Transmission electron microscopy analysis of skin lesions from sporotrichosis epidemic in Rio de Janeiro, Brazil.

Transmission electron microscopy can yield useful information in a range of scientific fields; it is capable of imaging at a significantly higher resolution than light microscopes and has been a very useful tool in the identification of morphological changes of the dermis as well as assessment of changes in the extracellular matrix. Our aim is to characterize by electron microscopy the cellular profile of lesions caused by Sporothrix schenckii from the sporotrichosis epidemic in its zoonotic form that occurs in Rio de Janeiro, Brazil.

Heat treatment for certain chronic granulomatous skin infections.

Four cases of chronic granulomatous skin infections (two due to Sporothrix schenckii and two to Mycobacterium marinum) were treated primarily by the intermittent application of local hyperthermia. This treatment was initiated either because of intolerance to conventional iodide therapy for sporotrichosis or as interim therapy while awaiting diagnosis of the mycobacterial infections. The response indicated that the application of heat is a useful adjunct to the therapy of these infections. Our experience, and that of others, suggests that in some cases heat alone may be curative.

Zoonotic Sporotrichosis Related to Cat Contact: First Case Report from Panama in Central America.

A previously healthy 34-year-old man presented with a four-week-old upper arm injury that started with a cat scratch and progressed into painless erythematous nodular ascending lesions in the left upper limb. We made the diagnosis of sporotrichosis based on the epidemiological association of the contact with a cat and culture identification. Sporotrichosis is a mycosis implantation caused by the Sporothrix schenckii complex. We present the first Central American case of sporotrichosis associated with contact with a feline.

Gallium-67 citrate localization in disseminated sporotrichosis.

A 25-year-old woman demonstrated Ga-67 citrate accumulation within a facial cutaneous lesion and two subcutaneous nodules within the right lower extremity. Cultures of the facial lesion and one of the subcutaneous nodules grew Sporothrix schenckii. Ga-67 citrate uptake has not been reported previously with this fungal infection. Ga-67 citrate scintigraphy was helpful in defining the extent of the disease, following its course, and determining the response to therapy.

Multifocal sporotrichosis presenting with lobar pulmonary consolidation.

A case of multifocal systemic sporotrichosis presenting with lobar pneumonia caused by Sporothrix schenkii is described. While five cases of pulmonary involvement in multifocal sporotrichosis have been previously reported, all had nodular or cavitary apical lesions. Similarly, unifocal pulmonary sporotrichosis usually presents with apical cavitary disease. The presence of skin nodules led to a culture diagnosis of sporotrichosis of the skin and prompted further diagnostic studies to elucidate the etiology of the pneumonia.

A prospective necropsy study of arthritis in acquired immunodeficiency syndrome.

Thirty-two knee and 23 sternoclavicular joints from 35 subjects with acquired immunodeficiency syndrome were examined prospectively at necropsy. There were two instances of opportunistic infectious arthritis: one caused by Staphylococcus aureus, the other by Sporothrix schenckii. In five other subjects, para-articular bone was infiltrated by granulomatous or neoplastic sequelae of the human immunodeficiency virus infection. There was no immunohistochemical (p24 antigen) or other evidence for existence of a specific acquired immunodeficiency syndrome arthritis.

A red herring colonization of Mycobacterium lentiflavum in cutaneous sporotrichosis lesions misleading the diagnosis.

A 42-year-old man presented with a sporotrichoid lymphocutaneous infection for 1 month. The histopathology of the lesions showed numerous intracellular mycobacteria within foamy histiocytes suggestive of nontuberculous mycobacteria and misled the empiric treatment. The final culture grew Sporothrix spp. and the lesions improved with antifungal treatment alone. This case report depicts a case of Mycobacterium lentiflavum colonization in cutaneous sporotrichosis lesions on histopathology to raise awareness among physicians about this rare coincidence that could mislead the diagnosis.

Opportunistic infections following renal transplantation.

Opportunistic infection is common following renal transplantation. Prompt diagnosis and management can be life saving. Four different types of opportunistic respiratory infections diagnosed at our center during the period of January 1998 to December 2000 are discussed. Of the four cases one had Aspergillus, second had Sporothrix, third had Nocardia and fourth case Actinomyces species. Microbiologist has an important role to play by being aware of such opportunistic infections and helping the clinician to make early aetiological diagnosis.

Role of melanin in the pathogenesis of cutaneous sporotrichosis.

In this study, we evaluate the clinical, histopathological and ultra structural aspects of the infection by field strain (MEL(+)) and mutated strain (MEL(-)) of Sporothrix schenckii. The development of sporotrichosis at the inoculation sites was similar for both groups of animals after the second week of the experimental period. Statistical differences appeared only at weeks 3 and 5. The pigmented isolate had greater tissue invasive capacity, promoting the formation of multifocal granulomas, whereas the albino isolate induced an enhanced inflammatory response, restraining the agent to the core…

Fixed cutaneous sporotrichosis.

Sporotrichosis is a subcutaneous mycosis that is caused by Sporothrix schenckii. Whereas the most common clinical presentation is the lymphocutaneous form, the fixed cutaneous form is not uncommon. Important clues from the clinical history, such as travel and occupation, can help to raise the suspicion of this infection in the differential diagnosis. However, histopathologic and tissue culture, which often require multiple specimens, are necessary to confirm the diagnosis. We present a patient with fixed cutaneous sporotrichosis whose delayed diagnosis led to appreciable scars and morbidity.

Sporotrichosis atypical presentation as a soft tissue tumour.

