Case report of a very large ectasia of the urethra in a 9 years old boy without any organic conflict and without any neuro-urologic trouble. It could be a particular syndrome.

A man presenting complete traumatic disruption of the membranous urethra, with a 1 cm gap between the proximal and distal urethra, underwent successful endoscopic reconstruction ten days after the injury. When the urethral catheter was removed after fifteen days, peak flow rate was 25 cc per second. One internal urethrotomy was necessary 8 months later. Twenty months after the injury, cystography and retrograde urethrography revealed satisfactory restoration of urethral continuity. Urethral carcinoma recurrence after radical cystectomy for bladder carcinoma is a rare but devastating condition. Among 140 male patients submitted to radical cystectomy, urethral carcinoma recurrence occurred in 11. Nine of whom died. Nine other patients underwent urethrectomy at the time of cystectomy or shortly afterwards. Five are alive with no evidence of disease, 4 died of local recurrence or distant metastasis. Prophylactic urethrectomy should be performed in the presence of carcinoma in situ in the prostatic urethra or involvement of the prostatic urethra from the bladder tumor. These …

Patent urachus is a common condition in calves which is frequently associated with omphalitis. A membranous urethral diaphragm prevented closure of the urachus in a female calf. The patent urachus was complicated by an ascending infection of the intraabdominal umbilical remnants. Following surgical removal of the urachus and umbilical vessels along with transection of the membranous diaphragm the calf experienced an uncomplicated recovery. This case stresses the importance of assuring urethral patency when managing a case of patent urachus.

Effects of oxybutynin on the urinary bladder and urethra were studied in comparison with atropine and flavoxate in cats and rabbits. Oxybutynin at 10 mg/kg, i.v., significantly inhibited the bladder contractions induced by electrical stimulation of the peripheral end in pelvic nerve. Oxybutynin was about one half the potency of atropine. On the contrary, 10 mg/kg of flavoxate, i.v., showed both effects of potentiation and inhibition. A proximal hypospadias, despite multiple procedures, may remain uncorrected either totally or because of multiple fistulae. In such circumstances there is often a severe shortage of skin on the ventral surface of the penis and scrotal skin may already have been used. The microvascular transfer of a radial forearm flap may then be used and, because of the thinness of the flap, a tube may be developed from it as well as covering skin. The long vascular pedicle ensures a good vascular input at the recipient site by anastomosis of the radial vessels to the femoral vessels. Two cases are presented

Mucinous papillary adenocarcinoma of the bulbous urethra developed in a 40-year-old man with congenital stricture. The tumor appeared as a localized polypoid exophytic mass without evidence of submucosal invasion (Stage 0). The tumor was determined not to be of prostatic origin clinically and by absence of immunoperoxidase staining for prostatic acid phosphatase, prostate specific antigen, and carcinoembryonic antigen. Five incontinent girls between 4 and 18 years old were made continent by constructing in each case a long extension of the urethra up to the clitoris. Previous operations at the proximal end of the urethra, to narrow the bladder neck, had not achieved continence. Lengthening the urethra was accomplished by tubularizing the anterior vaginal wall and covering that with a generous pedicle flap from the perineum. The influence of electro-acupuncture on the sphincter of the female urethra was checked on 20 patients with stress incontinence without gross anatomic variation. This was determined by means of simultaneous cysto-urethromethrometry. After 30 minutes of electric stimulation through acupuncture needles which were placed into the skin of the lower legs and lower abdomen, a significant increase in the so-called "closing pressure" was found. During 1974, 80 women with symptoms of relative stress incontinence underwent cystotonometric, sphincterometric and urethrometric examinations. The average basal bladder pressure, sphincter resistance, urethral length with the patient in the lying position and functional urethral length (difference between urethral length in the lying and the upright positions) were measured. Twenty-three patients with urethral loss have been treated by the simple creation ofa "spout" from the bladder wall. The procedure is suggested for the surgical treatment of lymphopathia venereum and for consideration in certain other conditions when urinary diversion is unacceptable.
