The role of CT angiography in the diagnosis of blunt traumatic thoracic aortic disruption and unsuspected carotid artery injury.

We have replaced aortography and open thoracic surgery to diagnose and treat blunt traumatic thoracic aortic disruption (TTAD) in favor of CT angiography (CTA) and endovascular repair. The purpose of this study is to review our experience with the management and outcomes of TTAD and associated carotid artery injuries. In January 2003, we initiated a protocol that used CTA to evaluate all patients with suspected TTAD from blunt trauma. When TTAD was diagnosed, patients were managed by endovascular repair using abdominal aortic extension cuffs. Twenty-nine patients with TTAD were managed by endo

Esophageal injury from thoracic pedicle screw placement in a polytrauma patient: a case report and literature review.

The authors present a case report illustrating a visceral complication that may occur as result of thoracic pedicle screw placement. The case describes the previously unreported occurrence of esophageal impingement secondary to anterior vertebral body perforation by a pedicle screw at the third thoracic vertebra. This case highlights the challenge of thoracic pedicle screw placement and the importance of preoperatively measuring the maximum anterior-posterior dimension of the vertebral body.

A comparison of FES with KAFO for providing ambulation and upright mobility in a child with a complete thoracic spinal cord injury.

This study compared functional and physiologic measures of ambulation and upright mobility with functional electrical stimulation (FES) versus knee-ankle-foot-orthoses (KAFO) in an 11-year-old boy with a T-10 level spinal cord injury. The child was a limited community ambulator with bilateral KAFO and loftstrand crutches. The FES system consisted of percutaneous intramuscular electrodes controlled by a portable stimulator and thumbswitch, an AFO for ankle and foot support, and loftstrand crutches. The subject used a swing-through gait pattern with both modes of mobility. The Functional Indepen

Abdomino-thoracic shooting wound with multivisceral involvement including injury of abdominal aorta in the supra-mesocolic segment.

Abdomino-thoracic wounds caused by shooting are very serious as they frequently affect several organs and require extreme emergency surgery. Within traumatic abdominal vascular wounds, aortic injury in the supramesocolic segment is associated with a high mortality, 64% (2). The case reported, a 19y. old young man, suffered multiple organ injuries by shooting involving abdominal aorta, stomach, liver, diaphragm and lung. He was operated in extreme emergency. The evolution has been simple, the patient leaving the hospital 19 days after surgery. This case presents a tactical peculiarity about the

High-Thoracic Spinal Cord Injury Impairs Biventricular Function and Blunts Cardiopulmonary Interactions in a Rodent Model.

Relative to the well-characterized detrimental effects of high-level spinal cord injury (SCI) on left ventricular (LV) function in both experimental models and clinical populations, the impacts of SCI on right ventricular (RV) function and cardiopulmonary interactions (for both LV and RV) remain largely unexplored. To address these gaps, we investigated biventricular function and cardiopulmonary interactions in adult male Wistar rats subjected to high-thoracic (T3) contusion SCI. Two weeks post-injury, animals were mechanically ventilated and instrumented for simultaneous LV, RV, and arterial

Traumatic chylothorax after blunt thoracic injury: A case series from a level 1 trauma centre.

Traumatic chylothorax is an uncommon complication of thoracic injury resulting from disruption of the thoracic duct or its tributaries. Non-iatrogenic blunt traumatic chylothorax is particularly rare and may present with delayed clinical features, making diagnosis challenging. We present a case series of four patients with blunt trauma-associated chylothorax managed at a level 1 trauma centre over a 22-year period. We aim to highlight patterns of delayed diagnosis, explore the relationship between injury level and chylothorax laterality, and describe the multidisciplinary management strategies

The Society for Vascular Surgery clinical practice guideline on the management of blunt thoracic aortic injury: Focused update.

Blunt thoracic aortic injury (BTAI) remains a devastating and potentially fatal complication of major trauma that requires expedient diagnosis and management by a collaborative team of trauma and vascular specialists. Trauma patients with BTAI commonly have multiple associated injuries, including intracranial or truncal hemorrhage, that increase the complexity of factors related to timing and conduct of repair. Optimal outcomes for these patients require consideration of these factors and updated knowledge of the available evidence by all stakeholders of care in a collaborative fashion. In 201

Clinical and Computational Analysis of Left Subclavian Artery Coverage on High-Risk Blunt Thoracic Aortic Injury.

Background/Objectives: Blunt thoracic aortic injury (BTAI) is a rare, highly lethal trauma typically occurring at the aortic isthmus. Advanced BTAI is primarily treated with thoracic endovascular aortic repair (TEVAR). Because emergent surgical debranching is rarely feasible, management depends heavily on patient anatomy, especially regarding the left subclavian artery (LSA). Patient-specific computational fluid dynamics (CFD) models offer critical insights into periprocedural planning and outcome prediction. Methods: This study investigates hemodynamic changes in a patient-specific BTAI case

Development and Validation of a Thoracic Aortic Calcification-Based Nomogram for Early Myocardial Injury in Breast Cancer Patients.

