Long-term evaluation of vitreomacular traction disorder in spectral-domain optical coherence tomography.

PURPOSE: The aim of this article was to estimate the natural course of vitreomacular traction (VMT) disorder using high-resolution spectral-domain optical coherence tomography. METHODS: Nineteen eyes of 19 patients with idiopathic symptomatic VMT and who underwent spectral-domain optical coherence tomographic follow-up examinations were included in the retrospective observational study. The average observation period of all patients who underwent the examination was 8 months (±4.4 months). We observed microstructural changes in the macula and the position, length, and vector forces of the att…

[Massive hemoptysis due to traction bronchiectasis caused by thoracic radiotherapy 23 years earlier].

We reported a 72-year-old woman who had a massive hemoptysis due to traction bronchiectasis in the left upper lobe. The patient underwent left radical mastectomy followed by thoracic radiotherapy for left breast cancer. The chest computed tomography showed traction bronchiectasis in the atrophic left upper lobe and the bronchial angiography showed hypervascularization of bronchial and internal thoracic arteries to the left upper lobe. Left upper lobectomy was performed after bronchial embolization for recurrent massive hemoptysis. Postoperative course was uneventful. Pathologic findings showed…

Permanent brachial plexus injury following vaginal delivery without physician traction or shoulder dystocia.

A vaginal delivery that resulted in a permanent brachial plexus injury unassociated with shoulder dystocia or physician traction is reported by the delivering physician. This case demonstrates unequivocally that not all permanent brachial plexus injury at vaginal birth is due to physician traction.

Penile traction therapy for treatment of Peyronie's disease: a single-center pilot study.

INTRODUCTION: Peyronie's disease (PD) is a fibrotic disorder of the penis whose etiopathophysiology remains unclear. At this time, there is no known reliable nonsurgical treatment. This study reviews our experience with external penile traction therapy to correct the deformity associated with this disorder. AIM: To evaluate prolonged external penile traction as a nonsurgical treatment for PD. METHODS: Ten men with PD completed this noncontrolled pilot study of traction therapy using the FastSize Penile Extender. Nearly all (90%) had failed prior medical therapy. Traction was applied as the o…

The effect of intra-operative skeletal (skull femoral) traction on apical vertebral rotation.

The study design is a retrospective review of consecutive case series. Our goal was to identify and quantify the effect of skeletal traction on the apical vertebral rotation (AVR). Intra-operative skeletal traction has been used for the correction of large magnitude idiopathic and neuromuscular scoliosis. The ability of skeletal traction to correct the rotational deformity of the spine has not been characterized. Following REB approval, retrospective analysis of 22 (AIS = 14, neuromuscular = 8) consecutive pediatric patients having surgical posterior instrumented correction and fusion for thei…

Medial tibial stress syndrome (tibial fasciitis): a proposed pathomechanical model involving fascial traction.

Although medial tibial stress syndrome is one of the most common lower-extremity overuse injuries, its pathomechanics remain controversial. Two popular theories have been proposed to account for this condition: tibial bending and fascial traction. This article evaluates the role of fascial traction in medial tibial stress pathomechanics. We hypothesized that with contraction of the deep leg flexors tension would be imparted to the tibial fascial attachment at the medial tibial crest. We also speculated that circumferential straps would dampen tension directed to the medial tibial crest. The am…

High incidence of vitreomacular traction in recurrent choroidal neovascularisation after repeated photodynamic therapy.

BACKGROUND: The causes of recurrent choroidal neovascularisation (CNV) after photodynamic therapy (PDT) remain controversial. Subretinal surgery was carried out after unsuccessful PDT. AIMS: To determine intraoperatively the status of the posterior vitreous interface. DESIGN: Interventional case series. METHODS: Conventional three-port vitrectomy was carried out in 10 eyes with CNV that had undergone 1-4 PDT sessions. The vitreous cutter was held close to the edge of the optic nerve to evaluate the status of the posterior vitreous. RESULTS: Lesion size showed an increase from 1.5 (standard

[Traction retinal detachment, optic atrophy, apallic syndrome after shaking trauma in an infant].

INTRODUCTION: Ophthalmological examinations are important in children with suspected shaken baby and/or battered child syndrome. Retinal and epiretinal haemorrhages can indicate non-accidental injuries. We observed a case of extensive retinal hemorrhages, edema of the optic disc followed by development of optic atrophy, neovascularisation and tractional retinal detachment over the course of months. CASE REPORT: A 6-week-old infant with no history of systemic disease or trauma was admitted to the children's hospital because of a disorder of consciousness, respiratory insufficiency, taut fontan…

Nerve traction during correction of knee flexion deformity. A case report and calculation.

We report the case of a child with cerebral palsy and spastic diplegia treated for bilateral fixed flexion of the knee by bilateral hamstring lengthening. An attempt to straighten the legs from 90 degrees to 20 degrees flexion damaged the sciatic nerve. There are no objective means of estimating how much deformity can be reduced safely. We present a method of calculating the extra strain in the sciatic nerve produced by reducing a flexion deformity. The result, combined with clinical judgement, provides guidelines for safe corrective surgery.
