Antituberculosis therapy in the treatment of peripheral uveitis.

Tuberculous uveitis usually appears as chronic anterior uveitis or disseminated choroiditis. From 1982 to 1989, we conducted a retrospective study of 23 patients with presumed tuberculous uveitis. All patients had a positive tuberculin purified protein derivative skin test. The diagnosis was based on history, positive skin test, and physical examination. We excluded other conditions that could induce uveitis, based on the absence of signs, symptoms, or laboratory results suggesting any other diagnosis. Tuberculous uveitis was also considered in the differential diagnosis when progressive ocula…

[Alternatives to steroid therapy in chronic uveitis].

In a search for alternative therapeutic methods other than corticosteroids and cytostatics, the effect of a dialyzable leukocyte extract (DLE), the antimetabolite 5-fluorouracil and the immunosuppressive agent cyclosporin A in corticosteroid-resistant idiopathic uveitis was studied. When DLE was administered to 26 patients who had uveitis forms with exogenous triggering (e.g., infection), as well as forms with an autoimmune background, there was a reduction in the number and duration of recurrences and a statistically proven prolongation of the inflammation-free intervals. This was particularl…

Cyclosporin A in the treatment of severe Behçet's uveitis.

Twelve patients with active Behçet's uveitis with marked deterioration of visual acuity in at least one eye were treated with cyclosporin A (CyA). An initial improvement in the severity of ocular inflammation and systemic features occurred in all cases and persisted until the dose was reduced or the drug withdrawn when a rapid recurrence of symptoms was noted. The visual acuity also initially improved in ten patients and this was maintained in seven cases until the dose of CyA was reduced. At this time, acuity was unchanged in two patients and was worse in three others-two of the latter as a r…

[Physiopathology of uveitis: the place of the dental examination].

The four main types of uveitis: anterior, posterior, intermediary and total, are related to principal major physiopathologic mechanisms: immunity disease with local production of antibodies; hypersensitivity disorder with persistence of sensitized cells in the eye. Etiologic agents of uveitis are far from being understood, continued knowledge of the mechanisms involved revealing their increasingly multifactorial nature. Detection of dental foci and their eradication are still necessary in 1985 for routine screening and treatment of uveitis.

[Interpretation of electroretinographic findings in posterior uveitis].

The present paper deals with the electroretinographic findings in 30 patients with inflammatory or postinflammatory disturbances of the fundus. The alterations of the scotopic ERG depend on the localization and extent of the disease. The individual components of the ERG are affected differently. It is evident that in a high percentage of patients with unilateral disease the ERG of the fellow eye was affected as well. This might be due to an immunologic reaction. It is pointed out that further investigations are required in order to resolve the problems involved.

Vitreous biopsy in uveitis of unknown cause.

We employed vitreous biopsy (needle aspiration and pars plana vitrectomy) in eight cases of patients with uveitis of unknown etiology. The results of cytological and culture analysis provided the diagnosis of disseminated histoplasmosis in one patient and ocular reticulum cell sarcoma in another patient. In six patients, no specific, treatable causative agents or conditions were identified. In two of these patients, however, various etiologic agents were ruled out, which provided a basis for excluding certain therapeutic options. Where chronic inflammatory cells were seen in the vitreous, the …

HL-A 27 and acute anterior uveitis.

51 (30 men and 21 women) of 90 consecutive patients with acute non-granulomatous anterior uveitis were HL-A 27 positive. This frequency of 55-7 per cent compares with 8-2 per cent in controls. Twenty-three patients (18 men and 5 women) had in addition evidence of systemic disease, including ankylosing spondylitis, sacroiliitis and Reiter's syndrome, sometimes associated with psoriasis. Twenty-eight of 63 patients without evidence of systemic disease were HL-A 27 positive, suggesting that the uveitis in many of these cases has a similar aetiology to those with rheumatic disease. The uveitis ass…

Epidemiology of uveitis (2013-2015) and changes in the patterns of uveitis (2004-2015) in the central Tokyo area: a retrospective study.

BACKGROUND: The distribution of uveitis varies with genetic, ethnic, geographic, environmental, and lifestyle factors. Epidemiological information about the patterns of uveitis is useful when an ophthalmologist considers the diagnosis of uveitis. Therefore, it is important to identify the causes of uveitis over the years in different regions. The purposes of this study were to characterize the uveitis patients who first arrived at the University of Tokyo Hospital in 2013-2015, and to analyze the changes in the patterns of uveitis from 2004 to 2012 to 2013-2015. METHODS: We retrospectively ide…

Epidemiology of Uveitis, Caused by HTLV-1, Toxoplasmosis, and Tuberculosis; the Three Leading Causes of Endemic Infectious Uveitis in Japan.

Ethnicity, geography, and socioeconomic factors are among the aspects influencing the prevalence of uveitis in specific areas and countries. Human T-lymphotropic virus type 1-associated uveitis and ocular toxoplasmosis are endemic to Southern Kyushu, the southern-most region of Japan. Recent reports have postulated that the prevalence of intraocular tuberculosis is increasing in Tokyo. This review focuses on local factors that affect the three major vectors for infectious endemic uveitis in Japan, as well as their routes of transmission and factors for improving diagnoses. This information wil…

Prevalence of uveitis in an outpatient juvenile arthritis clinic: onset of uveitis more than a decade after onset of arthritis.

PURPOSE: To determine the prevalence and severity of uveitis in an outpatient pediatric arthritis clinic in the midwestern United States during the 1990s. METHODS: The prevalence and clinical characteristics of uveitis were studied retrospectively for all children diagnosed with arthritis at Shriner's Hospital for Crippled Children and followed by the pediatric rheumatology and ophthalmology units of the St Louis Children's Hospital between 1992 and 1995. RESULTS: Seven children (9%) developed uveitis in a population of 78 patients with juvenile arthritis. Six of the seven children were fema…
