[Current insights into the pathophysiology of rosacea].

Rosacea is a chronic inflammatory skin disease mainly affecting the face. Four major clinical subtypes of rosacea can be identified: erythemato-telangiectatic, papulopustular, phymatous and ocular rosacea. Still, it is currently unclear whether these subtypes develop consecutively or if any subtypes may occur individually as part of a syndrome. Rosacea is characterized by facial flushing, erythema, chronic inflammation, edema and fibrosis. Several trigger factors can worsen the disease or cause recurring episodes of inflammation. Although some aspects in the pathophysiology of rosacea have bee

What do we know about chronic tension-type headache?

In the past few years there has been an increasing body of knowledge about etiological mechanisms of chronic tension type headache (CTTH), p … ctors of CTTH. An updated pain model has suggested that CTTH can be explained by referred pain from trigger points (TrPs) in the cranio-cervical muscles, mediated through the spinal cord and the trigeminal nerve nucleus caudalis. Different therapeutic strategies (pharmacological and non-pharmacological) are generally used for the management of these patients. CT …

Differential diagnosis and treatment in a patient with posterior upper thoracic pain.

BACKGROUND AND PURPOSE: Determining the source of a patient's pain in the upper thoracic region can be difficult. Costovertebral (CV) and costotransverse (CT) joint hypomobility and active trigger points (TrPs) are possible sources of upper thoracic pain. This case report describes the clinical decision-making process for a patient with posterior upper thoracic pain. CASE DESCRIPTION: The patient had a 4-month history of pain; limited cervical, trunk, and shoulder active range of motion; limited and painful mobility of the right CV/CT joints of ribs 3 through 6; and periscapular TrPs. Interve…

[Heat shock proteins or "stress proteins"].

INTRODUCTION: Heat shock proteins (HSP) are molecular chaperones which facilitate the biosynthesis and maturation of proteins within cells (protein folding). They promote assembly and disassembly of polypeptides and play a major role in cellular function, not only during the stress response but also at basal state. CURRENT KNOWLEDGE AND KEY POINTS: As HSP are immunogenic molecules and can be expressed on cellular membranes, their role in auto-immune and inflammatory diseases, particularly in systemic lupus erythematosus, Behçet's disease and rheumatoid arthritis, has been studied. Cellular im

[Morphological study of Organum cavum pre-lamina terminalis (OCPLT), implicated in thirst in rats].

It has been shown that micro-injections of angiotensin II in the Organum cavum pre-lamina terminalis (OCPLT) of the rat brain trigger water intake. Our anatomical study shows that the anterior, vertical extension of this Organum corresponds to the anterior inter-hemispheric fissure, and its posterior, horizontal extension to the Cavum septi pellucidi (CSP). Contrary to classical descriptions, the CSP persists in the adult rat, though reduced to one third of its initial antero-posterior extension. It appears rostrally as an opening into the anterior inter-hemispheric fissure. The absence of epe

Stealthy watermarking for medical AI models via spatial patch trigger and two-stage training.

Deep learning models for medical image analysis require substantial computational resources, domain expertise, and curated datasets, making them valuable intellectual property. Protecting the ownership of such models is critical, yet existing watermarking techniques often compromise diagnostic accuracy or introduce visible artifacts that are unacceptable in clinical settings. In this paper, we propose a stealthy watermarking framework specifically designed for medical AI models. Our approach utilizes a spatial patch trigger mechanism that mimics natural imaging phenomena commonly observed in d

Suicidal death of erythrocytes in cancer and its chemotherapy: A potential target in the treatment of tumor-associated anemia.

In analogy to apoptosis of nucleated cells, erythrocytes may enter eryptosis characterized by cell shrinkage and cell membrane scrambling. Eryptotic erythrocytes are rapidly cleared from circulating blood and may adhere to the vascular wall. Stimulation of eryptosis thus impairs microcirculation and leads to anemia as soon as the loss of erythrocytes cannot be fully compensated by enhanced erythropoiesis. Signaling stimulating eryptosis includes increase of cytosolic Ca2+ -activity, ceramide, caspases, calpain, p38-kinase, protein-kinase C, Janus-activated kinase 3, casein-kinase 1α, and cycli

Serotonin 6 receptor controls Alzheimer's disease and depression.

BACKGROUND: Cytomegalovirus (CMV) infection in immunocompetent adults is usually mild; severe hyperinflammatory phenotypes occur rarely. Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening syndrome characterised by uncontrolled immune activation; cytomegalovirus is a recognised but uncommon trigger. Overlapping features with other inflammatory disorders complicate timely diagnosis and management. CASE DESCRIPTION: We report a 28-year-old previously healthy male with several days of gastrointestinal and upper respiratory symptoms followed by persistent fevers and a transient petechi

Congenital Long QT Syndrome Unmasked by COVID-19 Infection Presenting as Ventricular Tachycardia Storm.

BACKGROUND: Congenital long QT syndrome (LQTS) predisposes to torsades de pointes and sudden cardiac death. Although it usually manifests in childhood or early adulthood, external triggers may unmask the disease later in life. COVID-19 has been associated with cardiovascular complications, but its association with ventricular tachycardia storm precipitated by congenital LQTS is rarely reported. CASE SUMMARY: A woman in her 50s presented with recurrent syncope during acute COVID-19. Electrocardiography showed marked QTc prolongation and torsades de pointes requiring cardioversion, deep sedatio

The knowns and unknowns of cardiac autoimmunity in viral myocarditis.

