Soft-tissue infections of ampicillin-resistant Haemophilus influenzae type b. The use of ampicillin and nafcillin in their treatment.

The combination of intravenous ampicillin sodium and nafcillin sodium followed by oral ampicillin and dicloxacillin sodium was effective in treating five infants with soft-tissue infections and bacteremia due to beta-lactamase-producing, ampicillin-resistant Haemophilus influenzae type b. This antibiotic combination seems to be an effective alternative to chloramphenicol for treatment of non-meningitic invasive disease due to moderately ampicillin-resistant (minimum inhibitory concentration, 2 to 24 microgram/mL) H influenzae type b. The use of this combination should be in conjunction with ap

S2k guidelines for skin and soft tissue infections Excerpts from the S2k guidelines for "calculated initial parenteral treatment of bacterial infections in adults - update 2018".

These first German S2k guidelines for bacterial skin and soft tissue infections were developed as one chapter of the recommendations for "calculated initial parenteral treatment of bacterial infections" issued under the auspices of the Paul-Ehrlich Society, of which the main part is presented here. Well-calculated antibiotic therapies require precise diagnostic criteria. Erysipelas is defined as non-purulent infection considered to be caused by beta-hemolytic strepto-cocci. It is diagnosed clinically by its bright-red erythema and early fever or chills at disease onset. Penicillin is the treat…

Hoffmann osteotaxis for tibial fractures: soft-tissue complications, infection and union in a historical Belgian cohort.

; selbanna@skynet.be Hoffmann osteotaxis is part of Belgian tibial fracture history. We describe an archival multicentre cohort and examine associations between soft-tissue category, secondary skin necrosis, historically recorded local infection and union outcomes. Aggregate tables from a publicly deposited 1980 Université libre de Bruxelles memoir were transcribed and checked against source totals. The unit of analysis was the fracture. The cohort included 1,147 fresh, initially non-infected tibial fractures treated with Hoffmann external fixation in seven collaborating centres: 890 men and 2

A Bite to Remember: A Case Report of Necrotizing Soft-Tissue Infection of the Forearm Caused by Prevotella intermedia.

INTRODUCTION: Human bite wounds account for a small proportion of bite injuries but carry a high risk of infection due to polymicrobial inoculation. While most infections present as cellulitis or abscess, necrotizing soft-tissue infections (NSTIs) are rare and potentially fatal. CASE REPORT: We present the case of a 72-year-old female who developed an NSTI of the forearm after an occlusal human bite. Despite empiric antibiotics, the infection progressed rapidly, and intraoperative cultures grew Prevotella intermedia, an uncommon anaerobe in NSTIs. She required urgent fasciotomy and debridemen

Ascending complex anal fistula secondary to lower extremity soft tissue infection: a case report.

BACKGROUND: Complex anal fistulas remain a surgical challenge due to their intricate anatomy, high recurrence rates, and potential for sphincter injury. We report a rare case of complex anal fistula with multiple external openings extending from the gluteal region to the thigh root, successfully managed by combined incision with drainage tube placement. CASE PRESENTATION: A 57-year-old man presented with recurrent purulent discharge from the perianal region and left lower limb for three months following debridement of a lower extremity soft tissue infection. Physical examination revealed mult

Case Report: Pyoderma gangrenosum masquerading as skin and soft tissue infection preceding myelodysplastic syndrome.

Pyoderma gangrenosum (PG) is a rare, complex, and severe inflammatory dermatosis categorized under the spectrum of neutrophilic dermatoses (NDs), pathologically characterized by sterile neutrophilic infiltration. It is frequently associated with systemic diseases and typically manifests concurrently with or after their diagnosis. We report a rare PG case of an elderly woman who presented 5 months prior to the onset of hematological malignancy (HM), and this unusual clinical presentation led to a misdiagnosis. The patient was initially suspected of having a lower-extremity skin and soft tissue

Perforated colon cancer complicated by necrotizing soft tissue infection: A case report and literature review.

Necrotizing soft tissue infections (NSTIs) are rare but life-threatening complications of colorectal cancer, often overlooked during patient diagnosis and treatment. NSTIs progress rapidly, leading to significant tissue damage and elevated mortality rates. This report presents the case of a 71-year-old male who presented with abdominal pain and distention, later diagnosed with perforated colon cancer and NSTI of the left abdominal wall. Following admission, the patient developed septic shock and acute kidney injury. Emergency surgery was performed, including radical resection of the colon canc…

Travel-related sepsis with colitis and necrotizing skin and soft tissue infection after oysters consumption.

