A 41-year-old male presented to the emergency department of a regional Australian hospital with chest and abdominal pain. He became rapidly haemodynamically unstable and was diagnosed with a ruptured splenic artery aneurysm and large volume hemoperitoneum. Due to the regional location of our small hospital, endovascular services are not available and the patient required emergency laparotomy. At laparotomy, a 2 L hemoperitoneum was evacuated, and the bleeding splenic artery aneurysm was identified and controlled. The aneurysm was approached with a unique technique via division of the gastro co

In forensic activity, splenic ruptures occur frequently in cases involving abdominal trauma due to road traffic collision, strokes, falls and work accidents. Splenic lesions can occur either immediately after the trauma or within variable timeframes after it, the last scenario being the case of a delayed splenic rupture. Delayed splenic rupture is ranked third in abdominal traumatology frequency, after liver and intestine rupture. Delayed splenic rupture is more frequently the result of abdominal contusions or compressions, rather than the result of direct wounds located in the left flank. In …

BACKGROUND: Splenic artery aneurysm is the most common aneurysm of visceral vessels. Their rupture usually leads to massive bleeding, being a direct life threat. Splenic artery aneurysms usually rupture into the free peritoneal cavity, and much less frequently into the lumen of the gastrointestinal tract.
CASE REPORT: We describe the case of a 38-year-old male patient, who, as a result of chronic pancreatitis, developed a false aneurysm of the splenic artery, which initially caused necrosis of the large intestine and bleeding into its lumen, and subsequently necrosis of the posterior stomach

Infectious mononucleosis (IM) is a self-limiting lymphoproliferative disorder affecting teenagers and young adults. Splenomegaly is a common manifestation of IM and results in a compromised organ that may rarely rupture spontaneously, with significant morbidity and mortality. The IM spleen should be protected from even minor trauma. Although traditional management of spontaneous splenic rupture in IM has been splenectomy, the role of nonoperative management is evolving. The advent of endovascular interventional modalities has augmented the physician's armamentarium in managing these patients n…

Splenic rupture is an uncommon complication of malaria, which requires urgent medical investigation, close follow-up and adequate treatment. Until present, this complication was reported more often in P. vivax infections than in infections with other species. Rupture can happen spontaneously or as a result of trauma, which may be minor and unnoticed. The diagnosis is made by physical examination, ultrasound and CT-scan. Especially in malaria endemic areas the management of splenic rupture in malaria should be focused on splenic preservation. We describe two cases of splenic rupture during a P.…

We report a case of ruptured intracranial aneurysm from metastatic choriocarcinoma in a patient presenting with intracerebral hemorrhage. Operative evacuation of the hematoma with clipping of a distal right middle cerebral artery aneurysm was performed. Postoperatively, the patient developed hypovolemic shock from spontaneous splenic rupture. Histopathologic examination of the cerebral aneurysm showed choriocarcinoma invading the vessel wall. Metastatic choriocarcinoma should be considered in the differential diagnosis of intracerebral or subarachnoid hemorrhage in women of child-bearing age.

The computer tomographic criteria of splenic rupture, new or old haematomas of the spleen (pseudocysts) and perisplenic or subphrenic abscesses are described with reference to three cases. It is shown that all the relevant problems in the pre-operative stage can be clarified by computer tomography, making arteriography unnecessary. The differential diagnosis between old haematomas and perisplenic abscesses can only be made by taking account of the history and other clinical information.

Vascular Ehlers-Danlos syndrome (vEDS) is a rare COL3A1-related disorder that predisposes pregnant patients to arterial and visceral rupture. We report a 35-year-old primigravida at 26 weeks who presented with abdominal pain, hemorrhagic shock, and a rapid hemoglobin decrease. CT showed splenic laceration with hemoperitoneum. Emergency laparoscopy was converted to laparotomy because of approximately 2,000 mL of hemoperitoneum and limited access. A 3 cm × 2 cm splenic laceration, friable perisplenic tissues, and diffuse uterine serosal petechiae and oozing without gross rupture were identified.