Early identification of patients with breast cancer who are at increased risk of treatment-related myocardial injury may support individualized cardiovascular surveillance. This retrospective study included 551 patients treated at Sun Yat-sen Memorial Hospital between January 2014 and December 2021. All patients had chest computed tomography data and at least three high-sensitivity cardiac troponin T (hs-cTnT) measurements. Early myocardial injury was defined as hs-cTnT > 14 ng/L within 6 months after treatment. Patients were randomly divided into training (n = 366) and validation (n = 185) co

Impact of Work-Related Injury on the Recovery of Health-Care Workers after Surgery for Thoracic Outlet Syndrome.

BACKGROUND: Thoracic outlet syndrome (TOS) may arise as a sequela of injury. Work-related (WR) injuries have been reported to have poor TOS outcomes. Our goal is to assess impact of WR on health-care workers (HCW) (physicians, nurses, allied health) with TOS. METHODS: We reviewed a prospectively maintained TOS database of 1,800 cases and identified HCW who underwent TOS decompression surgery for WR injuries. These were compared to a contemporary HCW group who sustained nonwork-related (NWR) injury. Datapoints included demographics, work activities, and injuries. Outcomes were assessed by retu

A random forest model using flow cytometry data identifies pulmonary infection after thoracic injury.

BACKGROUND: Thoracic injury can cause impairment of lung function leading to respiratory complications such as pneumonia (PNA). There is increasing evidence that central memory T cells of the adaptive immune system play a key role in pulmonary immunity. We sought to explore whether assessment of cell phenotypes using flow cytometry (FCM) could be used to identify pulmonary infection after thoracic trauma. METHODS: We prospectively studied trauma patients with thoracic injuries who survived >48 hours at a Level 1 trauma center from 2014 to 2020. Clinical and FCM data from serum samples collect…

Rapidly Progressing Dysphagia After Thoracic Spinal Cord Injury in a Patient With Ankylosing Spondylitis: A Case Report.

INTRODUCTION: Ankylosing spondylitis (AS) is a chronic systemic inflammatory disease affecting the axial skeleton, including the sacroiliac joint, which causes vertebral fusion in the advanced stage. However, reports of anterior cervical osteophytes compressing the esophagus and causing dysphagia in patients with AS are rare. Here, we present the case of a patient with AS and anterior cervical osteophytes who exhibited rapidly progressing dysphagia after thoracic spinal cord injury (SCI). CASE PRESENTATION: The patient, a 79-year-old man, was previously diagnosed with AS and had syndesmophyte

Treadmill Training for Common Marmoset to Strengthen Corticospinal Connections After Thoracic Contusion Spinal Cord Injury.

Spinal cord injury (SCI) leads to locomotor dysfunction. Locomotor rehabilitation promotes the recovery of stepping ability in lower mammals, but it has limited efficacy in humans with a severe SCI. To explain this discrepancy between different species, a nonhuman primate rehabilitation model with a severe SCI would be useful. In this study, we developed a rehabilitation model of paraplegia caused by a severe traumatic SCI in a nonhuman primate, common marmoset (Callithrix jacchus). The locomotor rating scale for marmosets was developed to accurately assess the recovery of locomotor functions

Maladaptation of renal hemodynamics contributes to kidney dysfunction resulting from thoracic spinal cord injury in mice.

Renal dysfunction is a hallmark of spinal cord injury (SCI). Several SCI sequalae are implicated; however, the exact pathogenic mechanism of renal dysfunction is unclear. Herein, we found that T3 (T3Tx) or T10 (T10Tx) complete thoracic spinal cord transection induced hypotension, bradycardia, and hypothermia immediately after injury. T3Tx-induced hypotension but not bradycardia or hypothermia slowly recovered to levels in T10Tx SCI and uninjured mice ∼16 h after injury as determined by continuous radiotelemetry monitoring. Both types of thoracic SCI led to a marked decrease in albuminuria and

Diagnosis of blunt thoracic aortic injury with electrocardiogram-gated computed tomography in pediatric patient: A case report.

Computed tomography (CT) is a sensitive and specific test for thoracic aortic injury, and is the choice of diagnostic test for adult patients. However, the diagnostic accuracy of conventional CT in pediatric patients has not been elucidated, and the diagnostic strategy has not been clarified. We present the case of an eight-year-old patient who had a thoracic injury, with left open pneumothorax, pulmonary contusion, and multiple left-sided rib fractures. Although the findings on conventional CT were insufficient either to diagnose or deny as having thoracic aortic injury, additional examinatio

Experimental high thoracic spinal cord injury impairs the cardiac and cerebrovascular response to orthostatic challenge in rats.

Spinal cord injury (SCI) impairs the cardiovascular responses to postural challenge, leading to the development of orthostatic hypotension (OH). Here, we apply lower body negative pressure (LBNP) to rodents with high-level SCI to demonstrate the usefulness of LBNP as a model for experimental OH studies, and to explore the effect of simulated OH on cardiovascular and cerebrovascular function following SCI. Male Wistar rats (n = 34) were subjected to a sham or T3-SCI surgery and survived into the chronic period postinjury (i.e., 8 wk). Cardiac function was tracked via ultrasound pre- to post-SCI