We describe in this article a case of a 55-year-old woman admitted to our hospital in France, for oedema and paraesthesia of the lower limbs after returning from a trip to Florida. In only four hours, the patient's condition evolved into a multiple organs failure, leading the physicians to carry out a TAP scan and a CT angiogram of the lower limb, which showed acute pan-colitis and diffuse infiltration of the skin and soft legs tissues. Leg fasciotomy was performed and revealed necrotizing fasciitis and myonecrosis. The evolution was unfavorable, leading to shock and death 12 h after hospital

Necrotizing Soft Tissue Infection Secondary to Spinal Hardware Malfunction: A Case Report on Surgical Debridement.

Necrotizing soft tissue infections (NSTIs) are characterized by "flesh-eating bacteria" that are rapidly progressive and require immediate diagnosis and aggressive surgical debridement along with broad-spectrum antibiotics. Common pathogens, such as Streptococcus pyogenes, Staphylococcus aureus, Enterococcus coli, and Clostridium, are the usual culprits of this disease. While NSTIs have a relatively low incidence, they are associated with higher rates of morbidity and mortality. Due to the aggressive nature of the pathogen, NSTI can cause widespread necrosis of soft tissue and muscle, leading …

A Report of Two Uncommon Cases of Mycobacterium chelonae with Localized and Disseminated Skin and Soft Tissue Infection.

Background: Mycobacterium chelonae is a ubiquitous, rapidly growing, nontuberculous mycobacteria that primarily affects immunocompromised patients. The most common presentation is an atypical, chronic skin and soft tissue infection. Due to its high resistance rate, early diagnosis based on clinical suspicion, risk factor assessment, and exposure history is crucial for initiating appropriate multi-drug treatment. Methods: We report two cases of M. chelonae skin and soft tissue infections, one presenting with localized disease and the other with disseminated involvement. One case had a specific …

Marjolin's ulcer in an ischial pressure sore presented with necrotizing soft tissue infection: A case report.

RATIONALE: Marjolin's ulcer (MU) is a rare skin malignancy derived from a chronic wound. Pressure ulcer related MU possesses poor prognosis and high metastatic rate, and it is difficult to be differentiated, especially when superimposed infection. PATIENT CONCERNS: Here we report a case with pressure ulcer related MU which presented as necrotizing soft tissue infection (NSTI) to demonstrate the manifestation, treatment, and prognosis of this rare disease. DIAGNOSES: A 45-year-old male patient had spinal cord injury at age 2 years. He presented ischial pressure sore complicated with NSTI init

Perforated Appendicitis Presenting as a Soft Tissue Infection of the Thigh: An Example of Successful Non-Operative Management.

Perforated appendicitis is a rare but serious clinical scenario typically requiring urgent surgical intervention. Herein, we discuss the case of a 62-year-old woman with COVID-19 and ruptured retrocecal appendicitis presenting as a right lower extremity soft tissue infection that was successfully managed using non-operative measures. This unique case illustrates the feasibility of conservative care - rather than urgent surgical intervention - in the treatment of an atypical presentation of complicated appendicitis in a high-risk patient.

Outcomes and Factors Associated with Occult Septic Shock in Emergency Department Patients with Soft Tissue Infection.

INTRODUCTION: Patients who develop occult septic shock (OSS) are associated with worse outcomes than those with early septic shock (ESS). Patients with skin and soft tissue infection (SSTI) may have underlying organ dysfunction due to OSS, yet the prevalence and the outcomes of patients with SSTI and early versus occult shock have not been described. This study compared the clinical characteristics of SSTI patients and the prevalence of having no septic shock (NSS), ESS, or OSS. METHODS: We retrospectively analyzed charts of adult patients who were transferred from any emergency department to

Murine Soft Tissue Infection Model to Study Group A Streptococcus (GAS) Pathogenesis in Necrotizing Fasciitis.

Group A streptococcus (GAS) necrotizing fasciitis (NF) causes high morbidity and mortality despite prompt intravenous administration of antibiotics, surgical soft-tissue debridement, and supportive treatment in the intensive care unit. Since there is no effective vaccine against GAS infections, a comprehensive understanding of NF pathogenesis is required to design more efficient treatments. To increase our understanding of NF pathogenesis, we need a reliable animal model that mirrors, at least in part, the infectious process in humans. This chapter describes a reliable murine model of human NF

Necrotizing soft tissue infection of the upper leg as first presentation of necrotizing pancreatitis: a case report.

Acute pancreatitis can be complicated with necrosis of the pancreatic or peripancreatic tissue. This necrosis can become liquified and form a well-defined wall (walled-off necrosis or WON) and can become infected and form abscesses. Necrotizing soft tissue infections are rare infections of the deep tissue and subcutaneous fat and are mostly caused by trauma or perforated visceral organs. They can, however, rarely be caused by infected retroperitoneal collections. To date only 3 case reports have been published of a necrotizing soft tissue infection complicating a necrotizing pancreatitis. Both…
