Paediatric surgery and anaesthesia in south-western Uganda: a cross-sectional survey.

OBJECTIVE: to study paediatric surgery rates in south-western Uganda, compare them to rates in England, and determine if existing surgical facilities and workforce meet World Health Organization (WHO) standards. METHODS: to obtain information on surgical facilities and workforce, we conducted a cross-sectional survey of all hospitals performing major surgery in 14 districts of south-western Uganda in 2007-2008. Using theatre logbook data, we determined the surgical rates, types of surgery performed and in-theatre surgical outcomes. FINDINGS: of 72 hospitals surveyed, 29 were performing major…

A case-control study of ocular surface squamous neoplasia (OSSN) in Uganda.

HIV increases the risk of OSSN. Here we investigate other factors in a case-control study from Uganda with 318 cases (48 CIN I, 66 CIN II, 81 CIN III and 123 with invasive disease) and 762 controls. Initial analyses were stratified by HIV serostatus (204 cases and 202 controls were HIV seropositive), but since findings were similar in infected and uninfected people, the combined results are presented here. The risk of OSSN increased with increasing time spent in direct sunlight (p(trend) = 0.003, adjusted for age, sex, residential district and HIV serostatus): compared to those who reported sp…

Brucellosis seroprevalence in livestock in Uganda from 1998 to 2008: a retrospective study.

A total of 17,359 samples were analysed serologically, of which 1,061, 15,758 and 585 samples were from Makerere, Entebbe and Tororo laboratories, respectively, were used to determine the seroprevalence of brucellosis. The overall seroprevalence of brucellosis was 10% while from individual laboratories was 38%, 32% and 7% for Makerere, Entebbe and Tororo laboratories, respectively. Majority of these positive brucellosis test results were in the cattle corridor with P value = 0.399. There were significant differences in brucellosis seroprevalence among species (P value = 0.014). The trends of b

The risk of return: intimate partner violence in northern Uganda's armed conflict.

The physical and psychological consequences of armed conflict and intimate partner violence are well documented. Less research focuses on their intersection and the linkages between domestic violence, gender-based discrimination, and the structural violence of poverty in armed conflict. This paper describes emerging themes from qualitative interviews with young women who have returned from abduction into the Lord's Resistance Army in northern Uganda, many of whom were forcibly given as "wives" to commanders. Their interviews reveal multiple levels of violence that some women experience in war,…

Outcome of cesarean section in asymptomatic HIV-1 infection in Kampala, Uganda.

AIM: Sepsis remains a major cause of morbidity and mortality in Mulago Hospital, Kampala, Uganda, following cesarean section (CS). The present study was designed to assess whether asymptomatic HIV infection could be contributing to the increased morbidity following emergency CS in the form of wound and genital tract infection (GTI) with consequent prolonged stay in the hospital. METHODS: Five hundred patients who had undergone emergency lower segment CS had their blood taken to check for HIV-1 serostatus on the first postoperative day. The patients were followed up for evidence of wound sepsi…

Spatial risk models for human plague in the West Nile region of Uganda.

The West Nile region of Uganda represents an epidemiologic focus for human plague in east Africa. However, limited capacity for diagnostic laboratory testing means few clinically diagnosed cases are confirmed and the true burden of disease is undetermined. The aims of the study were 1) describe the spatial distribution of clinical plague cases in the region, 2) identify ecologic correlates of incidence, and 3) incorporate these variables into predictive models that define areas of plague risk. The model explained 74% of the incidence variation and revealed that cases were more common above 1,3…

Flea diversity and infestation prevalence on rodents in a plague-endemic region of Uganda.

In Uganda, the West Nile region is the primary epidemiologic focus for plague. The aims of this study were to 1) describe flea-host associations within a plague-endemic region of Uganda, 2) compare flea loads between villages with or without a history of reported human plague cases and between sampling periods, and 3) determine vector loads on small mammal hosts in domestic, peridomestic, and sylvatic settings. We report that the roof rat, Rattus rattus, is the most common rodent collected in human dwellings in each of the 10 villages within the two districts sampled. These rats were commonly …

Infection with human papillomavirus and HIV among young women in Kampala, Uganda.

