Functional anatomy of the paranasal sinuses.

Paranasal sinus anatomy possesses great complexity and is rather variable from person to person. Significant differences in structure may also exist between the two sides of the same person. Therefore, a detailed knowledge of the anatomy of the sinuses is critical in performing procedures such as functional endoscopic sinus surgery.

Mucormycosis of the sphenoid sinus in an otherwise healthy patient. Case report and literature review.

Paranasal and rhinocerebral mucormycosis refers to uncommon opportunistic fungal infections, reported to occur especially in association with diabetic acidosis (the most common), immunosuppressive therapy, malignancy, or other chronic debilitating disorders. However, patients who have no underlying disease have occasionally been affected. According to the literature reviewed, only 13 well-documented cases without any predisposing factor have been previously reported. We describe a unique case of sphenoidal mucormycosis in an otherwise healthy individual, and the first patient to present with h

Loss of vision secondary to a sphenoid fungal ball with full post-surgical recovery.

An older adolescent immunocompetent male with a 2 month history of progressive frontal headaches, congestion and worsening vision, culminating in right optic neuropathy, was referred to our emergency department by ophthalmology. MRI revealed extensive sinus disease, though without direct optic nerve compression; CT scan identified a fungal ball extending into the right clinoid process, with bony erosion and involvement of the deep pneumatised optico-carotid recess. The patient underwent emergency endoscopic sinus surgery, where mucopus, polyps and fungal debris consistent with an infected fung

Aspergillus-associated cerebral aneurysm successfully treated by endovascular and surgical intervention with voriconazole in lupus nephritis patient.

During the last five decades, long-term therapy with immunosuppressive agents such as pulse cyclophosphamide in conjunction with high-dose corticosteroids has enhanced both patient survival and renal survival in patients with diffuse proliferative lupus nephritis. Nevertheless, severe side effects such as infectious complications remain the main cause of morbidity and mortality. Central nervous system aspergillosis is uncommon but life-threatening in lupus patients. In this single-patient case study, carotid aneurysm with sphenoidal sinusitis was suspected when severe epistaxis occurred during

The Use of 0° and 70° Endoscopes in Maxillary Antrostomy.

BACKGROUND AND OBJECTIVES: Performing an effective maxillary antrostomy is critical to improving chronic maxillary sinusitis symptomatology. Incomplete dissection of the uncinate process and failure to incorporate the natural drainage pathway may lead to recirculation and need for revision surgery. The purpose of this study is to determine if 70° endoscopes provide added value in determining incomplete dissection or residual disease. METHODS: Prospective study of 35 sinuses from 18 patients undergoing FESS for Chronic Rhinosinusitis (CRS) between 11/1/2020 and 4/30/2021. Two fellowship traine

Computed Tomography Evaluation of Unilateral Chronic Maxillary Sinusitis With Osteitis.

BACKGROUND: Computed tomography (CT) is the preferred noninvasive method for the evaluation of osteitis in chronic sinusitis. Some scholars believe that the bone changes associated with chronic sinusitis always show high attenuation (high density) and are positively correlated with the severity of the disease. However, sinus bone remodeling is a complex process that may cause high or low attenuation. There have been few reports on the spread of osteitis. Therefore, additional research on sinus CT is necessary. OBJECTIVE: To observe bony changes in chronic maxillary sinusitis (CMS) by CT and r

Suppurative intracranial complications of sinusitis in children.

OBJECTIVE: A review of suppurative intracranial complications of sinusitis in children. METHODS: Case series review over a two-year period from 1998 to 1999 in a children's hospital, Singapore. RESULTS: There were seven cases, all male, and age range 9 to 14. Six had subdural empyemas and one had meningitis. The most common presenting symptoms included fever, headache and vomiting. Sinusitis was suspected as the cause in only one patient initially. The intracranial infections were not apparent from the initial brain CT of two patients and were only confirmed later on repeated imaging. Four p

Deadly Sphenoid Fungus-Isolated Sphenoid Invasive Fungal Rhinosinusitis: A Case Report.

INTRODUCTION: Acute invasive fungal rhinosinusitis (AIFRS) is a potentially fatal infection, usually affecting immunocompromised patients. Isolated sphenoid sinus involvement is rare and has been reported in only a few cases. We discuss the clinical characteristics, histopathologic features, and differential diagnosis of AIFRS of the sphenoid sinus. CASE PRESENTATION: A 57-year-old man with a history of refractory non-Hodgkin lymphoma and neutropenia presented with a 1-week duration of left-sided headache and ipsilateral cheek paresthesia. Nasal endoscopy showed mucoid drainage from the sphen

Nasal septal abscess complicating isolated acute sphenoiditis: case report and literature review.

