A fifteen-month-old child was admitted with a week history of isolated fever. On CSF (Cerebral Spinal Fluid) examination, hyperproteinorachy, hyperglycorachy and hypochlorurachy were found. The diagnosis of tuberculosis meningitis was suspected but usual tests were unable to find Mycobacterium tuberculosis in CSF, urine and sputum. Only the Polymerase Chain Reaction detected the Mycobacterium tuberculosis genome in the CSF. A specific treatment was started immediately. Apyrexia was obtained within 2 days; the outcome was favorable, without sequelae.

BACKGROUND/OBJECTIVE: Spinal intramedullary tuberculoma is rare, accounting for 2/100,000 of cases of tuberculosis and only 2% of all cases of tuberculosis of the central nervous system. Diagnostic imaging is essential to improving diagnosis and management of this disease.
METHODS: The clinical profile, radiological data, and histological slides of 2 cases of intramedullary tuberculomas confirmed by pathologic examinations were reviewed.
RESULTS: In 2 cases, magnetic resonance imaging (MRI) showed thickening of the spinal cord and oval lesions with a low T1-weighted image signal and a typica

BACKGROUND: Differentiating between spinal tuberculosis, pyogenic (bacterial) spondylitis and spinal metastasis remains a major diagnostic challenge because their radiological features often overlap. Delayed or incorrect diagnosis may lead to inappropriate treatment, permanent disability or death.
OBJECTIVE: To develop and evaluate deep learning models for automated classification of spinal tuberculosis, pyogenic infection, and spinal metastasis using magnetic resonance imaging (MRI).
METHODS: T2-weighted sagittal MRI scans from 120 patients (40 per disease group) with pathologically or micr

The diagnosed patients treated at an osseous tuberculosis hospital are analyzed. In the reporting period, the diagnosis of osseous tuberculosis has not been verified. Late detection of osseous tuberculosis, as its prehospital hyperdiagnosis is one of the main reasons of a grave condition in this group of patients. The paper characterizes plastic reparative versus palliative decompressive operations for tuberculous spondylitis with spinal-cord abnormalities and shows that radical surgery has a higher efficiency (89%) in the treatment of an inflammatory process and in the regression of neurologi

Osteo-articular tuberculosis is rare in infants. The MRI findings reported for adolescents and young adults mainly relate to spinal involvement. Two cases of osteo-articular tuberculosis of infants located at the knee are presented. Vaccination has been correctly done. Skin test and chest radiography were normal. Evolution was insidious for one case. Osseous, medullary, cartilaginous and soft tissue abnormalities revealed by MRI were suspicious for tuberculosis. Diagnosis was confirmed at histology for both cases and bacteriology for one case. The aim of this study is to report the MRI feature

BACKGROUND: Primary spinal infections (PSIs) are a group of uncommon but serious infectious diseases that are characterized by inflammation of the endplate-disc unit. Pediatric spinal infection is rare and challenging to diagnose due to vague presenting symptoms. Most cases are conservatively managed with surgery rarely indicated. The authors performed a systematic review to study the baseline characteristics, clinical presentation, and outcomes of pediatric patients with PSIs who underwent surgical treatment.
OBSERVATIONS: PSI in pediatric patients might behave differently in terms of epidem

Fifty-six adults (average age, 38 years) with active tuberculosis of the lumbar and lumbosacral spine were treated conservatively with triple chemotherapy for 12 to 18 months. The minimum followup was 3 years. Three methods of assessments of the chronologic changes of sagittal spinal deformity were used on lateral radiographs of the lumbar spine obtained with the patient standing: deformity, kyphos angles, and total lumbar lordosis. The lumbosacral joint angle was measured for tuberculosis of the lumbosacral joint. The disease healed in all patients with a minimum increase of kyphosis, althoug

We performed a randomised, controlled clinical trial to compare ambulant short-course chemotherapy with anterior spinal fusion plus short-course chemotherapy for spinal tuberculosis without paraplegia. Patients with active disease of vertebral bodies were randomly allocated to one of three regimens: a) radical anterior resection with bone grafting plus six months of daily isoniazid plus rifampicin (Rad6); b) ambulant chemotherapy for six months with daily isoniazid plus rifampicin (Amb6); or c) similar to b) but with chemotherapy for nine months (Amb9). Ten years from the onset of treatment, 9

Acute respiratory failure is more common in miliary tuberculosis than in tuberculous bronchopneumonia and also has a worse prognosis. Chronic hypercapnic respiratory failure is frequent after both spinal tuberculosis and surgical treatments for pulmonary tuberculosis. It may develop insidiously or present acutely, for instance, during a chest infection. Hypoventilation appears during REM sleep before non-REM sleep or wakefulness and is readily treatable with non-invasive ventilation. The prognosis is good even if initially tracheostomy ventilation is required temporarily.

The incidence of active tuberculosis in patients undergoing maintenance dialysis for a long time is considerably higher than that in general population. A 39-year-old male treated by hemodialysis three times a week for six months was admitted to the hospital with a painless mass palpable under his right areola. X-ray examination of chest showed a hyperintense lesion. Computed tomography revealed a cystic mass in the superior segment of inferior lobe near the thoracic vertebrae. Needle aspiration of the lesion revealed granulomas and acid-resistant bacteria. Anti-tuberculous therapy was therefo

A 46-year-old man complained fever, headache, and vertigo after he was given steroid for sudden deafness. He was diagnosed as miliary tuberculosis by his chest CT findings. After admission, 4 anti-tuberculous drugs (INH, RFP, SM, and PZA) were prescribed but his laboratory findings showed SIADH, which was difficult to treat, and steroid was readministered. Brain MRI, examined 2 months after admission, showed brain tuberculomas, and examination of cerebrospinal fluid revealed a diagnosis of tuberculous meningitis. Three months later, meningitis deteriorated transiently, however symptoms and fin

Central nervous system (CNS) tuberculosis includes three clinical entities: tuberculous meningitis, intracranial tuberculoma, and spinal tuberculous arachnoiditis. All three categories are encountered frequently in regions of the world where the incidence of TB is high. Meningeal tuberculosis is a medical emergency: it is the most severe, lethal and disabling form of tuberculosis. Early diagnosis and treatment can be lifesaving. Even, in developed countries the diagnosis of tuberculous meningitis is difficult, frequently delayed or missed, and is often not microbiologically confirmed. Here I r

Osteoarticular localization is rare and dominated by spinal disease. The talus localization of tuberculosis is rarely described in the literature; it can be secondary of direct inoculation or results from hematogenous dissemination, most of the time from lung tuberculosis. Its diagnosis is difficult; the imaging is very suggestive, but the confirmation is bacteriological and/or histological. The prognosis depends on a rapid diagnosis and the precocity of the treatment which is essentially medical.

Although tuberculosis is prevalent in Eastern Nigeria, only 5 cases of tuberculoma of the central nervous system (CNS) were recorded over a 5 year period in a hospital in which about 4000 tuberculosis patients were treated. All the patients were Igbo males. In 4 cases the tuberculoma was supratentorial and in 1 case there was an extradural extraosseous spinal tuberculoma.
