Binocular disturbance after glaucoma drainage device implantation.

Binocular vision disturbance is a well-described complication of glaucoma drainage device (GDD) implantation. The pathophysiology is not well-understood, but may involve bulk effects from the implant and surrounding bleb, as well as modulation of muscle function due to surgical trauma and post-operative inflammation, resulting in a combined resection/posterior fixation effect. Retrospective studies have found the risks of motility disorder and diplopia vary widely, estimated to be 56%-86% and 57%-75%, respectively. More recently, cross-sectional studies and prospective trials estimate post-GDD

From rib fixation to dynamic chest wall reconstruction: historical lessons for modern surgical stabilization of rib fractures.

PURPOSE: Surgical stabilization of rib fractures (SSRF) has re-emerged as an important component of chest wall trauma care, yet its history is often presented as a simple timeline rather than as a source of operative and biomechanical lessons. This literature, problem-oriented historical review re-examines earlier rib fixation strategies in order to identify the mechanical and operative problems they were intended to solve, why many disappeared, and which principles remain relevant to contemporary SSRF practice. METHODS: A literature review was conducted focusing on historically important fix

Interfacet Distraction for Type B-Basilar Invagination Assisted by a Novel Sliding-Traction Head Holder: 2-Dimensional Operative Video.

Type B basilar invagination (BI) refers to odontoid process prolapse into skull base without dislocation, which is often associated with Chiari malformation and syringomyelia.1 Its pathogenesis and treatment are in controversy. Posterior fossa decompression has been introduced to treat the associated Chiari malformation; however, it may not be effective for symptoms with severe ventral nerve compression.2 C1-2 in situ fixation has been reported to treat minor instability in type B BI,3 yet the assessment of minor instability has not been widely accepted. We believe that the pathogenesis is chr

Multi-functional adhesive hydrogel as bio-interface for wireless transient pacemaker.

Traditional temporary cardiac pacemakers (TCPs), which employ transcutaneous leads and external wired power systems are battery-dependent and generally non-absorbable with rigidity, thereby necessitating surgical retrieval after therapy and resulting in potentially severe complications. Wireless and bioresorbable transient pacemakers have, hence, emerged recently, though hitting a bottleneck of unfavorable tissue-device bonding interface subject to mismatched mechanical modulus, low adhesive strength, inferior electrical performances, and infection risks. Here, to address such crux, we develop

Results of the Use of Bioabsorbable Magnesium Screws for Surgical Treatment of Mason Type II Radial Head Fractures.

BACKGROUND: In Mason classification type II radial head fractures, compared to plate fixation, fixation with cannulated headless screws and absorbable pins has been reported to provide more favorable postoperative outcomes, including less postoperative limitation in range of motion. The fact that radial head fractures are less prone to weight-bearing during fracture union further supports the use of absorbable screws as a suitable alternative treatment option in radial head fractures. This study aimed to perform fixation through open reduction using bioabsorbable magnesium screws for Mason typ

Biomechanical analysis of expansion screws and cortical screws used for ventral plate fixation on the cervical spine.

Compared to bicortical screws, the surgical risk of injuring intraspinal structures can be minimized with the use of monocortical screws. However, this reduction should not be achieved at the expense of the stability of the fixation. With monocortical stabilization, the expansion screws have the potential of absorbing high loads. Therefore, they are expected to be a suitable alternative to bicortical screws for revision surgeries and in osteoporotic bone. The purpose of this in vitro study was to investigate the stiffness of the two screw-plate systems used for ventral stabilization of the cer

Arthroscopic transosseous rotator cuff repair: the eight-shape technique.

All-arthroscopic anchorless transosseous suture techniques combine the advantages of the open transosseous repair with the benefits of arthroscopic technique. However, all the techniques described until now are very complex, difficult to reproduce and associated with an increased surgical time. The authors developed a novel all-arthroscopic anchorless transosseous suture technique easy to perform and to reproduce. This procedure maximizes the tendon-footprint contact area obtaining both medial and lateral fixation without using any device, employing only 1 suture tape so to avoiding the risk o

Medical canthoplasty with the Mitek Anchor System.

A variety of surgical techniques have been described for medial canthal tendon reattachment. They generally require technical skill and prolonged operating time. We have applied a new device, the Mitek Anchor System, in reattaching the medial canthal tendon to the medial orbital wall in 3 patients, and got excellent tendon fixation with this rapid and simple method. No complications, such as local infection or exposure, have occurred in any of the patients up to the present. Medial canthoplasty using the Mitek Anchor System has the advantage of being an easy technique with very accurate placem

Arthroscopic Fixation of a Posterior Cruciate Ligament Femoral Avulsion in an Adult: A Case Report.

Femoral avulsion of the posterior cruciate ligament (PCL) is an exceedingly rare injury pattern in adults, in contrast to the more commonly encountered mid-substance tears or tibial-sided avulsions. We present a rare case of a middle-aged male who sustained a complex multiligamentous knee injury following trauma. Initial imaging revealed avulsion fractures involving both the tibial plateau and medial femoral condyle. Further evaluation with MRI confirmed avulsions of the PCL from its femoral origin and anterior cruciate ligament (ACL) from its tibial attachment, along with posterior root tear
