BACKGROUND: There is a debate on the long-term outcomes of surgical decompression for lumbar spinal stenosis (LSS) as compared to conservative treatment, with even more limited outcomes in repeat surgeries. Hence, other less invasive treatment modalities, such as neuromodulation with a modern spinal cord stimulator (SCS), could be considered in the spectrum of management options for symptoms of neurogenic claudication (NC) related to LSS as an alternative to surgery.
OBJECTIVE: Assessing the outcomes and efficacy of SCS in neurogenic claudication in patients with or without a prior lumbar sur…

Traumatic spinal cord injury (SCI) impedes signal transmission by disrupting both the local neurons and their surrounding synaptic connections. Although the majority of SCI patients retain spared neural tissue at the injury site, they predominantly suffer from complete autonomic and sensorimotor dysfunction. While there have been significant advances in the characterization of the spared neural tissue following SCI, the functional role of injury-induced interneuronal plasticity remains elusive. In healthy individuals, spinal interneurons are responsible for relaying signals to coordinate both …

Chronic pain after spinal cord injury (SCI) is a form of central neuropathic pain that is debilitating and often refractory to current pharmacological treatments. Neurostimulation pain therapies, such as epidural spinal cord stimulation, have only moderate success in reducing SCI pain. The pathogenesis of SCI pain may involve a state of central neuronal hyperexcitability, especially in the spinal cord dorsal horn, that develops after injury. We hypothesize that the neuronal structures near the spinal cord injury site may be an important pain generator, and intraspinal microstimulation (ISMS) m…

INTRODUCTION: Spinal cord injury without radiological abnormality (SCIWORA) is a rare entity and usually involves the cervical spine. Thoracic spine involvement is very rare due to the stability provided by the rib cage. The mechanisms of injury and pathophysiology are still debatable.
CASE PRESENTATION: We present a case of an adult male who had road traffic accident and presented with paraplegia. The initial radiological investigations carried out in the emergency department were reported to be normal, however, subsequent magnetic resonance imaging revealed spinal cord contusion without ver…

BACKGROUND: Spinal cord monitoring is now considered standard care during surgery for spinal deformity. Combined somatosensory and motor evoked potential monitoring allows the detection of early spinal cord dysfunction in most patients. The purpose of the current study was to identify clinical factors that increase the risk of intraoperative electrophysical changes and to provide management recommendations.
METHODS: The records of 162 consecutive patients who underwent surgery for the treatment of spinal deformity at a tertiary referral center were reviewed. Electrophysical monitoring of thes…

Primary afferent neurons rarely, if ever, synapse on the sympathetic preganglionic neurons that regulate the cardiovascular system, nor do sympathetic preganglionic neurons normally exhibit spontaneous activity in the absence of excitatory inputs. Therefore, after serious spinal cord injury "spinal sympathetic interneurons" provide the sole excitatory and inhibitory inputs to sympathetic preganglionic neurons. Few studies have addressed the anatomy and physiology of spinal sympathetic interneurons, to a great extent because they are difficult to identify. Therefore, this chapter begins with de…

OBJECTIVE: Nontraumatic spinal cord injury (NT/SCI), which can occur secondary to spinal stenosis, has been shown to represent a significant proportion of individuals admitted for SCI rehabilitation. The objective of this study was to compare demographics and outcomes of patients with spinal stenosis-induced SCI (SS/SCI) with those with traumatic spinal cord injury (T/SCI) following inpatient rehabilitation.
DESIGN: This 7-year prospective review compared 81 patients with SS/SCI and 102 patients with T/SCI admitted to an SCI rehabilitation unit with similar levels and completeness of injury. …

Spinal endodermic cysts are rare and may be associated with other congenital anomalies (mediastinal cysts, bony defects of the vertebral body), derive from defective displacement of the endoderm of the intestinal tract or, more rarely, the respiratory one. The authors describe a case of endodermic cyst of the cervical spine localized anteriorly to the spinal cord. In the case we treated the patient was a 17-year-old male who presented hypostenia of the lower limbs accompanied by hyperreflexia and spasticity; physical examination was negative. MRI, CT and CT-myelography documented a well-define…

A case of spinal myoclonus that complicated spasticity management is presented. A 37-year-old man with a C6 American Spinal Injury Association class B spinal cord injury was referred for treatment of spasticity. He had failed previous treatments with baclofen and dantrolene but was partly relieved by diazepam, although with unacceptable side effects. Further evaluation, including simultaneous electroencephalogram, videotaping, and electromyography of the quadriceps, anterior tibialis, posterior tibialis, and medial hamstring suggested myoclonic jerks of spinal origin that initiated episodes of…

Spinal aneurysmal bone cyst is sufficiently rare for the authors to report this case with rapid evolution and development of paraplegia. Total removal was achieved, and clinical recovery remained complete six months after operation. The pathogenic, clinical, radiological, histological and therapeutic aspects are briefly reviewed and discussed.

