BACKGROUND: Water-induced pruritus is characterized by the development of intense and widespread itching after contact with water at any temperature and without observable skin lesions. Around 40-52% of patients with polycythaemia vera (PV) have water-induced pruritus, and more than 20% of the patients continue with symptoms despite an adequate control of the underlying disease. The aetiology is unknown and treatment is often unsuccessful. We report a patient with a haematologically controlled polycythaemia vera and water-induced pruritus that responded to phototherapy.

Induction of tolerance by subsequent UV exposures is the most effective therapy for solar urticaria; however, it is time-consuming and takes a long time until protection is achieved. Three patients with solar urticaria were exposed to multiple UVA irradiations at 1-hour intervals per day. With this rush hardening regimen, protection was achieved within 3 days.

An isomorphic response is defined as a nonspecific skin stimulus eliciting a disease-specific skin reaction. We report on the development of the Koebner phenomenon in a 32 year old patient with psoriasis after skin testing with UVB at a dose above 0.055 J/cm2. Cutaneous T-cell lymphoma, which usually presents as mycosis fungoides or Sézary syndrome, remains a mostly incurable, yet highly treatable group of diseases. Polymorphous light eruption is an acquired photodermatosis, whose causal factors have not yet been identified. The diagnosis is based on the clinical picture and on histology. During the symptomless periods, photo-testing is the most important technique to confirm the diagnosis, since there are no specific laboratory parameters. Regarding our own patients, typical lesions could be provoked by UVA in 50%, by UVB in 20% and by both in 30%. The majority of the patients can be protected during the summer season by intermittent preventive photo(chemo)therapy.

A patient with solar urticaria induced by wavelengths 290-420 nm is reported. Wheals appeared after a few seconds of exposure to the sun; longer exposure caused general malaise and syncope.
