidal deep brain stimulation (DBS) has been widely used for multiple movement disorders and been reported to improve sleep. We aimed to investigate the oscillatory pattern of pallidum during sleep and explore whether pallidal activities can be utilized to differentiate sleep stages, which could pave the way for sleep-aware adaptive DBS.
METHODS: We directly recorded over 500 h of pallidal local field potentials during sleep from 39 subjects with move

p-Chloro-m-xylenol (PCMX) is a widely used broad-spectrum antimicrobial agent. Its extensive production, utilization and discharge have led to frequent detection in aquatic environments and biological samples, posing potential ecological risks. However, its toxic effects on the visual system and visual-related behaviors of aquatic organisms remain poorly understood. Here, zebrafish larvae were exposed to 0.4, 1.2, 2 and 2.8 mg/L PCMX to investigate impacts on ocular morphology, retinal development, visual-mediated behaviors and circadian rhythms. The results demonstrated that PCMX at ≥0.4 mg/L

Parkinson's disease (PD) is characterized by abnormal beta oscillations (13-30 Hz) within the basal ganglia, which contribute not only to motor symptoms but also to sleep disturbances. In this study, we developed a computational model of the basal ganglia-thalamocortical (BGTC) network that includes the pedunculopontine nucleus (PPN), to investigate the mechanism by which the abnormal oscillations disrupt sleep. The model incorporates key nuclei, neurotransmitter systems, and neural pathways to simulate sleep-related brain activity in Parkinsonian conditions. Our simulations show that abnormal

BACKGROUND: Pediatric sleep disturbances are regularly diagnosed on the basis of parental reports. However, the impact of parental sleeping problems on parental perceptions and reports of their child's sleep has not yet been studied. We hypothesized that poor parental sleep decreases the parent-reported child sleep quality.
METHODS: A 1-week actigraph recording was performed in 100 children aged 2 to 6 years recruited in 16 day care centers. Their biological parents completed a sleep diary and a Sleep Disturbance Scale for Children (SDSC) on children's sleep. The parents also completed the Je
