The utility of pulmonary interstitial emphysema (PIE) in live birth determination is not well established. Because the distinction between l … stillborns), no PIE was identified. We examined the relationship between florid PIE in infants with sudden infant death syndrome (SIDS) or "sudden unexpected death in infancy, manner undetermined" (SUDI), and also its relationship to other variables. No association was found. The presence of equivocal PIE may be an artifact of tissue processing. Florid PIE is found o …

Cot death (sudden infant death syndrome) is one of the most common causes of death in the first year of life. Four cases with a pathological fear of cot death are presented. All the patients were depressed and in 2 cases the fear of cot death had an obsessional quality. In all cases there were complications during pregnancy (miscarriage, threatened abortion, recurrent vomiting in last trimester). In 1 case, the patient knew 3 mothers who had suffered cot deaths; in another, the infant was gravely ill in the neonatal period. Pathological fear of cot death can be recognised by the presence of tw…

Ventricular septal defect (VSD) generally has a good prognosis unless complicated by heart failure (HF). We report a case of sudden infant death because of clinically undiagnosed VSD in a seemingly healthy 16-day-old boy. Although a cardiac murmur was auscultated at birth, detailed clinical examination was not performed. Medicolegal autopsy revealed a perimembranous large VSD with a single coronary artery. The infant was diagnosed to have had HF based on the increased weight of the heart and extremely high serum brain natriuretic peptide levels. Histological examination revealed the degenerati…

INTRODUCTION: Sudden non-cardiac death (SNCD) is a clinical entity comprising deaths lacking previous clinically significant symptoms, and i … s according to the study population: infants and young children (up to four years of age); presumed sudden infant death syndrome patients; and older individuals (five years of age and older). SNCD with viral implication represents a minority of sudden death cases in all age groups, with infants and young children having a higher prevalence across studies. Respiratory …

Sudden death is non-violent, unexpected death occurring less than 24 hours from the onset of symptoms. In this retrospective descriptive study we report the frequency and main causes of sudden deaths in relation to total deaths at a large university hospital in the Eastern region of Saudi Arabia over a 6-year period. The medical files of 1273 consecutive deaths were retrospectively investigated and showed 223 (17.5%) cases of sudden death. Males were 56.0% of cases. The incidence of sudden death was highest in the 2 age extremes (32.2% were infants and 31.4% were elderly). There was a seasonal

The purpose of this report is to present the findings in a neonate with tetralogy of Fallot and a history of prolonged sleep-related apnea requiring resuscitation. At a baseline arterial oxygen pressure of 46 mm Hg, his preoperative responses to carbon dioxide during non-REM sleep (26.1 and 26.6 ml/kg/min/mm Hg of alveolar carbon dioxide tension [PACO2]) were identical to those reported in near-miss sudden infant death syndrome. Following a systemic-pulmonary arterial shunt, the arterial saturation reached 95 percent, and no further apnea occurred. The prompt normalization of the response to c…

The published evidence on the risk of sudden unexpected death in infants after discharge from the neonatal intensive care unit (NICU) is reviewed, together with the relevance of the various identified potentially modifiable post-natal risk factors, particularly sleeping position. Infants of low birthweight, short gestation, and those with adverse perinatal histories are at substantially increased risk of sudden infant death syndrome (SIDS), but the potential benefits from following the measures designed to reduce the risk of SIDS are proportionally greater than for term infants. The use of hom…

Many physicians and researchers are familiar with the tragic phenomenon known as sudden infant death syndrome (SIDS), the leading cause of postneonatal mortality in high-resource countries. A less familiar category of unexplained deaths is the problem of sudden unexplained death in childhood (SUDC), a more rare and unusual presentation of sudden death in children who are no longer infants and whose reasons for death defy explanation. A substantial body of research in SUDC now supports the possibility of an overlap with epilepsy and associated sudden death in that context (SUDEP). Stemming from…

BACKGROUND: Rates of sudden infant death syndrome (SIDS) are twice as high among Black infants compared to white infants in the US. While the contribution of sleep environment factors to this disparity is known, little is known about the risk of SIDS among Black infants in relation to maternal prenatal smoking, alcohol and drug use as well as infant smoke exposure.
OBJECTIVE: To assess the contribution of maternal substance use during pregnancy and the potential interactions with infant bedsharing in a high-risk, urban Black population.
METHODS: The Chicago Infant Mortality Study (CIMS) coll…

CONCLUSION: These results suggest that sudden infant death caused by neuromuscular disorders should be considered.

The postneonatal infant mortality (PNIM) of 2,205 infants admitted to a neonatal intensive care unit from January 1971 to December 1974 was 44 in 1,000 infants who survived to age 28 days. This rate is approximately ten times that of the general population. Congenital malformations (59%), infections (12%), sudden infant death syndrome (10%), and asphyxial brain damage (10%) were the most common causes of death. One third (26) of the infants remained in the hospital whereas two thirds (52) had been dismissed prior to death. All who remained in the hospital plus 36 who had been dismissed died of…

Sudden unexpected death in epilepsy (SUDEP) is a leading epilepsy-related cause of death. Researchers have highlighted the similarities between SUDEP and sudden infant death syndrome (SIDS), but perinatal risk factors such as those identified for SIDS have not been assessed previously for SUDEP. We conducted a population-based case-control study of 58 SUDEP individuals and 384 living epilepsy controls born after 1982, utilizing the Swedish Medical Birth Register together with other national health registers and individual medical records to examine if prenatal and perinatal factors are associa…

Clostridium botulinum type G has not been identified until now from humans or animals; it has been isolated only twice, from soil samples in Argentina. Type G organisms were isolated from necropsy specimens in four adults and an 18-week-old infant. Type G botulinal toxin was demonstrated in the serum of three of these individuals. The toxic dose in mice ranged from 2 to 7 50% lethal doses/ml. These persons died suddenly and unexpectedly at home, without any pathologic evidence to account for the cause of death in four cases. Symptoms in two individuals were similar to those observed in food-bo

Haemorrhagic shock encephalopathy syndrome is a recently described condition which appears to be an important cause of infant mortality and morbidity. Six infants, with some features of this condition, admitted to Dunedin Hospital during the last three years, are presented and their diagnoses reviewed. Diagnostic overlap may occur between haemorrhagic shock encephalopathy syndrome, hypoxic-ischaemic encephalopathy and near miss sudden infant death syndrome. An investigative approach to the infant with shock and acute encephalopathy is outlined.

BACKGROUND: Since the early 90s a striking rise in deformational plagiocephaly (DP) has been reported, and a causal link between the "back to sleep" position recommended to reduce the risk of sudden infant death syndrome. Recent data suggested that supine position is a risk factor only when combined with other environmental factors
OBJECTIVE: To evaluate the impact of early intervention in the newborn environment on the prevalence of DP at 4 months of life.
METHODS: A multicentric, prospective, controlled study in healthy term neonates. Within 72 h of birth, all parents received the usual re…

Four infants with episodic silent complete obstructed apnea are described. All had central sleep apnea, three had hypoventilation, and none had obstructive apnea observed on polygraphic recordings. Two infants were siblings of a sudden infant death syndrome victim and one was a sibling of a "near miss" infant. One infant died as a result of a prolonged episode that began while she was awake. In two infants apnea has been controlled with theophylline administration and in the third infant resolution of the episode was temporally related to atropine administration.
