[Acquired flat foot of the adult by posterior tibial dysfunction. Options for surgical treatment].

We present the possible etiopatogenic causes of posterior tibial dysfunction or painful flat foot of the adult and the cause-and-effect relationship that may exist. We also expose the gradation of the lesion and the different therapeutic options for the surgical treatment of the deformity. Since 1939, multiple articles have been published, which have been endorsed by clinical, experimental, electromyographic and biomechanical studies; publications that have been consulted and evaluated for the development of this review. In our opinion: the dysfunction of the posterior tibial is caused in prin

Insertional Tendinopathy of the Achilles: Debridement, Primary Repair, and When to Augment.

Insertional Achilles tendinopathy is a degenerative enthesopathy associated with pain and dysfunction. Nonsurgical management is first attempted for a period of 3 to 6 months and may consist of physical therapy with eccentric training and other modalities. Surgical treatment can be successful with a variety of approaches. A thorough debridement through a midline tendon-splitting approach is associated with high satisfaction rates. Flexor hallucis longus transfer to augment the repair is considered in older, heavier patients or if more than 50% of the tendon was debrided. Early functional rehab

Anterior tibial tendon insertion: an anatomical study.

Anatomical variations of the anterior tibial tendon insertion were studied by dissection in 44 normal feet of 22 embalmed adult cadavers. Three types of insertion were found. The most common insertion (56.8%) was a division of the tendon into two slips, equal in width, the one slip being inserted into the first cuneiform and the other slip into the base of the first metatarsal. The next most common (27.3%) was a division of the tendon into a large slip, inserted into the first cuneiform, and a small slip, inserted into the base of the first metatarsal. The third type, and least common tendon i

Opponoplasty without postoperative immobilization.

Opponoplasty using tendon transfer is a useful reconstructive procedure that restores lost thenar muscle function. Tendon transfers, however, require postoperative immobilization periods of up to four weeks before the sutured tendons reach required strength. We developed an opponoplasty procedure using α-TCP (alpha-tricalcium phosphate) cement that does not require postoperative immobilization and was applied to nine hands out of nine cases. The procedure is performed under local anesthesia without a pneumatic tourniquet and on an outpatient basis. In this procedure, the flexor digitorum super…

Interosseous transfer of tibialis posterior for common peroneal nerve palsy.

The interosseous route remains popular for tibialis posterior tendon transfer for drop-foot. It leaves a smaller range of movement than the circumtibial route, but lengthening the calcaneal tendon may improve this. The results of this present series indicate that, in order to predict a good functional result, the ankle must be held in at least 20 degrees of dorsiflexion at the time of tendon transfer.

Spontaneous rupture of the extensor pollicis brevis tendon in a baseball pitcher: a case report.

A 15-year-old pitcher on a boys' Little League baseball team suffered spontaneous rupture of the extensor pollicis brevis (EPB) tendon. Since the EPB tendon was, at surgery, found hypoplastic and non-functional, transfer of the extensor indicis proprius (EIP) tendon was carried out. After a 6-month period of rehabilitation and follow-up, the patient was able to resume playing baseball. Although rupture of the EPB is rare, transfer of the EIP tendon is one of the treatments of choice for such injuries.

Arthroscopically assisted latissimus dorsi transfer with a minimally invasive harvesting technique: surgical technique and anatomic study.

Latissimus dorsi musculotendinous transfer has been described for the treatment of massive rotator cuff defects. The aim of this paper is to present the relevant surgical anatomy of an arthroscopically assisted technique associated with a modified harvesting of the tendon. For tendon harvesting, a skin incision of about 5-6 cm in length is made in the axillary crease orthogonal to the longitudinal axis of the upper arm. For the musculotendinous transfer, we propose to increase vector action of the latissimus dorsi by passing the transferred tendon ahead the triceps muscle under or over the Ter

An in vitro study comparing the use of suture anchors and drill hole fixation for flexor digitorum longus transfer to the navicular.

BACKGROUND: The surgical management of posterior tibial tendon dysfunction often includes transfer of the flexor digitorum longus (FDL) tendon through a tunnel in the navicular. Fixation often is obtained by sewing the tendon back onto itself. The purpose of this study was to compare this standard method of fixation with suture anchor fixation, a technique that may be associated with less surgical morbidity, because it requires the harvesting of less tendon length. METHODS: FDL tendon transfer to the navicular was done in 13 fresh-frozen cadaver specimens. In six feet comprising the standard …

Posterior Tibial Tendon Allograft Reconstruction for Stage II Adult Acquired Flatfoot: A Case Series.

Surgical treatment for a stage II adult acquired flatfoot has consisted of reconstruction of the diseased posterior tibial tendon with flexor digitorum longus tendon transfer, combined with osteotomies to address the underlying deformity. This case series presents an alternative to tendon transfer using allograft tendon for posterior tibial tendon reconstruction. Four patients who underwent stage II flatfoot reconstruction with posterior tibial tendon allograft transplantation were included. All patients had preoperative radiographs demonstrating flatfoot deformity and magnetic resonance imagi…

Endoscopically Assisted Percutaneous Harvesting of the Flexor Hallucis Tendon in Zone 2: An Anatomical Study.

BACKGROUND: Flexor hallucis longus (FHL) transfer is a well-established method for treating chronic Achilles tendon ruptures and tendinopath … The additional length gained from this technique is unlikely to be required for the majority of FHL tendon transfer procedures. If additional length is needed, we would recommend the use of intraoperative ultrasonography or a mini-open approach to minimize injury risk. LEVEL OF EVIDENCE: Level V, expert opinion.

Tibialis anterior transfer "into talus" for control of the severe planus pediatric foot: a preliminary report.

Transfer of tibialis anterior into the talus has been utilized for correction of the vertical talus, as well as for paralytic valgus foot deformities with the suggestion for its use in severe flatfoot deformities. The authors present transfer of tibialis anterior through the talus for controlling the severe planus pediatric foot. The procedure is presented, and six feet having undergone this procedure are analyzed. This procedure, in combination with others, has proven successful in reconstructing this pediatric deformity clinically and radiographically.

[Subcutaneous rupture of the long thumb extensor tendon. Etiology, clinical aspects and therapy].

Subcutaneous ruptures of the extensor pollicis longus tendon are mostly due to an earlier wrist trauma and rarely caused by a degenerative systemic disease. The tendon usually ruptures close to the distal edge of the retinaculum extensorum where it is vulnerable in various respects and in addition exposed to increased mechanical strain. Functional loss of the extensor pollicis longus greatly impairs the function of the entire hand and requires surgical therapy. Transfer of the extensor indicis tendon has proven successful in 95% and thus appears to be the most appropriate therapeutic procedure

Management of Chronic Achilles Tendon Ruptures.

Chronic Achilles tendon rupture is a difficult condition to treat due to tendon retraction and histologic changes. Traditional treatments include primary repair (for defects <2 cm), VY advancement (using gastrocnemius aponeurosis for larger gaps), Achilles tendon turndown (using gastrocnemius fascia, often with flexor hallucis longus transfer), allografts, and tendon transfers (using peroneus brevis or flexor hallucis longus tendons). New synthetic graft options, such as acellular collagen matrices and Artelon, may improve chronic Achilles tendon rupture healing without the morbidity associate…
