Implementing guidelines and training initiatives to improve cross-cultural communication in primary care consultations: a qualitative participatory European study.

BACKGROUND: Cross-cultural communication in primary care is often difficult, leading to unsatisfactory, substandard care. Supportive evidence-based guidelines and training initiatives (G/TIs) exist to enhance cross cultural communication but their use in practice is sporadic. The objective of this paper is to elucidate how migrants and other stakeholders can adapt, introduce and evaluate such G/TIs in daily clinical practice. METHODS: We undertook linked qualitative case studies to implement G/TIs focused on enhancing cross cultural communication in primary care, in five European countries. W

Maternal and newborn health services in Papua New Guinea: Health system stakeholder priorities for health system strengthening.

Papua New Guinea (PNG) has one of the highest maternal and newborn morbidity and mortality rates in the Asia-Pacific region, with the COVID-19 pandemic exposing longstanding system weaknesses. This study examines established health system gaps affecting maternal and newborn health (MNH) outcomes in PNG, identifies those considered both high-priority and feasible to address by local stakeholders, and highlights locally driven solutions for sustainable improvements. Qualitative data were collected using card-sorting, with analysis guided by the AAAQ framework (Availability, Accessibility, Accept

Benefit in liver transplantation: a survey among medical staff, patients, medical students and non-medical university staff and students.

BACKGROUND: The allocation of any scarce health care resource, especially a lifesaving resource, can create profound ethical and legal challenges. Liver transplant allocation currently is based upon urgency, a sickest-first approach, and does not utilize capacity to benefit. While urgency can be described reasonably well with the MELD system, benefit encompasses multiple dimensions of patients' well-being. Currently, the balance between both principles is ill-defined. METHODS: This survey with 502 participants examines how urgency and benefit are weighted by different stakeholders (medical st

Consensus on technical procedures in radiology to include in simulation-based training for residents: a European-wide needs assessment.

OBJECTIVES: To identify and prioritize technical procedures for simulation-based training that should be part of the education of residents in radiology. METHODS: This European-wide needs assessment study used a modified Delphi technique to gather consensus from different key education stakeholders in the field. The first round was a brainstorming phase to identify all procedures that a newly specialized radiologist should potentially be able to do. In the second round, each procedure was explored for the need for simulation training; the participants determined frequency, number of radiologi

Patient and Provider Perspectives of a Web-Based Intervention to Support Symptom Management After Radioactive Iodine Treatment for Differentiated Thyroid Cancer: Qualitative Study.

BACKGROUND: Patients diagnosed with differentiated thyroid cancer (DTC) who receive radioactive iodine (RAI) treatment experience acute, medium, and late treatment effects. The timing and severity of these effects vary by individual; common posttreatment effects include dry mouth, salivary gland swelling, dry eyes, and nose bleeds. The nature of symptoms that patients experience after RAI treatment can significantly and negatively impact health-related quality of life. Adequate information during the postprimary treatment phase remains an unmet need among the population of patients diagnosed w

A Delphi Study to Develop a Core Outcome Set for Capturing and Measuring Nurse Well-Being.

BACKGROUND: Poor nurse well-being is a significant concern, adversely affecting patient care quality and satisfaction and contributing to poor job satisfaction, increased sickness absence and workforce retention issues. There are calls for evidence-based policies and interventions to address poor nurse well-being, but no consensus exists on which outcomes should be captured to assess nurse well-being. We used a salutogenic and consensus approach to develop a Core Outcome Set (COS) for nurse well-being. METHODS: A Delphi methodology was employed. Participants were recruited from two stakeholde

Developing and planning country-specific integrated knowledge translation strategies: experiences from the GELA project in Malawi, Nigeria, and South Africa.

BACKGROUND: The Global Evidence, Local Adaptation (GELA) project aims to maximise the impact of research on poverty-related diseases by increasing researchers' and decision-makers' capacity to use global research to develop locally relevant guidelines for newborn and child health in Malawi, Nigeria and South Africa. To facilitate ongoing collaboration with stakeholders, we adopted an Integrated Knowledge Translation (IKT) approach within GELA. Given limited research on IKT in African settings, we documented our team's IKT capacity and skills, and process and experiences with developing and imp

Perceptions of a gender-neutral approach to human papillomavirus (HPV) vaccination in Cameroon: a qualitative study.

BACKGROUND: Cameroon’s Expanded Program on Immunization (EPI) introduced the HPV vaccine into the routine immunization schedule in Cameroon in October 2020. However, coverage for this vaccine was just 20% by December 2022, prompting the government to adopt and implement a gender-neutral vaccination (GNV) from January 2023. The goal was to increase acceptance, reduce stigma, and improve vaccine coverage. This study aimed to assess the perceptions of key immunization stakeholders in Cameroon on the GNV approach to HPV vaccination. METHOD: A qualitative study was conducted from June to September

Co-creation of a gender responsive TB intervention in Nigeria: a researcher-led collaborative study.

BACKGROUND: In Nigeria, men constitute over half of the people notified with tuberculosis (TB), experience longer delays before reaching care, and are estimated to account for two thirds of people who miss out on care. The higher TB risk and burden in men has implications for the whole population and reaching them earlier with TB services will reduce onward transmission in households, communities, and workplaces. The absence of a comprehensive guidance and the lack of substantial empirical evidence on TB care approaches that are responsive to the needs of men in Nigeria exacerbates this proble