CONTEXT: Chronic spinal cord injury (SCI) secondary to spinal tumors may cause persistent motor and sensory dysfunction despite surgical treatment and rehabilitation. Ependymomas are the most common intramedullary spinal tumors in adults, but long-standing neurological deficits after tumor surgery are difficult to treat.
FINDINGS: We report a 51-year-old man with chronic SCI after thoracic spinal ependymoma surgery performed in 2000. He had persistent right-predominant lower-extremity weakness, sensory loss below T10, limited walking endurance, and cane-dependent ambulation. EES was performed…

STUDY DESIGN: Prospective observational cohort study linked with administrative data.
OBJECTIVES: Magnetic Resonance Imaging (MRI) is routinely performed after traumatic spinal cord injury (TSCI), facilitating early, accurate diagnosis to optimize clinical management. Prognosis from early MRI post-injury remains unclear, yet if available could guide early intervention. The aim of this study was to determine the association of spinal cord intramedullary haematoma and/or extent of cord compression evident on initial spine MRI with neurological grade change after TSCI.
METHODS: Individuals with…

BACKGROUND: Spinal cord injury (SCI) can lead to severe impairments in motor, sensory, and autonomic functions, significantly affecting patients' functional independence and quality of life. Transcutaneous spinal cord stimulation (tSCS), as an emerging non-invasive neuromodulation technique, has shown potential in improving motor function. However, existing studies demonstrate substantial heterogeneity, and its clinical efficacy remains unclear.
OBJECTIVE: To systematically evaluate the effects of tSCS on motor function and functional independence in individuals with SCI, and to explore the p…

Tools for monitoring traumatic spinal cord injury (SCI) severity and progression are limited. Ultrasound (US) imaging may be suitable but requires improved understanding of post-SCI spinal cord changes observed on US. Using serial US images in a clinically relevant porcine model of SCI, this study: (1) determined temporal changes to the spinal cord and subarachnoid space over 24-h post-SCI; and, (2) quantitatively compared US to magnetic resonance imaging (MRI) and histology at 24-h post-SCI. Ten anesthetised females pigs received a thoracic contusion SCI across three injury groups, and midsag…

RATIONALE: Currently, the global incidence of spinal cord injury (SCI) ranges from approximately 10.4 to 83 cases per million individuals, with an estimated 500,000 new cases diagnosed annually. Current clinical treatments for SCI primarily include early surgical intervention, pharmacological therapy, and personalized rehabilitation programs. Transcranial magnetic stimulation (TMS) is a noninvasive, painless, and relatively safe treatment option that avoids the side effects and dependency associated with medications. In recent years, TMS has emerged as a promising approach in SCI rehabilitatio…

Oxidative stress is a frequently occurring pathophysiological feature of spinal cord injury (SCI) and can result in secondary injury to the spinal cord and skeletal muscle atrophy. Studies have reported that glycine and N-acetylcysteine (GlyNAC) have anti-aging and anti-oxidative stress properties; however, to date, no study has assessed the effect of GlyNAC in the treatment of SCI. In the present work, we established a rat model of SCI and then administered GlyNAC to the animals by gavage at a dose of 200 mg/kg for four consecutive weeks. The BBB scores of the rats were significantly elevated…

Spinal cord stimulation (SCS) applied between T8 and T11 segments has been shown to be effective for the treatment of chronic pain of the lower back and limbs. However, the mechanism of the analgesic effect at these medullary levels remains unclear. Numerous studies relate glial cells with development and maintenance of chronic neuropathic pain. Glial cells are electrically excitable, which makes them a potential therapeutic target using SCS. The aim of this study is to report glia to neuron ratio in thoracic segments relevant to SCS, as well as to characterize the glia cell population at thes…