Myocarditis can result from various infectious and non-infectious causes that can lead to dilated cardiomyopathy (DCM) and heart failure. Among the infectious causes, viruses are commonly suspected. But the challenge is our inability to demonstrate infectious viral particles during clinical presentations, partly because by that point, the viruses would have damaged the tissues and be cleared by the immune system. Therefore, viral signatures such as viral nucleic acids and virus-reactive antibodies may be the only readouts pointing to viruses as potential primary triggers of DCM. Thus, it becom

Melatonin: a significant contributor to the pathogenesis of migraine.

It is generally accepted that migraine is caused by a primary biochemical disorder of the central nervous system involving neurotransmitters, specifically serotonin. The pathogenetic mechanism triggered by external and internal stimuli is not well explained or understood. This article points to the possibility that the pineal gland, a primary source of central serotonin and melatonin, contributes significantly to migraine attacks.

Temporalis Muscle-Originated Supraorbital Neuralgia Misdiagnosed as Trigeminal Neuralgia.

In patients with pharmacotherapy-refractory supraorbital neuralgia, a myofascial etiology should be included in the differential diagnosis. The authors report a case of a 73-year-old man with left supraorbital pain triggered by mastication, initially misdiagnosed as trigeminal neuralgia (V1). Physical examination identified a myofascial trigger point (MTrP) in the ipsilateral temporalis muscle, leading to a revised diagnosis of temporalis muscle-originated supraorbital neuralgia. Management involved ultrasound-guided pulsed radiofrequency of the supraorbital nerve combined with dry needling of

The ancient Greek poet Sappho and the first case report of the fight-or-flight response.

Sappho has always been regarded as one of the greatest lyric poets of ancient Greece. Her famous poem Fragment 31 V., also known as the "Ode to Jealousy", accurately describes the profound emotional reaction triggered by the sight of her beloved. The poet's precise description of each sign and symptom triggered by this arousal makes Sappho 31 V., to the best of our knowledge, the first analytical description of the acute stress response, the so-called "fight-or-flight" response, in human history. Here, Fragment 31 V. is re-read from a medical point of view, correlating the ancient Greek lyric

Systemic Sarcoidosis Associated with Exposure to Borrelia burgdorferi in a 21-Year-Old Man.

OBJECTIVE: Here we describe a rare case of systemic sarcoidosis in a healthy young man with neuroborreliosis as a putative trigger. CASE: A 21-year-old forestry worker was diagnosed with systemic sarcoidosis involving his lungs, kidneys and skin. Additional diagnostics revealed signs indicative of a recent infection with Borrelia burgdorferi. The patient was treated successfully with prednisolone and antibiotics. CONCLUSION: Sarcoidosis is a multisystem granulomatous disorder of unknown aetiology. This case suggests that an infection with B. burgdorferi, the causal agent of Lyme disease, cou

Toward continuous and non-invasive monitoring: a scoping review of in vitro blood glucose devices from electrochemistry to optics and micro-system integration.

Abnormal blood glucose levels trigger acute symptoms and drive long-term complications across vascular, renal, neural, and ocular systems. Overall, this review traces the transition in blood glucose monitoring from fingerstick SMBG to continuous monitoring and to non-invasive routes and classifies technologies by invasiveness into IBGM, MIBGM, and NIBGM. Key evaluation metrics include MARD, time lag, wear time, and calibration burden, in particular for clinical translation. It outlines the evolution from reflectance photometry to enzyme amperometry in first- and second-generation sensors and t

Incidence and Short-Term Prognosis of Acute-On-Chronic Liver Failure Defined by Japanese Criteria: A Single-Center Retrospective Cohort Study From Urban Japan.

BACKGROUND AND AIM: Acute-on-chronic liver failure (ACLF) carries high short-term mortality, but real-world evidence under the Japanese ACLF criteria remains limited. We assessed incidence, clinical profile, outcomes, and prognostic factors at a tertiary center in urban Japan. METHODS: We retrospectively reviewed 363 consecutive hospitalizations of patients with cirrhosis (2014-2022) at a tertiary care center in Japan. ACLF per Japanese criteria was categorized as confirmed (meeting both PT INR/PT activity and bilirubin thresholds) or extended (meeting either biochemical criterion alone). We

Drug-mediated metabolic tipping between antibiotic resistant states in a mixed-species community.

Microbes rarely exist in isolation, rather, they form intricate multi-species communities that colonize our bodies and inserted medical devices. However, the efficacy of antimicrobials is measured in clinical laboratories exclusively using microbial monocultures. Here, to determine how multi-species interactions mediate selection for resistance during antibiotic treatment, particularly following drug withdrawal, we study a laboratory community consisting of two microbial pathogens. Single-species dose responses are a poor predictor of community dynamics during treatment so, to better understan

Clinical effectiveness of electroacupuncture in meralgia paraesthetica: a case series.

BACKGROUND: Meralgia paraesthetica is a fairly common condition resulting from entrapment of the lateral femoral cutaneous nerve. I have found that acupuncture produces a rapid improvement, sometimes effecting a cure, after only one or two treatments. I therefore invited referrals in order to collect a case series. METHODS: A series of 10 patients, which included two who had refused surgery, but excluded those with significant lumbar spine problems, were treated. Visual Analogue Scale pain scores and analgesic intake were recorded weekly, starting before treatment. Four patients were receivin

Effects of chlorpromazine with and without UV irradiation on gene expression of HepG2 cells.

Damage to DNA can trigger a variety of stress-related signals that alter the expression of genes associated with numerous biological pathways. In this study, we have used a cDNA microarray representing 1089 genes related to DNA damage and repair, cell cycle, transcription, metabolism and other toxicologically important cell functions to identify genes regulated in response to DNA damage in HepG2 cells induced by UV-activated chlorpromazine (CPZ). CPZ itself is not genotoxic but, upon UV irradiation with a non-genotoxic dose in the UVA range, it produces reactive free radical intermediates with
