Characteristics and Validation of Pediatric Extremity Vascular Trauma in Dedicated Pediatric and Adult Trauma Centers: A Scoping Review Protocol.

BACKGROUND: Pediatric extremity vascular injuries constitute a rare yet serious entity that can lead to serious complications especially if left untreated or become late diagnosed. In our scoping review, we sought to evaluate different characteristics and outcomes of pediatric and combined adult trauma centers (ATCs) in the management of pediatric extremity vascular injury. METHODS: We sought to analyze various characteristics and parameters that differentiate a dedicated pediatric and a combined pediatric ATC in terms of effectiveness and quality of care in the acute setting and to describe

Splenic injury due to colonoscopy: nursing considerations.

Complications associated with the performance of a colonoscopy are low. Splenic tearing or trauma postcolonoscopy is a rare but serious complication. Identification of a splenic injury is difficult because of the nonspecific nature of the patient's symptoms. A meta-analysis approach was used to identify common risk factors, onset and presentation of symptoms, and procedural difficulty associated with splenic injury postcolonoscopy. A review of 10 published case studies and 1 case presented by the author were compared with published findings for patients sustaining a splenic injury following co

Time and motion: a study of trauma surgeons' work at the bedside during the first 24 hours of blunt trauma care.

BACKGROUND: The current literature defines the costs of trauma care in terms of hospital costs and charges. We sought to define the qualitative and quantitative labor costs of trauma care by measuring the various components of bedside care provided by surgeons at a community hospital. METHODS: We conducted a prospective time-and-motion study during the initial 24 hours of blunt trauma patients' stay in the hospital at a Level II trauma center. The services provided by two surgeons and one nurse practitioner were examined. All patients were resuscitated and seen initially by one of the physici

Closed head injuries in children: is hospital admission always necessary?

The objective of this study was to identify a group of patients with mild closed head injury, lack of other significant trauma, and normal head computerized tomograph (CT), who could be safely observed at home by a reliable caretaker. Data were from a retrospective chart review of pediatric emergency department (PED) and hospital course of an urban university children's hospital. The pediatric trauma registry was used to identify patients one to 17 years old seen in the PED with closed-head injury and normal head CT between June 1991 and August 1992. A total of 746 patients with heads injury w

Individual Pharmacotherapy Management (IPM) - I: a group-matched retrospective controlled clinical study on prevention of complicating delirium in the elderly trauma patients and identification of associated factors.

BACKGROUND: Delirium is one of the most frequent complications in hospitalized elderly patients with additional costs such as prolongation of hospital stays and institutionalization, with risk of reduced functional recovery, long-term cognitive impairment, and increased morbidity and mortality. We analyzed the effect of individual pharmacotherapy management (IPM) in the University Hospital Halle in geriatric trauma patients on complicating delirium and aimed to identify associated factors. METHODS: In a retrospective controlled clinical study of 404 hospitalized trauma patients ≥70 years we c

Outcomes of admitted geriatric trauma victims.

Conflicting data exist as to the outcome of elderly victims of trauma. With recent improved outcomes for functional recovery, aggressive management of these patients has been advocated. The purpose of this study is to determine outcomes of admitted elderly trauma victims based on initial mechanism of injury and the degree to which other factors affected their overall outcome. A prospective study involving admitted patients > or =65 years was performed at an urban university center from September 15, 1996 until August 31, 1997. Patients sustaining any potentially serious form of trauma were inc

Trauma, psychological distress and markers of systemic inflammation among US women: A longitudinal study.

BACKGROUND: Prior evidence links posttraumatic stress disorder (PTSD) and depression, separately, with chronic inflammation. However, whether effects are similar across each independently or potentiated when both are present is understudied. We evaluated combined measures of PTSD and depression in relation to inflammatory biomarker concentrations. METHODS: Data are from women (n's ranging 628-2797) in the Nurses' Health Study II. Trauma exposure, PTSD, and depression symptoms were ascertained using validated questionnaires. We examined (a) a continuous combined psychological distress score su

Factors associated with post-traumatic stress symptoms in healthcare providers after performance of cardiopulmonary resuscitation: a mixed-methods study.

The experience of performing cardiopulmonary resuscitation (CPR) can cause post-traumatic stress symptoms that negatively impact healthcare providers and reduced their clinical competency. This two-phase mixed-methods was conducted to investigate the factors that cause post-traumatic disorder (PTSD) in healthcare providers who perform CPR. Phase 1 included a survey with a trauma screening questionnaire (TSQ). Phase 2 included gathering interview data and applying conventional content analysis to identify themes. Of the 286 medical staff who completed the TSQ, 32 (11.2%) of respondents experien

Extreme Control and Instability: Insight Into Head Injury From Intimate Partner Violence.

