First report of supraventricular tachycardia after intravenous pulse methylprednisolone therapy, with a brief review of the literature.

The occurrence of supraventricular tachycardia after high-dose intravenous methylprednisolone pulse therapy (HIVMPT) in a patient with active rheumatoid arthritis is described for the first time. This case report further expands the range of arrhythmias that can occur with HIVMPT. Other arrhythmias previously reported to occur after HIVMPT include atrial fibrillation, atrial flutter, junctional rhythm, and ventricular tachycardia. To the best of our knowledge, supraventricular tachycardia has not been reported previously, although severe bradycardia, hypotension, asystole, cardiovascular colla

Aortic regurgitation due to non-traumatic rupture of the aortic valve commissures: report of two cases.

Two cases of severe aortic regurgitation due to non-traumatic rupture of the aortic valve commissures are reported. The cause of rupture was hypertension in one patient, but it could not be identified in the other, where microscopic examinations of the aortic wall and the aortic cusps showed no particular pathologic changes. M-mode echocardiography revealed enlargement of the left ventricle, diastolic flutter of the anterior mitral leaflet and diastolic separation of the closure line of the aortic cusps in both patients. Two-dimensional echocardiography showed a downward displacement of the pr

Repair of left atrial abnormalities in children by the superior approach.

We have used the superior approach through the dome of the left atrium to repair cardiac lesions in the left atrium in 15 children ranging from 3 months to 17 years old. The single hospital death occurred in a 16-month-old infant with levo (L) transposition of the great arteries, Ebstein's malformation of the left atrioventricular valve, and ventricular septal defect. Exposure of the intraatrial structures was excellent, injury to adjacent cardiac structures did not occur, and hemorrhage from the left atrial suture line was not a problem. Though a few transient arrhythmias were seen, all survi

Intracardiac electrophysiologic techniques in recurrent syncope of unknown case.

Twenty-five patients with recurrent episodes of syncope, unexplained despite thorough medical and neurologic evaluation, underwent intracardiac electrophysiologic study with programmed stimulation. Electrophysiologic study yielded a presumptive diagnosis in 17 patients: nine with rapid ventricular tachycardia by programmed stimulation, three with intra-His conduction delays, one with symptomatic atrial flutter, one with sick sinus syndrome, and three with persistent hypervagotonia manifested as atropine-reversible prolongation of atrioventricular nodal refractoriness. Therapy based on these fi

Emergency zero-fluoroscopy catheter ablation for refractory ventricular tachycardia in third-trimester pregnancy: a case report.

BACKGROUND: The incidence of symptomatic arrhythmias during pregnancy has markedly increased in recent years. Atrial fibrillation/flutter and life-threatening ventricular arrhythmias are of particular concern due to their association with elevated maternal and fetal risks. Cardioversion, antiarrhythmic drugs, or catheter ablation are treatment options for hemodynamically unstable arrhythmias. However, indications of therapeutic options and standardized protocols remain scarce due to the limited shared clinical experiences and evidence. CASE SUMMARY: On 17 June 2025, our team managed a 32-week

Intraoperative sudden arrhythmias in cervical spine surgery adjacent to the stellate ganglion: A case report.

BACKGROUND: Atrial arrhythmias such as paroxysmal supraventricular tachycardia (PSVT) and atrial flutter (AF) are common in the perioperative setting. They commonly resolve spontaneously. However, occasionally, they may continually progress to fatal arrhythmias or cause complications. Therefore, prompt and appropriate management is important. CASE SUMMARY: A 46-year-old female patient diagnosed with cervical C6-7 radiculopathy characterized by decreased sensation in the right third, fourth and fifth fingers underwent C6-7 anterior cervical disc fusion surgery. Electrocardiography showed PSVT

A life-threatening arrhythmia detection method based on pulse rate variability analysis and decision tree.

Extreme bradycardia (EB), extreme tachycardia (ET), ventricular tachycardia (VT), and ventricular flutter (VF) are the four types of life-threatening arrhythmias, which are symptoms of cardiovascular diseases. Therefore, in this study, a method of life-threatening arrhythmia recognition is proposed based on pulse rate variability (PRV). First, noise and interference are wiped out from the arterial blood pressure (ABP), and the PRV signal is extracted. Then, 19 features are extracted from the PRV signal, and 15 features with highly important and significant variation were selected by random for…

Worsening Heart Failure and Atrial Flutter in a Patient Secondary to Cardiac Resynchronization Therapy Dyssynchrony: A Case Report.

Cardiac resynchronization therapy-defibrillator (CRT-D) and/or cardiac resynchronization therapy-pacemaker (CRT-P) play an important role in improving cardiac synchronization and reducing the risk of ventricular fibrillation arrest (VFA) in patients with severe left ventricular systolic dysfunction (LVSD). Patients with LVSD may notice worsening symptoms when CRT-D or CRT-P is in dyssynchrony. We present a case of 59-year-old patient who presented with worsening shortness of breath (SOB) and progressive exertional dyspnea for the past few weeks accompanied by pink, frothy sputum, occasional ur

Total anomalous pulmonary venous drainage beyond childhood.

We studied the anatomic characteristics and results of surgery in 27 patients with total anomalous pulmonary venous drainage who were 15 years or older between January 1997 and July 2007. Mean age was 19.7+/-11.6 years (15-48 years). The anatomic subtypes were supracardiac (n=15), cardiac (n=7), and mixed (n=5). Fourteen patients were in NYHA class II and 13 were in NYHA class III. Eleven patients had severe and the rest had moderate pulmonary arterial hypertension; six patients had significant right ventricular dysfunction. All patients underwent complete repair. A small inter-atrial communic

Effects of cavotricuspid isthmus catheter ablation on paroxysmal atrial fibrillation.

