We report a case of a 35-year-old woman affected by costal exostosis, originating from the posterior arc of the left fifth rib, who complained of a persistent intractable neuralgia in the left T5 dermatome. Both pain and the risk of visceral injury led us to resect exostosis. The procedure was performed using a uniportal videothoracoscopic approach without additional incisions. For bone resection, we used Piezosurgery, a soft tissue-sparing system based on ultrasound vibrations. Piezosurgery allowed the complete resection of exostosis without injuring the intercostal nerve and vessels. The his

A 43-year-old woman with a 1-year history of generalized myasthenia gravis was referred for a video-assisted thorascopic surgery thymectomy. The preoperative chest computed tomography revealed mild diffuse thymic hyperplasia. Pathology of the resected thymus gland revealed a 6-mm World Health Organization (WHO) Type AB microthymoma without thymic germinal centres. The entity of microthymoma was reviewed with a focus on examining the pathological and therapeutic differences between thymic hyperplasia and microscopic thymoma.

The technique of air sterilization by thermal effect was revisited in this work. The impact of incorporating a high efficiency heat recovery exchanger to a sterilizing cell was especially assessed. A mathematical model was developed to study the dynamics and the steady state of the sterilizer. Computer simulation and reported data of thermal inactivation of pathogens permitted obtaining results for a proof-of-concept. The simulation results confirmed that the incorporation of a heat recovery exchanger permits saving more than 90% of the energy needed to heat the air to the temperature necessar

Spontaneous haemopneumothorax (SHP) is a rare, potential life-threatening emergency. Patients suffering from spontaneous haemopneumothorax can present at the emergency department with dyspnoea and unexplained signs of significant hypovolemia. Discussion about patient selection, timing and technique of operation is still alive. Standard chest roentgenogram is the most useful way to diagnose spontaneous haemopneumothorax, although false negative results exist. In most cases, initial conservative treatment requires later surgical intervention. So early surgical management is needed. In haemodynam

A 4-year-old male presented with abdominal pain. A computed tomography scan of the abdomen was negative, but a pleural effusion and mass was noted in the lower left thorax. Video-assisted thoracoscopic surgery revealed the mass to be a rare case of extralobar pulmonary sequestration that had undergone infarction.

In spite of being a relatively rare disorder, synchronous bilateral spontaneous pneumothorax requires proper attention because of its consequences that essentially constitute an acute life-threatening chest cavity event. Standard of care is stabilization of the patient's condition with emergency drainage of both pleural cavities that should be followed by early surgical revision, preferably using miniinvasive approach involving intervention of the lung, or pleura. The case report uses the example of a 19-year-old patient to alert to possible errors in the process of therapy of this disorder, p

Video-assisted thoracic surgery (VATS) is increasingly being used for congenital cystic lung diseases but there are very few reports of VATS lobectomy because of its technical difficulties. Congenital pulmonary airway malformations (CPAM) is a rare congenital developmental abnormality; an immediate and proper surgical resection must be performed for prevention of malignant transformation and recurrent pulmonary infection. Here, we report a case of a 1-year-old girl with type II-CPAM who was successfully treated with VATS lobectomy. Under single lung ventilation, the child was placed in the lat

PURPOSE: The aim of this study was to highlight the unique case of an intrathoracic mesothelial cyst in a 5-year-old child that was treated with a thoracoscopic resection.
BACKGROUND: Mesothelial cysts are benign cysts lined by a single layer of mesothelial cells. These rare lesions can be found on, or adjacent to, serous membranes but are only occasionally located within the thorax. In children, less than 10 intrathoracic cases have been described thus far, most of which are diaphragmatic in origin.
CASE: An asymptomatic 5-year-old female presented with a left-sided mediastinal mass inciden

