Treatment of bladder dysfunction and high grade vesicoureteral reflux does not influence the spontaneous resolution rate.

PURPOSE: We investigated whether the treatment of bladder dysfunction in infants with congenital high grade vesicoureteral reflux could influence the spontaneous resolution rate of the reflux and the number of recurrent urinary tract infections. MATERIALS AND METHODS: A total of 115 infants with high grade vesicoureteral reflux were included in a followup study of bladder function and reflux resolution between 1993 and 1999. The present study deals with 20 of these infants with mainly grade V reflux, most of whom had recurrent urinary tract infections in combination with high post-void residu

Risk factors for catheter-associated urinary tract infection in an intensive care unit: a matched case-control study.

OBJECTIVE: Catheter-associated urinary tract infections (CAUTIs) are common healthcare-related infections in intensive care units (ICUs). This study investigated the risk factors for CAUTIs in critically ill patients. METHODS: This study was a single-centre, retrospective, matched case-control study of patients undergoing indwelling catheterization in the ICU from December 1, 2016, to October 31, 2021. Patients with catheterizations were matched 1:4 with controls that were hospitalized in the ICU during the same period (with a difference in admission time of no more than two months). RESULTS

The efficacy of tolterodine for prevention of catheter-related bladder discomfort: a prospective, randomized, placebo-controlled, double-blind study.

UNLABELLED: Bladder discomfort secondary to an indwelling urinary catheter is distressing, particularly for patients awakening from anesthesia. We sought to discover the incidence and severity of bladder discomfort in patients who were catheterized intraoperatively and to evaluate the efficacy of tolterodine, a pure muscarinic receptor antagonist, in preventing this. Two-hundred-fifteen consecutive adult patients, ASA physical status I and II, either sex, undergoing urologic surgery requiring bladder catheterization were enrolled. Group C (control, n = 165) received placebo and group T (tolter

Micronucleus frequencies in urothelial cells of catheterized patients with chronic bladder inflammation.

Epidemiological studies suggest an association between chronic inflammation and increased risk for cancer, although the mechanism underlying this relationship is unresolved. In the present study, we test the hypothesis that DNA damage is induced in the epithelium of tissues during such inflammation by products of activated inflammatory cells. Individuals on long-term indwelling urinary catheterization were used as a study population. These individuals have chronic bladder inflammation and, as a population, an increased risk for bladder cancer. Urine of 29 patients and 26 age-matched non-cathet…

Urological management in the patient with acute spinal cord injury.

Understanding the neurophysiology of micturition has led to pharmacologic manipulation and improvement in rates of mortality and morbidity. Use of intermittent self-catheterization and the artificial urinary sphincter (AUS) has improved the "quality of life" of these patients. Implantation of various penile prostheses has improved sexual rehabilitation. Urinary diversion is the "last resort" surgery.

Sacral Neuromodulation in Non-Neurogenic Lower Urinary Tract Symptoms: Clinical Efficacy and Safety.

OBJECTIVE: The aim of this study is to evaluate the effectiveness of sacral neuromodulation (SNM) on clinical symptoms, lower urinary tract function, and objective measurements in patients diagnosed with overactive bladder (OAB) and chronic nonobstructive urinary retention (CNOUR), and to identify factors affecting treatment response. MATERIALS AND METHODS: This multicenter retrospective chart review included patients diagnosed with OAB and CNOUR who underwent SNM between 2015 and 2025 at four tertiary referral centers. All patients underwent a standard two-stage SNM protocol. Clinical evalua

Chromaturia and Providencia: A Case Report on Purple Urine Bag Syndrome.

Purple urine bag syndrome (PUBS) is an uncommon condition typically observed in chronically ill, catheterized patients. This case report describes a 54-year-old Indian man with a history of recurrent strokes and long-term urinary catheterization who presented with lower abdominal discomfort and purple discoloration of the urine. Microbiological analysis revealed Providencia rettgeri, confirming the diagnosis of PUBS. The pathogenesis of PUBS involves the interaction between tryptophan and bacterial enzymes, resulting in the formation of colored compounds. Early identification of PUBS facilitat…

Making the hospital safer for older adult patients: a focus on the indwelling urinary catheter.

The needs of hospitalized geriatric patients differ from the needs of hospitalized younger adults. In an attempt to improve systems of care … unnecessary use of the IUC. Thoughtful and judicious use of the IUC, such as minimizing the use of urinary catheterization, either by not inserting an IUC or by removing it as soon as it is no longer needed, will most likely reduce inpatient morbidity and improve the health of the hospitalized older adult.

Randomized controlled trial to evaluate a novel two-catheter technique for urethral catheterization in anesthetized healthy female cats and small dogs.

