Evaluation of patients with globus pharyngeus with barium swallow pharyngoesophagography.

OBJECTIVE: The objective of this study was to determine the value of barium swallow pharyngoesophagography in the investigation of patients with globus pharyngeus who had no known disease, no operation history in the head and neck region, and no thyroid pathology. We compare the clinical presentation with the radiologic findings to assess the diagnostic accuracy of barium swallow in such patients. METHODS: This was a retrospective study of the clinical records and radiologic reports of 194 patients with globus pharyngeus, between October 1998 and October 2004, that were reviewed and analyzed

Treatment for swallowing difficulties (dysphagia) in chronic muscle disease.

BACKGROUND: The management of dysphagia (difficulty in swallowing), a common complication of long-term progressive muscle disease in children and adults, is currently unclear. OBJECTIVES: Our objective was to determine the most appropriate intervention for dysphagia in people with chronic, untreatable, non-inflammatory muscle disease. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group trials register, Cochrane Central Register of Controlled Trials (Cochrane Library Issue 2, 2003), MEDLINE (from January 1966 to Week 2 June 2003), EMBASE (from January 1980 to Week 2 June 200

Differential diagnosis between infantile and mature swallowing with ultrasonography.

In order to investigate the difference in tongue movement between visceral (infantile) and somatic (mature) swallowing patterns, 12 visceral (seven females, five males) and 14 somatic (eight females, six males) swallowers were examined with the B+M-mode ultrasound technique. Movements of the tongue tip and submental musculature during swallowing were recorded on video cassette and evaluated with a personal computer. The results demonstrated that the tongue dorsal surface, which was thought to be ideal for observing tongue function, was not suitable for differentiating between visceral and soma

Voice and swallowing in patients enrolled in a larynx preservation trial.

BACKGROUND: The main goals of larynx preservation protocols are preservation of a functional larynx with intact voice and maintenance of normal deglutition. However, few studies have addressed functional outcomes. OBJECTIVES: To evaluate voice and swallowing in patients enrolled in a larynx preservation protocol. DESIGN AND SETTING: Acoustic analysis of 15 patients and videofluoroscopic evaluation of 14 patients who underwent chemoradiotherapy in an attempt to preserve the larynx. PATIENTS: Forty-three patients with larynx or hypopharynx squamous cell carcinomas were treated with weekly pac

Microstomia caused by swallowing of caustic soda: report of a case.

A case of microstomia caused by swallowing caustic soda is presented. A 54-year old man developed a progressive stricture of the circumoral region following accidental ingestion of caustic soda when he was 9 years old. He was treated by a general surgeon who performed bilateral commissurotomies when he was 19 years old and lived normally until he needed major dental prosthetic treatment. His dentist was unable to perform the treatment due to the mouth stricture. The surgical option was to perform bilateral buccal mucosal flaps. A review of the literature and the surgical technique are presente

Mechanisms of airway protection during retching, vomiting, and swallowing.

We investigated the mechanisms of airway protection and bolus transport during retching and vomiting by recording responses of the pharyngeal, laryngeal, and hyoid muscles and comparing them with responses during swallowing and responses of the gastrointestinal tract. Five dogs were chronically instrumented with electrodes on the striated muscles and strain gauges on smooth muscles. Retching and vomiting were stimulated by apomorphine (5-10 ug/kg iv). During retching, the hyoid and thyroid descending and laryngeal abductor muscles were activated; between retches, the hyoid, thyroid, and pharyn

Morphological findings in dynamic swallowing studies of symptomatic patients.

The aim of this study was to assess the role of videofluoroscopy in the detection of structural abnormalities of the pharynx and esophagus in patients with different symptoms of impaired deglutition. Dynamic radiographic recording of deglutition was performed in 3193 consecutive patients (1578 men, 1615 women; mean age 54 years) suffering from dysphagia, suspicion of aspiration, globus sensation, and non-cardiac chest pain. We assessed different structural lesions from the oral cavity to the esophagus and classified them into eight categories. Their frequency and association with the different

[Manometric analysis of the oral and pharyngeal phases of the act of swallowing].

Manometric analysis of swallowing was conducted on 35 subjects, aged 44 to 60. The course of physiological changes in pressure during oral and pharyngeal phase of swallowing was analysed. To objectify the evaluation of analysed phenomena, parameters that include both pressure values inside pharynx and oesophagus and time dependences connected with changes in their value were used as proposed by the authors of this paper and as applied by other authors involved with these issues.

Voluntary and reflex influences on the initiation of swallowing reflex in man.

The electrophysiological features of voluntarily induced and reflexive/spontaneous swallows were investigated. In normal subjects, swallows were elicited by infusing water either into the mouth (1-3 ml) or directly into the oropharyngeal region through a nasopharyngeal cannula (0.3-1 ml). For water infused orally, subjects were either requested to swallow voluntarily or instructed to resist swallowing and maintain the horizontal head position until swallowing occurred reflexively. Spontaneous saliva swallowing was investigated in patients with severe dysphagia who had a prominent clinical pict…

An inter-disciplinary approach to swallowing problems in acute stroke.

Many speech and language therapy (SLT) departments are in danger of being swamped with dysphagia referrals, often to the detriment of other work. At the same time surveys have shown that large numbers of patients with acute stroke have swallowing problems which are poorly managed during the critical early phase, not referred or missed altogether. The Collaborative Dysphagia Audit (CODA) study, carried out in six British hospitals, showed that nurses could quickly be trained to carry out basic screening assessments for dysphagia and that implementation of a co-ordinated inter-disciplinary dysph

Motor strategy in rapid sequential swallowing: new insights.

This study examined the physiological properties and movement strategies of normal, rapid sequential swallowing during simultaneous videofluoroscopy (VFS) and submental surface electromyography (EMG). Ten subjects performed discrete (5 and 15 cc) and sequential (150 cc in tilted and upright head postures) swallowing tasks. Analyses included VFS event timing, movement/bolus passage characteristics, EMG amplitude waveforms, and peak and offset EMG amplitudes. Results revealed that sequential swallows were significantly shorter than discrete swallows in several VFS event durations, but significan…

Abnormal patterns of breathing during swallowing in neurological disorders.

Swallowing momentarily inhibits breathing, and normally the great majority of apnoeas which accompany a swallow are followed by expiration (xE swallows). This swallowing-breathing interaction is regarded as one of several mechanisms by which the airway is protected from aspiration during swallowing. The purpose of this study was to investigate the interaction of breathing and swallowing in two groups of neurological patients. Twenty-two patients with defined neurological disorders involving various structures or pathways (brain, spinal cord and peripheral nerves) were studied to determine whet…

When it's hard to swallow. What to look for in patients with dysphagia.

Swallowing disorders can be divided into oropharyngeal dysphagia and esophageal dysphagia. The most common cause of oropharyngeal dysphagia is cerebrovascular accidents; other causes may include oropharyngeal structural lesions, systematic and local muscular diseases, and diverse neurologic disorders. Esophageal dysphagia may result from neuromuscular disorders, mortality abnormalities, and intrinsic or extrinsic obstructive lesions. Through clinical history taking helps define the tpe of dysphagia and can guide diagnostic testing. Important questions to ask patients with the disorder include