The literature describing multimodal treatment of thymomas consists almost exclusively of retrospective case series. We have used a systematic review to investigate the role of surgery in the management of thymomas. The use of surgery as the sole therapeutic maneuver in thymoma depends on the stage considered. Subtotal resection followed by adjuvant treatment may prolong survival, but studies are equivocal. Some data supports re-resection of recurrent thymoma in the belief that survival will be prolonged. Approaches to thymectomy other than sternotomy in early stage thymoma are technically sou

A 19-year-old man presented with pneumonia, cough, and occasional dyspnea. Chest CT scan and bronchoscopy with biopsy revealed a typical carcinoid tumor obstructing the orifice of the right middle lobe, leading to lobar collapse. Preoperative surgical planning included radial endobronchial ultrasound, which confirmed that the tumor was not invasive into the bronchus intermedius. With that information, a video-assisted right middle lobectomy was performed with a wedge bronchoplasty in order to preserve the right lower lobe. The operation was performed completely thoracoscopically with three 1.2

Schwannomas are tumors that are typically benign. They are derived from Schwann cells (glial cells of the peripheral nervous system that serve to separate and isolate nerve cells from adjacent structures). The most common type of schwannoma is a benign tumor of cranial nerve VIII and is referred to as an acoustic neuroma. When extradural, such tumors usually present as masses that can invade adjacent structures, thereby becoming symptomatic, as in the case of intrathoracic schwannomas (typically found in the posterior mediastinum). Herein, we present a case of a schwannoma treated through vide

Fourteen years have passed since video-assisted thoracic thymectomy (VATS-T) began to be used to treat diseases of the thymus. Indications for VATS-T have been expanded by ensuring sufficient space for easy endoscope manipulation using the sternum-lifting method. A variety of VATS-T techniques are available, including the cervical approach, bilateral VATS, infrasternal approach, and others. Although these techniques have the common feature of not involving sternotomy, they differ from each other in terms of the extent of thymectomy. In Japan, bilateral VATS (involving neck incision combined wi

We present here a rare case of left-side catamenial pneumothorax, in which endometrial tissue pleura was found on the visceral pleura histologically. A chest roentogenogram confirmed the left pneumothorax, but did not reveal bullae or any associated anomalies, in a 41-year-old woman with three documented episodes of left-side pneumothorax occurring every menstruation. Video-assisted thoracoscopic surgery revealed not only tiny holes in the diaphragm, but also a scattered small brown spots on the visceral pleura. Histological examination of the lung sections revealed the existence of endometria

A 70-year-old man was admitted to our hospital because of hemoptysis and abnormal shadows on chest film. He was given a diagnosis of pulmonary aspergilloma with fungus ball by computed tomography and other evaluations. Success rate of systemic or topical treatment with antifungal agents is reported to be 80%. At this case lobectomy under VATS was chosen because he had chronic liver dysfunction by hepatitis type C and the lesion was localized in the right upper lobe. Bronchial artery embolization was performed prior to the operation in order to minimise bleeding on lysing the adhesion between t

Single-incision thoracoscopic surgery (SITS) for primary spontaneous pneumothorax offers advantages over multiport video-assisted thoracoscopic surgery, but lesion retraction remains challenging. We describe a modified SITS technique using spinal needle anchoring for precise lung stabilization. A bent spinal needle inserted through an intercostal space mimics a second-port grasper, enabling multiple re-hooking maneuvers for optimal lesion alignment. This technique allows for a smaller incision, minimizes instrument crowding, and reduces postoperative pain. The needle insertion site leaves no v

Radiofrequency ablation (RFA) comparative efficacy of treatments using video-assisted thoracoscopic sympathectomy (VATS) in the long term remains uncertain in patients with palmar hyperhidrosis (PHH). This study aimed to compare the efficacy and safety of RFA and VATS in patients with PHH. We recruited patients aged ≥ 14 years with diagnosed PHH from 14 centres in China. The treatment options of RFA or VATS were assigned to two cohort in patients with PHH. The primary outcome was the efficacy at 1-year. A total of 807 patients were enrolled. After propensity score matching, the rate of complet