OBJECTIVE: To evaluate a novel 2-catheter technique for urethral catheterization in female cats and small dogs and compare the time required for and success rates achieved by use of the novel technique versus traditional methods (blind technique in cats and digital palpation in dogs) as performed by personnel (catheter placers [CPs]) with different levels of experience in urinary catheter placement. ANIMALS: 39 healthy sexually intact female animals (24 cats and 15 dogs weighing < 10 kg). PROCEDURES: 2 CPs were board certified in veterinary surgery, 1 of whom had experience with the novel te

Determining the noninfectious complications of indwelling urethral catheters: a systematic review and meta-analysis.

BACKGROUND: Although the epidemiology of catheter-associated urinary tract infection is well-described, little is known about noninfectious complications resulting from urethral catheter use. PURPOSE: To determine the frequency of noninfectious complications after catheterization. DATA SOURCES: MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, CINAHL, Conference Papers Index, BIOSIS Previews, Scopus, and ClinicalTrials.gov were searched for human studies without any language limits and through 30 July 2012. STUDY SELECTION: Clinical trials and observational studies assessing

Prevalence and risk factors of urinary incontinence and bladder retention in gastric bypass surgery: a cross-sectional study.

Morbid obesity is a known risk factor for developing symptoms of urinary incontinence. However, it is not known if this leads to a high prevalence of perioperative incontinence and bladder retention after bariatric surgery because routine use of bladder catheterization is usual during this kind of surgery. The study was conducted at a general hospital in the Netherlands. Sixty morbidly obese female patients undergoing laparoscopic gastric bypass surgery were included in the present study. Preoperative urinary incontinence and risk factors were investigated by use of questionnaires. The followi

A 66-Year-Old Woman with Achondroplasia and Quadriparesis with an Ovarian Cyst Erroneously Diagnosed as Urinary Retention by an Automated Bladder Scan.

BACKGROUND Automated bladder scanning has become a principal tool in the assessment and management of chronically debilitated patients residing in skilled nursing facilities, hospices, and acute inpatient settings. To a large extent, the bladder scan, generally performed by nursing staff, has replaced physical examination while addressing the differential diagnoses of anuria or voiding disturbances that require consideration of urinary catheterization. Health care providers can quickly master this easily performed technique, and currently, due to confidence in the bladder scan finding, physica…

[Continent urinary diversion using a tubulized sigmoid segment. An alternative to trans-appendicular diversion].

OBJECTIVE: The authors propose the use of a sigmoid tube reimplanted submucosally in the bladder and brought out onto the skin in the midline or in the umbilicus as a method of continent urinary diversion allowing urinary catheterization several times a day when the appendix cannot be used. MATERIALS AND METHODS: Three adolescents with neurogenic bladder were treated according to this procedure; the summit of the sigmoid colon loop was selected to form a continent tube from a segment 4 cm wide, opened along its antimesenteric border and sutured longitudinally. RESULTS: The postoperative cour…

[Determination of residual urine volume using a portable ultrasound apparatus in a rehabilitation center for spinal lesions].

Due to the high incidence of voiding dysfunction (patients with SCI) in the rehabilitation setting, a safe, efficient method for monitoring residual bladder volume is needed. A controlled trial was performed to compare the use of a portable ultrasound unit that calculates bladder volume to catheterization regarding complications, speed of usage and patients feelings. Urethral catheterization, the standard method for measurement of bladder volume is associated with patient discomfort plus risks of urethral trauma and urinary tract infection. [Two Cases of Spontaneous Rupture of the Urinary Bladder Associated with Radiation Cystitis, Repaired with Omentum Covering].

We experienced two cases of spontaneous rupture of the urinary bladder associated with radiation cystitis. Case 1 and Case 2 were a 45-year-old female and a 71-year-old female, respectively. Both of them were admitted to our hospital with complaints of high grade fever and severe lower abdominal pain. They had a past history of total hysterectomy followed by radiationtherapy more than10 years ago. The cystography and the subsequent computed tomography revealed spontaneous rupture of the urinary bladder. We performed the operationto repair the perforationsite with omentum covering. Case 1 had a

Covered cloacal exstrophy--a poorly recognized condition: hints for a correct diagnosis.

INTRODUCTION: Covered cloacal exstrophy requires a high index of suspicion for its diagnosis. Low implantation of the umbilical cord, separated pubic bones, and anorectal malformation are the most common signs. METHODS: Thirty-one patients with this defect were retrospectively analyzed. RESULTS: Besides the anorectal malformation, the patients had important unique anatomic findings, including a colon shorter than 20 cm (17 patients) and absent bladderneck (27 patients). Twenty-four patients underwent a colonic pullthrough; of those, only 5 of them have voluntary bowel movements. Twelve patie

Herpes zoster induced neuropathic bladder--a case report.

Herpes zoster infection involving the sacral dermatomes has been associated with bladder dysfunction and, although rarely, with acute urinary retention. Less than 150 cases have been reported in the literature. After reviewing our institute's chart records covering a period of time dating from 1991 to 2001, we found that three of our patients had developed acute urinary retention following herpes zoster skin lesions of the S2-4 dermatomes. Herein we report our findings. These three patients had previously been found to have normal voiding status. However, at the time of complaint urodynamic st