This report presents an extremely rare case of nasal septal abscess complicating acute sphenoiditis in a non-immunocompromised adult patient. A 56-year-old woman came to our emergency service with a 2 wk history of nasal obstruction, headache, and facial pain. A nontraumatic nasal septal abscess complicating acute isolated sphenoiditis was diagnosed. Under general anaesthesia, we drained the septal abscess and performed an endoscopic transnasal sphenoidotomy. Bacteriological cultures revealed viridans streptococci in the septal abscess and sphenoid cavity. We discuss the patient's diagnosis, p

Eosinophilic fungal rhinosinusitis of the lacrimal sac.

OBJECTIVES/HYPOTHESIS: Chronic sinusitis is a condition affecting millions of individuals each year. Recent findings indicate that chronic rhinosinusitis is a response to fungi mediated by the eosinophil in some of these individuals. We report a case of eosinophilic fungal rhinosinusitis of the lacrimal sac, an entity not previously reported. STUDY DESIGN: Case report. METHODS: A 69-year-old man presented with a 1-cm cystic lesion in the right-side medial canthal region. This lesion was fluctuant, and mucoid material was freely expressed from the medial canthal area. Endoscopic examination r

Radiographic detection of retained neuropatties in the paranasal sinuses: a cadaveric diagnostic accuracy study.

BACKGROUND: Retained surgical materials are a preventable source of postoperative complications. This study evaluates the radiographic detectability of retained neuropatties in the paranasal sinuses to support safer postoperative imaging and surgical practice. METHODS: Sixteen previously dissected cadaveric heads were labeled and assigned to predefined sinonasal or nasopharyngeal site using an allocation table (e.g., frontal sinus anterior table, maxillary sinus, sphenoid sinus floor, nasal cavity, nasopharynx). For each head–site combination, a large neuropatty (1 × 3 in) was first inserted a

Challenging diagnosis and treatment of rare paranasal sinus metastasis from thyroid cancer: a case report and literature review.

BACKGROUND: Metastasis of thyroid carcinoma to the paranasal sinuses is extremely rare. Herein, a case of clinically occult thyroid cancer arising from a long-standing thyroid nodule that metastasized to the sphenoid sinus is presented, accompanied by a literature review. CASE PRESENTATION: A 48-year-old woman visited to the otorhinolaryngology department with visual disturbance and partial nasal obstruction. Computed tomography imaging revealed a tumor in the right sphenoid sinus with adjacent bony destruction, suggestive of paranasal sinus cancer. Thyroid ultrasonography (US) was performed

Silent Corticotroph Adenomas Demonstrate Predilection for Sphenoid Sinus, Cavernous Sinus, and Clival Invasion Compared with Other Subtypes.

BACKGROUND: Nonfunctional pituitary neuroendocrine tumors (PitNETs) exhibit wide variability in growth pattern based on subtype. Silent corticotroph adenomas (SCAs) demonstrate aggressive growth compared with other nonfunctional pituitary adenomas (NFPAs), especially into the cavernous sinus. In this study, we sought to characterize other growth patterns of SCAs compared with NFPAs. METHODS: We performed a retrospective analysis of all patients with nonfunctional PitNETs treated with surgical resection via endoscopic endonasal approach at a single institution from August 1, 2018, to May 11, 2

Extracranial sinonasal tract meningiomas: a clinicopathologic study of 30 cases with a review of the literature.

Extracranial meningiomas of the sinonasal tract are rare tumors. These tumors are frequently misclassified, resulting in inappropriate clinical management. To date, there has been no comprehensive study to evaluate the clinicopathologic aspects of meningioma in these anatomic sites. Thirty cases of sinonasal tract meningiomas diagnosed between 1970 and 1992 were retrieved from the files of the Otorhinolaryngic Registry of the AFIP. Histologic features were reviewed, immunohistochemical studies were performed, patient follow up was obtained, and the results were statistically analyzed. The pati

The unusual first sign of presentation of hepatocellular carcinoma: a rare case report.

BACKGROUND: Hepatocellular carcinoma (HCC) is the most common primary liver malignancy, characterized by aggressive biological behavior and a high propensity for extrahepatic spread. The most common sites of metastasis are the lungs, regional lymph nodes, bones, adrenal glands, and peritoneum. In contrast, the paranasal sinuses are an unusual site for metastatic HCC, and involvement of the sphenoid sinus represents an even rarer event. CASE DESCRIPTION: We report the case of a 67-year-old male patient who presented with transient diplopia, left eyelid ptosis, and refractory epistaxis as the i