AIM: The aim of this study was to understand the social context of the lives of women who experienced a head injury from intimate partner violence. BACKGROUND: Sixty percent to 92% of survivors of intimate partner violence receive head trauma during the abuse. Little research exists regarding the episodes of abuse when women receive a head injury, or the reasons women might not seek medical care for the head injury or the abuse. METHOD: Twenty-one interviews from nine women who self-reported passing out from being hit in the head were analyzed using thematic analysis. FINDINGS: Themes of ex

Survival after trauma in geriatric patients.

In contrast to other studies, a recent report from the authors' institution has shown a good prognosis for functional recovery in geriatric patients that survive trauma. Because most survivors regained their pre-injury function, the authors examined factors related to nonsurvival in this population of 82 consecutive blunt trauma victims older than the age of 65. Seventeen patients died (21%). Compared with survivors, nonsurvivors were older, had more severe overall injury, and had more severe head and neck trauma but did not differ in severity of trauma that did not involve the head and neck,

Pandemic flu and the sudden demand for ECMO resources: a mature trauma program can provide surge capacity in acute critical care crises.

BACKGROUND: Patients with severe H1N1 pneumonia created a sudden demand for extracorporeal membrane oxygenation (ECMO) capacity. In a single referral center, the established procedures, protocols, and staff of the Level I trauma service were adapted to help manage this nontrauma critical care crisis. METHODS: When airway pressure release ventilation and high-frequency oscillator ventilation failed, we used standard ECMO circuits and the VDR-4 critical care ventilator. We cannulated patients percutaneously in the intensive care unit and transported them on ECMO. Trauma service resources includ

Alcohol-related brief interventions as a criterion for American College of Surgeons Level I Trauma Center verification: how best to train the interventionists?

BACKGROUND: The American College of Surgeons Committee on Trauma recently required that Level I trauma centers have the capability to perform counseling in the form of brief interventions (BIs) for injured patients identified as problem drinkers. However, it is not yet known what type of training is optimal for trauma center personnel who will conduct these BIs. METHODS: We conducted a prospective cohort study at the University of California, Davis Medical Center, a Level I trauma center. We compared two methods of training trauma nurse practitioners (NPs) without prior counseling expertise t

A Concept Analysis of the Help-Seeking Journey Among Domestic Violence Survivors.

The concept of help-seeking has been widely used across health disciplines to describe individuals' efforts to access care, yet its meaning remains inconsistent, particularly in the context of domestic violence. Survivors of intimate partner violence often experience multiple barriers in seeking help, with research showing that, on average, seven attempts are made before leaving an abusive relationship. Clarifying the help-seeking journey is therefore critical to understanding survivors' vulnerability and informing supportive nursing practice. This concept analysis applied Walker and Avant met

A Case of Polytrauma in a Farmer Following a Cow Attack: "Code Farmer".

This case involves a farmer who presented to a level 1 trauma center following an attack by a cow. It highlights the emerging use of the term "code farmer" among emergency department physicians and nurses, a colloquial alert reflecting the concern that farmers may present with seemingly minor complaints that often mask more serious or insidious underlying injuries or health conditions.  The phrase "code farmer" is an emerging term gaining traction among emergency medicine physicians and nurses. It reflects a recognition that farmers often endure strenuous work environments, long hours, and a h

Deployed Women Veterans: Important Culturally Sensitive Care.

PROBLEM: Today, with almost 23 million veterans in the nation, and currently only about 10 million, or less, of them seeking active services associated with the Veterans Administration (VA) health facilities, these men and women veterans will be seeking some, more, or even all of their health care over their life time in civilian-based facilities. METHODS: Pertinent literary sources were reviewed to gather applicable data about the problem. FINDINGS: Every patient that enters your health facility should be asked an essential assessment question: "Have you served in the military?" Importantly

Application of Orem's self-care model in postoperative rehabilitation of patients undergoing digit replantation.

Digit replantation is often caused by sudden trauma, with 75% of amputated patients being low-income, low-education workers, and adding societal pressure. Orem's self-care model emphasizes self-care and may be an optimal nursing model for digit replantation, but its clinical application and research are limited. This study explores Orem's self-care model's impact on digit replantation. This single-center retrospective observational study included 389 patients who underwent digit replantation in our hospital from June 2019 to November 2023. Based on previous nursing methods, patients were divid
