Nine hundred twenty-three thoracoscopic sympathectomies for primary hyperhidrosis were performd in 462 patients (180 men, 282 women). Mean age was 29.2 (11-61) years. Synchronous bilateral sympathectomies were performed in 461 patient: right side alone in 1 patient. This study focuses on changes in adrenergic sensitivity in untransected sensory axons that innervate an area of skin made neuropathic by transection of neighboring nerves. Surgical thoracoscopy is performed with laparoscopic material and instrumentation and is not limited to the therapeutic applications of classical pleuroscopy such as thoracic sympathectomy or pleurectomy for pneumothorax. It is a simpler, more rapid technique which causes less tissue damage and appears to be more effective than the other transparietal techniques. It has been performed twice for sympathectomy and five times for recurrent pneumothorax with excellent results.

Compensatory sweating after sympathectomy does not have a satisfactory, free-of-secondary-effects treatment. Glycopyrrolate has been successfully used to treat other types of hyperhidrosis. Compensatory sweating after sympathectomy could respond to the topical application of glycopyrrolate. Ten patients were selected with compensatory sweating after sympathectomy. Computed tomography had revealed division of the right upper lobe by a fissure and the azygos vein, and the presence of an azygos lobe had been diagnosed. Endoscopic thoracic sympathectomy (resection of T3 ganglion) was performed.