It has been demonstrated that successful cavotricuspid isthmus ablation of typical atrial flutter combined with atrial fibrillation (AF) sometimes influences the preablation history of paroxysmal AF. However, the effectiveness of only isthmus ablation on AF itself is unclear. Endocardial catheter mapping during induced AF was performed around the tricuspid annulus using duodecapolar clectrode catheters in 39 patients with drug-refractory paroxysmal AF. Isthmus ablation was performed in 16 patients (41%) in whom catheter mapping during AF showed an organized activation pattern around the tricus

Spinal fusion in Duchenne muscular dystrophy: a multidisciplinary approach.

We studied 27 Duchenne muscular dystrophy patients having spinal fusion for scoliosis. One patient died intraoperatively of cardiac arrest; all others have done well with no instances of malignant hyperthermia, postoperative ventilatory system dependence, pneumothorax, persisting infection, neurologic damage, nonunion, or pain. The anesthetic management included primarily intravenous general anesthetics with minimal myocardial depressant effects, avoiding succinylcholine and inhalation agents. Preoperative cardiac studies aided anesthetic management intra-operatively. There was an almost unive

Oral verapamil fails to prevent supraventricular tachycardia following coronary artery surgery.

A prospective randomised trial was performed on 100 patients undergoing coronary artery bypass grafting without concomitant procedure. The study group commenced oral verapamil 40 mg three times daily on the first post-operative day while the control group received no antiarrhythmic agents. The pre-operative characteristics of both groups were similar with the exception of the incidence of hyperlipidemia which was greater in the verapamil group (P = 0.04). Myocardial protection was achieved with cold crystalloid cardioplegia. Cardiopulmonary bypass times, aortic cross clamp times and graft numb

Smartwatch detection of atrial flutter and atrial fibrillation: when the apple falls far from the tree - case report.

Consumer wearables are increasingly used to document palpitations, but their algorithms are almost exclusively validated for atrial fibrillation (AF). We report a 70-year-old man with recurrent palpitations, no prior cardiovascular history, and controlled hypertension. A single-lead Apple Watch ECG classified sinus rhythm at 75 bpm, while a same-day 12-lead ECG revealed typical atrial flutter with sawtooth waves and regular atrioventricular conduction. After adequate anticoagulation, the patient underwent successful electrical cardioversion with 120 J and remains in stable sinus rhythm. This c

Resolution of tachyarrhythmia-related fetal hydrops after corticosteroid administration for fetal lung maturation.

A case of hydrops fetalis (HF) that resolved after corticosteroid therapy despite persisting fetal tachycardia of 190 bpm is reported. The fetus with confirmed normal karyotype had HF in the presence of atrial flutter and sustained ventricular rate of 200-210 bpm at gestational week (GW) 31(-6/7). Aetiologies of HF other than fetal tachyarrhythmia were unlikely in this infant. The patient's family declined pharmacological treatment with maternal digoxin and desired to continue pregnancy with only intensive monitoring of fetal condition. Maternal transplacental β-methasone was given to facilita

Vernakalant--a promising therapy for conversion of recent-onset atrial fibrillation.

BACKGROUND: This paper reviews the pharmacology, electrophysiology, efficacy and safety of intravenous and oral vernakalant hydrochloride (RSD-1235), a novel antiarrhythmic drug that has relative atrial selectivity by blocking potassium channels that are present in the atria and not the ventricle. In addition, this drug has important rate-dependent sodium channel blocking properties. METHODS: Currently, there few commercially approved intravenous antiarrhythmic agents for the conversion of atrial fibrillation. Intravenously, vernakalant has been demonstrated to be useful in terminating over 5

Association of myotonic dystrophy and sick sinus syndrome, with special reference to electrophysiological and histological examinations.

Sick sinus syndrome is a rare but potentially important cardiac disorder in patients with myotonic dystrophy. We evaluated 3 patients with myotonic dystrophy complicated with sick sinus syndrome using intracardiac electrocardiography and endomyocardial biopsy. Electrocardiography identified sinus arrest, atrial flutter and right bundle-branch block in 2 cases and marked sinus bradycardia and first-degree atrioventricular block in 1 case. Their sinus node recovery times were significantly prolonged as demonstrated by the overdrive suppression test. Two patients had Adams-Stokes syndrome and one

Diltiazem. A reappraisal of its pharmacological properties and therapeutic use.

Diltiazem is a calcium antagonist effective in the treatment of stable, variant and unstable angina pectoris and mild to moderate systemic hypertension, with a generally favourable adverse effect profile. It is also effective in terminating supraventricular tachycardia and in controlling the ventricular response to atrial fibrillation/flutter. Atrioventricular block, the risk of which may be exacerbated by concomitant beta-adrenoceptor antagonist therapy, occurs rarely as an adverse effect of diltiazem treatment. Diltiazem appears to exert complex cardioprotective effects which have been of be

[Qualitative and quantitative one- and two-dimensional echocardiography assessment of chronic aortic insufficiency].

It is reported on the results of one- and two-dimensional echocardiographic investigations, which were partly supplemented by investigations in Doppler-technique, in 20 patients with an aortic insufficiency in the clinical stage II and 20 patients with an aortic insufficiency in stage III. As comparison serve the results of the examinations of 20 normal persons. The diagnosis of an aortic insufficiency was to be made in all cases by the evidence of fluttering waves on the anterior and posterior mitral valve, partly also on the ventricular septum. The diagnosis can be ascertained by the Doppler