BACKGROUND: Information on the prevalence of cervical infection with different human papillomavirus (HPV) types among young women is essential to support the introduction of HPV vaccine in Uganda. METHODS: Cross-sectional findings are presented from a cohort study of 1,275 sexually active women aged 12-24 years seeking health services at a clinic for teenagers in Kampala, Uganda. We assessed the presence of 39 HPV types by use of highly sensitive polymerase chain reaction assays. RESULTS: The prevalence of HPV infection was 74.6%, and the prevalence of human immunodeficiency virus infection …

Women's health in rural Uganda: problems, coping strategies, and recommendations for change.

This study draws upon 63 semi-structured interviews with rural women in central Uganda to elicit women's health problems, coping mechanisms, and recommendations for improved health services. The health problems most commonly reported by the women were sexually transmitted diseases, especially syphilis, abdominal pain, genital sores, and mental stress. Women indicated several barriers to obtaining health care, such as inaccessibility to health-care facilities, lack of time and money, and dependence on men for permission to leave the home. In response, they had devised several coping strategies …

Understanding the barriers to clubfoot treatment adherence in Uganda: a rapid ethnographic study.

PURPOSE: The Ponseti method has been demonstrated to be an effective, low-technology method of correcting congenital clubfoot. The purpose of this paper is to identify barriers to adherence to the Ponseti method of clubfoot treatment in Uganda. Understanding of barriers underlies successful and culturally appropriate approaches to program implementation. METHOD: A qualitative study (rapid ethnographic study), using semi-structured interviews, focus groups and observation, was conducted. Interviews were conducted with parents of children with clubfoot (42), adults with clubfoot (2), community …

Nutritional status of young children in AIDS-affected households and controls in Uganda.

This study conducted in Uganda assessed the nutritional status of young children and their disease history in the 3-month period before the study. Two groups of children were randomly selected: the first group consisted of 105 children living in homes where a family member fell sick of AIDS, whereas the second group consisted of 100 children who were living in homes where nobody was affected by AIDS. Acute malnutrition (wasting) was rare. There was no difference in the severity of stunting in the two groups (Z scores, -2.1 versus -2.2, P = 0.70). In those children living in AIDS-affected homes…

A qualitative study of condom use among married couples in Kampala, Uganda.

Twenty-five years into the HIV/AIDS epidemic, condom use among married/stable couples remains low and under-researched in developing countries, even countries with high HIV prevalence. Introducing condoms into a long-standing relationship, in spite of HIV risk, is likely to be awkward. We conducted a qualitative study in Kampala, Uganda, with 39 couples reporting 100% condom use in the previous three months. The women were recruited from among women in a clinical trial who were using condoms and whose partners also agreed to participate. Twenty-two of the women and six of the men reported havi…

Audit of severe maternal morbidity in Uganda--implications for quality of obstetric care.

BACKGROUND: For every maternal death, there are probably 100 or more morbidities, but the quality of health care for these women who survive has rarely been an issue. The purpose of this study is to explore audit of severe obstetric morbidity and the concept of near miss in four referral hospitals in Uganda. METHODS: This was an exploratory systematic enquiry into the care of a subset of women with severe morbidity designated as near miss cases by organ failure or dysfunction. Patient factors and environmental factors were also explored. Data were abstracted from clinical records and from int…

Refusal to take ivermectin: the associated 'risk' factors in Hoima district, Uganda.

Community-directed treatment with ivermectin (CDTI), for the control of onchocerciasis, was launched in Uganda in 1997. In East Africa, as in other endemic areas, the refusal to take the drug reduces treatment coverage and therefore poses a serious threat to attempts to eliminate onchocerciasis as a disease of public-health importance. In early 2003, an attempt was therefore made to determine the factors associated with refusal to take ivermectin during mass treatment in 2002, by interviewing the individuals, from three Ugandan villages with CDTI, who had been eligible to receive ivermectin. T…

Maternal self-medication and provision of nevirapine to newborns by women in Rakai, Uganda.

To assess the effectiveness of maternal self-administration of nevirapine for prevention of mother-to-child transmission (MTCT) of HIV, we conducted a program to provide maternal and newborn doses of nevirapine to pregnant women in rural Uganda. Women provided blood for HIV testing and were offered voluntary counseling and testing (VCT) during annual community HIV surveys. HIV-positive women who accepted VCT were offered nevirapine tablets and syrup. Blood samples were collected postpartum from women and their babies. Infants were tested for HIV by polymerase chain reaction (PCR), and a subsam…
