Precision cytokine modulation to overcome tumor microenvironment-driven resistance to immune checkpoint blockade.

Immune checkpoint inhibitors (ICIs) can elicit durable remissions, yet most solid tumors show primary non-response or acquired resistance because the tumor microenvironment (TME) limits T-cell priming, trafficking and effector fitness. Cytokines orchestrate these barriers by shaping spatial immune architecture, regulating suppressive myeloid programmes and inducing inhibitory ligands such as PD-L1. In this Review, we synthesize mechanistic and clinical evidence and propose a push-pull framework for cytokine modulation during checkpoint blockade: locally amplifying Th1/CTL-supporting signals (I…

Nanosystem-Mediated Intercellular Crosstalk Modulation in the Tumor Microenvironment for Enhanced Cancer Immunotherapy.

Intercellular crosstalk in the tumor microenvironment (TME) is a central determinant of cancer progression and therapy response, in which malignant, immune, stromal and vascular compartments exchange information mainly through extracellular vesicles, signaling pathways, and metabolic cues. These communication circuits can enforce immune exclusion, sustain immunosuppressive cell states and enable adaptive resistance, thereby limiting the durability of otherwise effective therapies. Nanosystems have emerged as enabling tools to modulate this crosstalk across major tumor-centered interaction axes…

Targeting the αvβ5 Integrin Modifies the TGFβ-Rich Tumor Microenvironment of Pancreatic Cancer.

UNLABELLED: Pancreatic ductal adenocarcinoma (PDAC) is notorious for its aggressive, therapy-resistant nature that is in part driven by the desmoplastic, hypoperfused, and immunosuppressive tumor microenvironment (TME). In this study, we demonstrated that the αv integrin and neuropilin-1 (NRP-1) dual targeting iRGD peptide reverses some of these TME features by inhibiting transforming growth factor β (TGFβ) activation in the tumor, a process mediated by the αvβ5 integrin. In addition to PDAC epithelial cells and fibroblasts, regulatory T cells (Treg) in PDAC tumors also expressed the αvβ5 inte…

Targeting Nicotinamide Metabolism to Reprogram the Tumor Microenvironment: Mechanisms and Novel Therapeutic Strategies.

The tumor microenvironment (TME) is characterized by heightened metabolic activity, dysfunctional vasculature, and chronic inflammation. Together, these features create an ecosystem that supports tumor progression. This review examines how nicotinamide (NAM) metabolism, mediated through its key enzymes Nicotinamide phosphoribosyl transferase (NAMPT) and Nicotinamide N‑methyltransferase (NNMT), may contribute to malignant remodeling of the TME at three interconnected levels: (1) fueling the hypermetabolic state of cancer cells and contributing to TME acidification; (2) modulating tumor angiogen…

CXCR2-mediated metabolic interaction between prostate cancer cells and the immunosuppressive tumor microenvironment.

BACKGROUND: Neuroendocrine prostate cancer (NEPC) is characterized by strong immune evasion and profound metabolic reprogramming. A high regulatory T cell (Treg)/CD8+ T cell ratio and an increased proportion of M2/M1 tumor-associated macrophages (TAMs) in the NEPC tumor microenvironment (TME) are associated with poorer progression-free survival, a phenomenon linked to lipid accumulation within the TME. Understanding the regulatory mechanisms governing both the metabolic and immune landscapes of NEPC is critical. METHODS: To investigate these mechanisms, prostate cancer cell lines and patient …

Single-cell transcriptomic characterization of the tumor microenvironment in prostate cancer bone metastases.

BACKGROUND: Prostate cancer (PCa) bone metastases cause significant morbidity and mortality in advanced disease. The tumor microenvironment (TME) of bone metastases drives disease progression and therapeutic resistance, yet comprehensive characterization of its cellular heterogeneity remains limited. This study aims to characterize cellular populations and molecular signatures of PCa bone metastases using single-cell RNA sequencing (scRNA-seq) data from the Gene Expression Omnibus (GEO) database. METHODS: scRNA-seq data from PCa bone metastasis samples were obtained from GEO. Quality control,…

Tumor microenvironment-specific nanomedicine: from biology-driven to multi-omics-guided precision engineering.

The tumor microenvironment (TME) is a structurally and functionally heterogeneous ecosystem in which extracellular matrix (ECM) remodeling, vascular-lymphatic dysfunction, hypoxia, acidosis, and spatially organized immunosuppressive networks jointly constrain nanoparticle accessibility, intratumoral distribution, and therapeutic efficacy. Conventional nanomedicine strategies based on passive accumulation, ligand-mediated targeting, and stimulus-responsive release have produced important advances, but they remain limited by empirical design principles that often overlook interpatient and intrat…

Tumor Microenvironment-Responsive Polymeric Nanocarriers for the Treatment of Triple-Negative Breast Cancer.

Triple-negative breast cancer (TNBC) is an aggressive and heterogeneous subtype lacking effective targeted therapies. Its tumor microenvironment (TME) exhibits distinct features, including acidity, redox imbalance, elevated reactive oxygen species (ROS), and hypoxia, which provide exploitable triggers for targeted drug delivery. Stimuli-responsive polymeric nanocarriers have emerged as promising platforms that enable spatiotemporally controlled and site-specific therapeutic release in response to these endogenous cues, as well as exogenous stimuli such as temperature and light. These systems i…

Ecosystem-matched reprogramming of the tumor-immune microenvironment to overcome cancer immunotherapy resistance.

