[Prevalence of certain risk factors in lacunar strokes as compared with cortico-subcortical strokes].

Clinical records were reviewed to analyse occurrence of hypertension and other risk factors that may predispose to the development of lacunar stroke. The comparison of risk factors associated with deep, small, ischaemic lesions of the brain (revealed in 67% by CT, in 31% by MRI and in 2% by autopsy) and large superficial or superficial and deep ischaemic lesions (diagnosed in 89% by CT, 4% by MRI and 7% by autopsy) was done. Hypertension, treated and untreated, was more frequent in lacunar stroke, but the difference was not statistically significant. Statistically significant difference was fo…

Prehospital Stroke Management Optimized by Use of Clinical Scoring vs Mobile Stroke Unit for Triage of Patients With Stroke: A Randomized Clinical Trial.

IMPORTANCE: Transferring patients with large-vessel occlusion (LVO) or intracranial hemorrhage (ICH) to hospitals not providing interventional treatment options is an unresolved medical problem. OBJECTIVE: To determine how optimized prehospital management (OPM) based on use of the Los Angeles Motor Scale (LAMS) compares with management in a Mobile Stroke Unit (MSU) in accurately triaging patients to the appropriate hospital with (comprehensive stroke center [CSC]) or without (primary stroke center [PSC]) interventional treatment. DESIGN, SETTING, AND PARTICIPANTS: In this randomized multicen…

A Family History of Stroke Is Associated with Increased Intima-Media Thickness in Young Ischemic Stroke - The Norwegian Stroke in the Young Study (NOR-SYS).

BACKGROUND AND PURPOSE: Positive family history (FH+) of cardiovascular disease (CVD) is a risk factor for own CVD. We aimed to analyze the effect of different types of FH (stroke, coronary heart disease (CHD), peripheral artery disease (PAD) on carotid intima-media thickness (cIMT) in young and middle-aged ischemic stroke patients. METHODS: First-degree FH of CVD was assessed in ischemic stroke patients ≤ 60y using a standardized interview. Carotid ultrasound was performed and far wall cIMT in three carotid artery segments was registered, representing the common carotid (CCA-IMT), carotid bi…

The impact of imbalances in baseline stroke severity on outcome in the National Institute of Neurological Disorders and Stroke Recombinant Tissue Plasminogen Activator Stroke Study.

STUDY OBJECTIVE: The National Institute of Neurological Disorders and Stroke (NINDS) Recombinant Tissue Plasminogen Activator (rtPA) Stroke Study demonstrated a clinically meaningful and statistically significant benefit of tissue plasminogen activator (tPA). Adjusting for the baseline National Institutes of Health (NIH) Stroke Scale, the benefit of tPA remained. However, other authors suggest that an imbalance in baseline stroke severity between the tPA and placebo groups confounded the results. Another issue that has been raised concerns a possible increase in early mortality for individuals…

Assessing the Quality of Stroke Services in Brazil Using the World Stroke Organization Roadmap.

INTRODUCTION: Organizing acute stroke care effectively reduces disability and mortality. Since the Ministry of Health in Brazil established the National Stroke Policy in 2012, stroke care has improved significantly. However, despite the increase in stroke centers, some hospitals have not fully implemented the recommended structure and protocols. This study aims to evaluate the quality of stroke services in Brazil based on the World Stroke Organization (WSO) Roadmap, marking the first step toward the Certification of Stroke Centers in Latin America. METHOD: From 2020 to 2022, we assessed the s…

The association between rheumatoid arthritis and stroke risk by serologic status and stroke subtypes.

BACKGROUND: Rheumatoid arthritis (RA) has been associated with an increased stroke risk, but associations by serostatus (seropositive RA (SPRA) vs seronegative RA (SNRA)) and with subtypes of stroke (ischemic stroke (IS) or hemorrhagic stroke (HS)) are not well established. In addition, it is not well-known whether the use of biologic and targeted synthetic disease-modifying anti-rheumatic drugs (b/tsDMARDs) are associated with altered stroke risk. METHODS: This nationwide cohort study used the Korean National Health Insurance Service database and included participants who were first diagnose…

Effect of encephalomyosynangiosis procedure on post stroke recovery in a permanent model of ischemic stroke.

Acute ischemic stroke (AIS) remains a leading cause of disability worldwide. Limited treatments for AIS with thrombolytics are further constrained by narrow therapeutic windows and no neuroprotective effects. Damaged or underdeveloped vasculature and poor angiogenesis after AIS remains a major cause of failed neuroprotective therapy after AIS. There is a critical need for alternative approaches that enhance angiogenesis and neuroprotection to improve post-stroke recovery. Here, we use an indirect intracranial bypass technique via encephalomyosynangiosis (EMS), which involves the transposition …

Leukoaraiosis as a Promising Biomarker of Stroke Recurrence among Stroke Survivors: A Systematic Review.

Stroke is the leading cause of functional disability worldwide, with increasing prevalence in adults. Given the considerable negative impact on patients' quality of life and the financial burden on their families and society, it is essential to provide stroke survivors with a timely and reliable prognosis of stroke recurrence. Leukoaraiosis (LA) is a common neuroimaging feature of cerebral small-vessel disease. By researching the literature of two different databases (MEDLINE and Scopus), the present study aims to review all relevant studies from the last decade, dealing with the clinical util…

An Examination of Modifiable Risk Factors in Stroke Survivors, with a view to recurrent stroke prevention.

BACKGROUND: Stroke is a major public health problem worldwide, as more people survive their initial stroke event and subsequently live with chronic disability. While extensive research has focused upon preventing primary strokes comparatively little research has examined secondary stroke prevention. METHODS: We analysed data collected from a survey of 576 participants from the 45 and Up Study who had a clinical diagnosis of stroke. Health status was assessed using a variety of validated instruments. Demographics and modifiable risk factors, including health behaviours and diagnosed conditions…

Stroke and stroke-like episodes in inborn errors of metabolism: Pathophysiological and clinical implications.

Inborn errors of metabolism causing stroke (ischemic or haemorrhagic) or stroke-like episodes (e.g., that are also called "metabolic strokes" and include acute brain lesions not related with alterations of blood flow) cover a wide range of diseases in which acute metabolic decompensations after trigger events (e.g., fever, dehydration, sepsis etc.) may have a variable frequency. The early diagnosis of these conditions is essential because, despite their rarity, effective symptomatic treatments may be available for acute settings (e.g., arginine for Mitochondrial myopathy, Encephalopathy, Lacti…