Sporotrichosis is a mycosis caused by a saprophytic dimorphic fungus named Sporothrix schenckii. Infections occur following traumatic inoculation of fungus from plants and infected cat bites and scratches. We report a case of a farmer who presented with a solitary subcutaneous nodule initially diagnosed as a soft tissue tumour. A history of agricultural activity and feline contact should draw the clinician's attention to sporotrichosis, as the diagnosis can be easily missed in atypical cases. The diagnosis, microbiology and management of the case are discussed.

[Sporotrichosis of the domestic cat (Felis catus): human transmission].

A case of sporotrichosis transmitted by cat to a veterinarian hospital employees is reported. Inquiry at domiciliary area of the cat's owner revealed two other presumable cases of human sporotrichosis transmitted by cats, and confirmed the diagnosis (by culture of Sporothrix schenckii) of disease in three other domestic cats. Feline sporotrichosis is characterized by ulcerative, cutaneous lesions and systemic dissemination, which invariably cause animal's death. The transmission of sporotrichosis to other animals and humans is enhanced by the great amount of fungus present in cat's lesions.

Sporotrichosis of the knee. A case report.

Articular infections due to the fungus Sporothrix schenkii are rare. Only 46 cases of Sporotrichosis arthritis have been well-documented in the English literature to date. The presentation may be limited to a monoarticular synovitis, hence the diagnosis may be confused with other more common causes resulting in costly delays in proper treatment. Reported is a case of Sporotrichosis of the knee misdiagnosed multiple times over a three-year period yet illustrating important clinical features that could lead to earlier diagnosis and management.

Cell-mediated immune responses in sporotrichosis.

Cell-mediated immunity (CMI) was evaluated in five patients with cutaneous sporotrichosis and six patients with systemic sporotrichosis. Whereas patients with cutaneous disease showed normal CMI, the patients with systemic disease had significant abnormalities in CMI. Most patients exhibited responses to a sporothrix antigen, measured either in conventional assays of lymphocyte transformation or in preincubation assays. Two patients with protracted illnesses and continued abnormalities in CMI were treated with transfer factor and showed improvement in parameters of CMI and control of their dis…

Development and validation of a new quantitative reverse transcription PCR assay for the diagnosis of human sporotrichosis.

Sporotrichosis is an emergent public health problem. The mycological diagnosis of this infection is based on culture, which is fastidious and may represent a biohazard for technicians. Although not widely implemented in routine diagnosis, molecular methodologies are fast, have good accuracy, and can be easily standardized, aiding in the early diagnosis of neglected mycoses. This study aimed at implementing a new pan-Sporothrix quantitative reverse transcription PCR (RT-qPCR) assay, and then validating it on clinical samples from confirmed human sporotrichosis cases. A total of 68 human samples…

Effective treatment using itraconazole combined with terbinafine in the treatment of nasal sporotrichosis: A case report.

RATIONALE: Sporotrichosis is a subacute or chronic infection caused by sporothrix schenckii complex. The misdiagnosis rate of sporotrichosis is very high. Fungal microscopic examination and timely culture help us make an accurate diagnosis and treatment. We observed that combined treatments are more effective than monotherapy in treatment of sporotrichosis. PATIENT CONCERNS: A 47-year-old female complained of pustules and scabs on her nose tip that lasted for 1 month at our hospital. She was diagnosed with skin infection and treated with antibiotics for 20 days. Nonetheless, the treatment did…

Diagnostic value of a nested polymerase chain reaction for diagnosing cutaneous sporotrichosis from paraffin-embedded skin tissue.

BACKGROUND: The gold standard for diagnosis of cutaneous sporotrichosis involves the isolation of the fungus, Sporothrix, by a culture test. Generally, the sampling for the culture test is performed at the same time as skin biopsy under local anaesthesia. However, the culture test may occasionally return a false negative result. OBJECTIVE: The aim of our study was to investigate the diagnostic value of a molecular method for diagnosing cutaneous sporotrichosis from formalin-fixed and paraffin-embedded (FFPE) tissues. METHODS: Over a 30-year period, we collected 52 cases of cutaneous sporotri…

Application of platelet-rich plasma and platelet lysate in the treatment of experimental lymphocutaneous sporotrichosi.

BACKGROUND AND PURPOSE: Sporotrichosis is a subcutaneous and chronic fungal infection that is caused by a dimorphic fungus, namely Sporothrix schenckii sensu lato. Lymphocutaneous sporotrichosis is the most clinical form, which accounts for nearly 80% of the cases of cutaneous sporotrichosis. Platelets contain several substances with antimicrobial properties. Regarding this, the present study was performed to investigate the effect of blood-based biomaterials, especially platelets in the treatment of lymphocutaneous sporotrichosis. MATERIALS AND METHODS: This study was performed on 12 golden …

Central Nervous System Infection with Other Endemic Mycoses: Rare Manifestation of Blastomycosis, Paracoccidioidomycosis, Talaromycosis, and Sporotrichosis.

The central nervous system (CNS) is not a major organ involved with infections caused by the endemic mycoses, with the possible exception of meningitis caused by Coccidioides species. When CNS infection does occur, the manifestations vary among the different endemic mycoses; mass-like lesions or diffuse meningeal involvement can occur, and isolated chronic meningitis, as well as widely disseminated acute infection that includes the CNS, are described. This review includes CNS infection caused by Blastomyces dermatitidis, Paracoccidioides brasiliensis, Talaromyces marneffei, and the Sporothrix …
