Return to Play Following Ankle Sprain and Lateral Ligament Reconstruction.

Ankle sprains are the most common musculoskeletal injury occurring during athletics. Proper initial treatment with supportive pain control, limited immobilization, early return to weight bearing and range of motion, and directed physical therapy are essential for preventing recurrent injury. Reconstruction of the lateral ligaments is indicated for patients with continued instability and dysfunction despite physical therapy. Return to athletic activity should be reserved for athletes who have regained strength, proprioception, and range of motion of the injured ankle. Athletes with a history of

Alteration in global motor strategy following lateral ankle sprain.

BACKGROUND: Lateral ankle sprain (LAS) has often been considered an injury leading to localized joint impairments affecting the musculoskeletal system. Persistent chronic ankle instability and bilateral alterations in motor control after a first ankle sprain episode suggest that the origin of relapses might be a maladaptive reorganization of central motor commands. The objectives of this study were (1) to compare the quality of motor control through motor strategy variables of two groups (with and without LAS) from a military population (n = 10/group), (2) to evaluate the contribution of the l

Therapeutic ultrasound for acute ankle sprains.

BACKGROUND: Ultrasound is used in the treatment of a wide variety of musculoskeletal disorders, which include acute ankle sprains. This is an update of a Cochrane review first published in 1999, and previously updated in 2004. OBJECTIVES: To evaluate the effects of ultrasound therapy in the treatment of acute ankle sprains. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (September 2010), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2010, Issue 3), MEDLINE (1966 to September 2010), EMBASE (1983 to September 2010),

Bilateral balance impairments after lateral ankle trauma: a systematic review and meta-analysis.

Research indicates that balance is impaired in the involved limb following an ankle injury. However, bilateral balance impairments are a viable reason for previous non-significant findings between involved and uninvolved limbs. The purpose of this investigation was to conduct a meta-analysis on studies reporting the effects of lateral ankle trauma on balance of the involved and uninvolved limb after acute ankle injury and chronic ankle instability. Twelve studies qualified for inclusion and assessed static balance for both the involved and uninvolved limbs post-injury and a control group. Meta

Identification of ankle sprain motion from common sporting activities by dorsal foot kinematics data.

This study presented a method to identify ankle sprain motion from common sporting activities by dorsal foot kinematics data. Six male subjects performed 300 simulated supination sprain trials and 300 non-sprain trials in a laboratory. Eight motion sensors were attached to the right dorsal foot to collect three-dimensional linear acceleration and angular velocity kinematics data, which were used to train up a support vector machine (SVM) model for the identification purpose. Results suggested that the best identification method required only one motion sensor located at the medial calcaneus, a

Sonography and MRI of latissimus dorsi strain injury in four elite athletes.

OBJECTIVE: The objective of this study was to describe the MR and sonographic findings in latissimus dorsi (LD) muscle strain in athletes, and to review the most common injuries described in the literature, most of which are humeral avulsions. MATERIAL AND METHODS: Four injuries and two reinjuries of the myotendinous junction of the LD were followed from the day of injury until the return to play. Sonography (US) and MR imaging were performed in each case to confirm the diagnosis and to monitor the healing process. RESULTS: All cases had acute and isolated pain in the back of the shoulder wh

Impingement syndromes of the ankle.

The term "ankle impingement" encompasses several distinct clinical entities resulting from entrapment of either an osseous or soft tissue structure. Three types of ankle impingement -- anterior, anterolateral, and posterior -- are well understood and have been described relatively well in the medical literature. Three additional categories of ankle impingement -- anteromedial, posteromedial, and syndesmotic -- are being recognized increasingly in the orthopedic community but have only scant references in the literature. The six types of ankle impingement are discussed separately, including a d

Sensorimotor deficits with ankle sprains and chronic ankle instability.

The presence of sensorimotor deficits in patients who have suffered ankle sprains or who have chronic ankle instability has been recognized for several decades; however, a body of research literature has developed that elucidates potential physiologic explanations for these deficits. Alterations in a spectrum of sensorimotor measures make it apparent that conscious perception of afferent somatosensory information, reflex responses, and efferent motor control deficits are present with ankle instability. The specific origin of these deficits local to the ankle ligaments or at the spinal or supra

Biomechanics of ankle instability. Part 1: Reaction time to simulated ankle sprain.

PURPOSE: To test the hypothesis that ankles with functional instability will demonstrate slower muscular reaction times than their contralateral stable ankle (SA) and stable healthy controls to a simulated nonpathological ankle sprain mechanism. METHODS: Nineteen male volunteers with a history of unilateral ankle sprain and functional ankle instability (FAI) and 19 healthy male controls performed reaction time tests on a purpose-built platform that simulated a nonpathological combined inversion/plantarflexion ankle sprain mechanism. Participants provided informed written consent. Reaction tim

Adductor strain and hip range of movement in male professional soccer players.

PURPOSE: To determine whether hip range of movement (ROM) can predict the occurrence of adductor strain among male professional soccer players. METHODS: 120 subjects were prospectively selected from 6 professional soccer clubs in Australia. Internal rotation, external rotation, and ROM of the hip were measured using a goniometer before and during the course of a soccer season (2003-2004). When adductor strain occurred, further assessments of the hip were performed. Injured subjects' hip ROMs were measured again at the end of the season. RESULTS: Eight of the 120 subjects had 9 adductor strai

Repeated ankle sprains and delayed neuromuscular response: acceleration time parameters.

STUDY DESIGN: A comparative study. OBJECTIVES: To assess whether in subjects with unilateral chronic ankle instability the dynamic reaction time of the affected ankle differs from the healthy ankle and from ankles of a control group. BACKGROUND: Reaction time is an essential element in joint protection against sudden unexpected excessive movement requiring fast and coordinated muscle action. During a sudden ankle inversion movement, a reflex action of the evertor muscles is needed to counteract the movement. Adequate neuromuscular response is crucial and a delayed response could contribute t
