A limited sternal splitting incision is useful for the surgical approach to different diseases of the upper mediastinum. To provide optimal exposure, a specifically designed sternal retractor has been developed. Through a V-shaped opening and elevation of the sternum, surgery of the thymus gland, mediastinal goiters, and, more generally, biopsy or resection of most anterior mediastinal masses are possible. The device has been used in 30 patients who had different diseases, with totally satisfactory results.

This study describes minimally invasive surgery using video-assisted thoracic surgery (VATS) for thymic diseases. Our procedure has been changed in the last decade from sternum-lifting thoracoscopic surgery with mini-thoracotomy to complete thoracoscopic surgery. Twenty-two months experience with a transcutaneous bilirubinometer (TCB) at Waikato Hospital is reported. During this period 2787 paired TCB and serum bilirubin samples were collected. Correlation coefficients for TCB (forehead and sternum) versus serum bilirubin in both Maori and caucasian infants ranged from 0.57 to 0.67.

Although the Nuss procedure for pectus excavatum has been widely employed, a variety of complications have been reported. The most dangerous complication is cardiac injury from the insertion of the introducer. The ribs, sternum, and vertebrae all play an important role in stabilizing the thorax. Failure of one of these components places additional stress on the other supporting structures. We present a case of a 62-year-old man with multiple myeloma and osteopenia who sustained fractures to all three components.

There is a wide variation in the severity of median clefts of the lower lip and mandible. We report a severe case demonstrating clefts of the lower lip and mandible together with lack of supporting structures in the neck and upper sternum. Since a case with a cleft of the manubrium sterni is very rare, the literature is reviewed and discussed.

BACKGROUND: Hemopericardium is a serious complication that can occur after cardiac surgery. While most post-operative causes are due to inflammation and bleeding, patients with broken sternal wires and an unstable sternum may develop hemopericardium from penetrating trauma.

We describe a technique of correction of a severe pouter pigeon breast. Three osteotomies were performed on the sternum, one resecting the curved fused angle of Louis. Multiples chondrotomies allowed to flatten the cartilage part. The reconstruction was firmly hardened with a STRATOS titanium device. The cosmetic result was satisfactory.