OBJECTIVE: To evaluate the reliability of preoperative abdominal ultrasonography as a staging tool for dogs with hemoperitoneum due to presumed splenic tumor rupture, focusing on the detection of metastatic lesions in the liver.
ANIMALS: 99 dogs from 20 emergency and specialty hospitals across the US.
METHODS: Dogs with nontraumatic hemoperitoneum secondary to splenic tumor rupture were included. A post hoc analysis was conducted on data from a nationwide prospective trial investigating novel treatments for canine hemangiosarcoma. The accuracy of preoperative staging was assessed by comparin

BACKGROUND: Isolated splenic artery dissection (SAD) is extremely rare, life-threatening, and particularly difficult to diagnose. Moreover, SAD presenting as digestive hemorrhage has not been reported.
CASE PRESENTATION: A 44-year-old man presented with recurrent life-threatening hematochezia. Magnetic resonance and computed tomographic angiography showed isolated SAD with an intrapancreatic hematoma. Selective angiography confirmed the diagnosis of rupture of SAD. Hemosuccus pancreaticus was considered the potential mechanism of digestive hemorrhage. It was successfully managed by endovascul

Hydatid disease is a parasitic infection characterized by cyst formation in any organ, although the liver and lungs are most commonly involved. Hydatid disease of the spleen is uncommon, representing <8% of all human hydatid diseases. Splenic hydatid cysts usually coexist with liver hydatid cysts (secondary form), although the spleen is the primary location (primary form) in some cases. The clinical signs and symptoms of splenic hydatid cysts depend on their size, relationship with adjacent organs, and complications. One of the complications of splenic hydatid cysts is cyst rupture either afte

BACKGROUND: Spontaneously ruptured large splenic cyst is a rare in children, and traditionally total or partial splenectomy has been performed for treating an emergent case. We herein present a first case with spontaneously ruptured pediatric splenic cyst treated with emergent laparoscopic dome resection with omental suturing to the parenchymal edge of the spleen.
CASE PRESENTATION: A 12-year-old girl with a spontaneously ruptured large non-parasitic splenic cyst (SC) was successfully treated by emergent laparoscopic dome resection with omental suturing to the edge of the splenic parenchyma.

Hyperinflammation syndrome is a life-threatening condition of heterogeneous etiology. Common manifestations include fever, organomegaly, leukopenia or leukocytosis and malaise. We would like to present a rare case of hyperinflammatory syndrome of unknown origin complicated by shock, severe coagulopathy, spontaneous splenic rupture, and intracranial hemorrhage. A 24-year-old man with no past medical history was admitted to intensive care unit presenting with hemodynamic instability, severe acidosis, splenomegaly, bi-cytopenia, with significantly elevated inflammatory markers. Broad spectrum mic…

Spontaneous (atraumatic) splenic rupture (SSR) is an uncommon and potentially fatal emergency, most frequently linked to infection, cancer, or hematologic disorders. Although clotting factor XIII (FXIII) and factor VII (FVII) deficiencies are uncommon bleeding disorders on their own, their coexistence and association with spontaneous splenic rupture have not been reported before. A 26-year-old adult male with known congenital FXIII deficiency, epilepsy, right-sided hemiparesis, and developmental delay presented with diffuse abdominal pain and vomiting. He was first evaluated at another hospit…

Splenic rupture is a critical surgical condition that poses an immediate threat to the patient's life. In most cases, mechanical trauma to the organ results in the rupture of the capsule, leading to hemorrhage into the peritoneal cavity. Spontaneous (pathological) spleen rupture (SPSR) is considered when the etiology of the rupture is non-traumatic. Spontaneous (pathological) spleen rupture is most commonly associated with hematological malignancies, though it remains a rare condition. Only 8 cases have been caused by mantle cell lymphoma (MCL). This review presents the case of a 59-year-old m…

This is the case of a 38 year-old Lebanese woman G2P1, history of previous cesarean section, presenting at 30+5 weeks of gestation with acute left-sided flank pain and a two-day history of chills and dysuria. In light of the clinical presentation, the patient was initially diagnosed with pyelonephritis and managed accordingly; however, her clinical status deteriorated with worsening hypotension and lethargy despite resuscitative measures and a normal abdominal ultrasound. Failure to revive the patient eventually led to a cardiac arrest for which a peri-mortem cesarean section was performed at