Immune checkpoint blockade has redefined cancer therapy, yet durable responses remain constrained by resistance states that arise from coordinated tumor-cell, immune, stromal, vascular, lymphatic, and metabolic programs rather than from a single defective pathway. This review develops a translational framework in which tumor-intrinsic immune invisibility, myeloid and regulatory lymphocyte suppression, defective dendritic-cell priming, stromal and vascular exclusion, lymphatic control of antigen drainage, metabolic stress, and ILC2/type 2 immune plasticity are interpreted as interdependent ecos

Remodeling the tumor immune microenvironment: mechanisms of crosstalk between regulated cell death macrophages.

Although advances have been made in tumor immunotherapy, the immunosuppressive tumor microenvironment and treatment resistance still limit durable clinical benefit. Rather than functioning exclusively as terminal cytotoxic phenomena, regulated cell death (RCD) pathways generate surface cues, soluble mediators and cellular cargo that reshape immune responses. Here, we review the reciprocal interactions linking tumor-associated macrophages (TAMs) with apoptosis, necroptosis, pyroptosis, autophagy-related regulation, ferroptosis and cuproptosis. We examine how death execution, the kinetics and ma…

Multimodal therapeutic strategies against gastric cancer: from conventional treatments to tumor microenvironment targeting.

Gastric cancer is one of the most common malignant tumors of the digestive system, with persistently high global morbidity and mortality rates. The multi-level heterogeneity of the gastric cancer tumor immune microenvironment (TIME) is closely associated with treatment efficacy and prognosis. This heterogeneity is reflected not only in the types and functions of various cells within the microenvironment but also in multiple aspects such as molecular profiles, metabolic pathways, and the spatial distribution of tumor cells. Currently, the interaction between gastric cancer and its microenvironm

TP53-mutated MDS and AML: immune dysregulation, tumor microenvironment, and emerging therapeutic strategies.

TP53 mutations drive oncogenesis and therapeutic resistance in myelodysplastic syndromes (MDSs) and acute myeloid leukemia (AML), impairing p53-regulated functions such as apoptosis, immune surveillance, and genomic stability, leading to immune evasion and metabolic reprogramming. The tumor microenvironment in TP53-mutated MDS and AML fosters leukemic progression through cytokine dysregulation, altered metabolism, and immune suppression. Current therapies, including chemotherapy and hypomethylating agents, offer limited efficacy, resulting in poor overall survival rates for these high-risk pat…

A Biomimetic Nanomedicine for Remodeling the Immune Microenvironment to Potentiating Anti-Tumor Therapy.

Systemic therapeutic modalities that possess the capability to activate immune response represent efficacious strategy for managing advanced hepatocellular carcinoma (HCC). Nonetheless, the therapeutic efficacy is substantially constrained by the tumor immunosuppressive microenvironment (TIME) engendered by hypoxia and lactate accumulation in the tumor region. Here, an efficient therapeutic strategy based on a biomimetic nanomedicine is established to remodel TIME and stimulate immune activation for HCC treatment. First, Glucose oxidase (GOx)-MnO2 self-assembled particles (GM) are synthesized

Immunological Landscape and Molecular Therapeutic Targets of the Tumor Microenvironment in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the most common liver cancer, with poor survival rates in advanced stages due to late diagnosis, tumor heterogeneity, and therapy resistance. The tumor microenvironment (TME) in HCC has a crucial role in tumor progression, characterized by a complex interaction of immune cells, stromal components, and immunosuppressive signaling pathways. Chronic inflammation driven by viral infections, metabolic dysfunction, and alcohol consumption triggers an immunosuppressive TME, promoting immune evasion and tumor growth. Immune cell populations, such as myeloid-derived su…

Tumor microenvironment modulation by SERPINE1 increases radioimmunotherapy in murine model of gastric cancer.

An elevated extracellular matrix (ECM) and interstitial fluid pressure (IFP) in gastric cancer limits the targeting of HER2-expressing cells when radioimmunotherapy (RIT) with 64Cu-trastuzumab (64Cu-TRZ) is utilized. Here, we used Losartan (LOS) to downregulate ECM and IFP in gastric cancer mice model. In our study we treated the gastric cancer mice model with a dose of 40 mg/kg of LOS. We found that the LOS treatment increases a twofold higher Alexa-647-TRZ accumulation which significantly enhanced 64Cu-TRZ. We determined that the LOS-treated samples exhibited reduced mRNA and protein express

A novel tumor microenvironment-related gene signature for prognostic prediction of intrahepatic cholangiocarcinoma.

Intrahepatic cholangiocarcinoma (ICCA) is a highly aggressive bile duct cancer with limited treatment options and poor prognosis. This study aims to develop a gene signature-based model to predict ICCA patient survival. We identified differentially expressed genes (DEGs) from an ICCA mRNA microarray dataset and applied survival, Cox, and LASSO regression analyses to select key genes for constructing the GPSICCA risk score model. The model was then validated in two additional ICCA cohorts, and its relationship with the tumor microenvironment (TME) was analyzed. Additionally, multiplex fluoresce…
