Tinnitus, hearing loss, and more rarely disequilibrium are common sequela of temporal bone trauma. Hemotympanum may cause a transient and immediate conductive hearing loss. HRCT depicts ossicular dislocation (most frequently incus), producing a long-term conductive hearing loss. Labyrinthine trauma causing neurosensory hearing loss or/and acute vertigo may be depicted by MRI, showing an abnormal non-enhancing high signal T1 of the membranous labyrinthine fluid. MRI also may show low signal T1 and T2 fibrotic areas of the membranous labyrinth, especially of interest if cochlear implant surgery …

The migraine--a benign paroxysmal vertigo of childhood complex is the most frequent etiology of pediatric dizziness, with an incidence of 34.7%. We present a series of 34 children in whom this entity was diagnosed. We describe the most important characteristics and discuss the possible etiological factors. We review the theories about a common pathophysiological origin for migraine, benign paroxysmal vertigo of childhood, and paroxysmal torticollis. Evolution of these entities confirms the idea of a common origin and a different vestibular symptomatology, depending on the age of the child expe…

BACKGROUND: Even for experienced examiners quantitative and objective medicolegal assessment of vestibular vertigo is difficult. Current medicolegal opinions place disproportionate emphasis of subjective symptoms.
METHODS: Based on parameters from spontaneous and provoked vestibular nystagmus with Frenzel's glasses, objective criterias for adequate medicolegal assessment of vestibular disorders are presented. Diagrams in tabular form for unilateral and bilateral vestibular dysfunction and benign peripheral positional vertigo allow assessment of reduction in earning capacity.
CONCLUSIONS: The…

An isolated vertigo may occur in vertebrobasilar ischemia, especially as the first symptom of cerebellar ischemia or basilar artery occlusive disease. Based on neuroanatomical findings, these short-lasting vertigo attacks result from transient insufficiency of the anterior inferior cerebellar artery (AICA), which supplies the inner ear and leads to ischemia of the upper parts of the vestibular labyrinth. Persisting vertigo with or without lateropulsion may occur in cerebellar infarctions with involvement of the medial inferior hemispheres due to occlusion of the medial branch of the posterior …

We searched for mutations in the voltage-gated calcium channel gene, CACNA1A, in nine propositi of families with migraine headaches and episodic vertigo inherited in an autosomal dominant pattern. All 47 exons and flanking introns in CACNA1A were subjected to single-strand conformation polymorphism analysis of polymerase chain reaction-amplified genomic DNA. Exons with aberrantly migrating fragments were sequenced using standard techniques. We also determined the CAG repeat length at the 3' end of CACNA1A. Several polymorphisms were found but no mutations identified in any of the 47 exons of t…

Instillation of gentamicin in the middle ear to cure severe Meniere's disease is of considerable value in the control of vertigo. The drug causes diminished endolymph production and, most often, a peripheral vestibular loss. After vestibular rehabilitation, led by a physiotherapist, the ataxia should be resolved as a result of central compensation. Fifteen to twenty percent of the patients experience an added hearing loss in the treated ear, although this percentage is reduced if the treatment is given as a single instillation on 2 consecutive days. Using this strategy, the mean added hearing …

BACKGROUND: Vertigo is a common symptom that often remains unexplained despite extensive medical evaluation. Psychiatric and psychosomatic disorders are usually considered after all somatic causes of vertigo have been ruled out.
METHODS: Eighty-three patients referred to neurological or psychosomatic outpatient treatment received an extensive neurootologic and psychosomatic evaluation: one (or two) diagnostic psychiatric psychodynamic exploration(s), a structured interview, psychometric tests (SCL-90-R, STAI-G X2 and GBB). The patients were divided into four diagnostic groups: psychic causes …

Results are reported for 40 patients with disabling vestibular symptoms, treated with endolymphatic sac surgery (ELS), gentamicin injections in the middle ear, or vestibular neurectomy by the middle fossa approach. If ELS failed, an additional treatment with a second ELS procedure or gentamicin instillations was performed. If the gentamicin treatment failed, the patients were treated with a second series of gentamicin instillations or vestibular neurectomy or both. The results in relieving the patients of their vertigo were found to be almost the same with each of the treatment modalities: app…

Visually induced vertigo, or motion sickness, is thought to be mainly influenced by motion perception, not by shape perception. We examined this point by comparing the effect of the foveal vision on the visually induced vertigo with that of the peripheral vision. We set up three kinds of display conditions: Foveal vision (F), peripheral vision (P), and foveal + peripheral vision (F + P). Videotaped motion was presented on the display. The results showed that a drifting eye movement was larger after the experiment than before the experiment in the (P) and the (F + P) conditions, but not in the …

The blood flow in the vertebral artery was measured in 507 patients with vertigo using ultrasonic blood rheography. In 150 of the 507 cases (32%), abnormal findings including laterality and decreased flow were detected. Twenty-one cases whose clinical course was observed showed a decrease in the differences in velocity between sides as vertigo improved. VBI (vertebro-basilar insufficiency) showed a worse prognosis in patients with laterality compared with those without and a positive correlation was demonstrated between side differences and fluctuating blood pressure or serum lipids. Some anti…

A measurable parameter in the study of static equilibrium was described. The locus of postural sway was measured with a set of computer during 1 minute period in 68 normal subjects and 82 patients suffering from central and peripheral vertigoes with eyes closed. The results demonstrated: 1) There are five patterns of postural sway: centripetal, forward-backward, right-left, diffuse and multicentric patterns; centripetal pattern was the most prominent figure pattern in the normal subjects, diffuse pattern was the most prominent figure pattern in the central vertigoes. 2) Length of locus and vel…

Ménière's Vertigo is a process of long evolution applying for several clinical variables during many periodical controls. The A. of the paper present an informatics program aiming to compile each patient data regarding these items: 1. a quick knowledge of the evolutive course both clinic and therapeutical experienced by the patient during years; 2. control of the efficacy of the treatments done; and 3. the comparison of own results with those of other authors. In the article are expose the practical development of the program and the data gained with.

We present two patients with clinical features of infarction in the distribution of the anterior inferior cerebellar artery (AICA) who had vertigo as an isolated symptom for several months prior to infarction. Both had risk factors for cerebrovascular disease and other episodes of transient neurologic symptoms not associated with vertigo. At the time of infarction they developed vertigo, unilateral hearing loss, tinnitus, facial numbness, and hemiataxia. MRI identified hyperintense lesions in the lateral pons and middle cerebellar peduncle on T2-weighted images. Audiometry and electronystagmog…

The clinical features and therapeutical aspects of 31 patients with benign paroxysmal positional vertigo (BPPV) were studied. Attention is drawn to diagnostic criteria such as the "Hallpike manoeuvre" and guidelines are given for the differentiation of BPPV and cervical nystagmus. The results of this study are discussed with special regard to habituation training.

Through history taking and physical examination, the primary care physician should be able to distinguish between the four most common causes of recurrent vertigo. Particular caution is advised when dealing with the first attack of vertigo, because more sinister possibilities may be present, although most of the disorders that cause vertigo are benign. Categories of treatment that can be used include medical, rehabilitative or exercise-related, surgical, dietary, and unconventional (eg, acupuncture, manipulation).

Eight cases of vertigo in childhood are reported. Four of them suffered from benign paroxysmal vertigo, and the other four from atypical migraine. History of a head injury and attacks of vertigo lasting only a few seconds are typical of benign paroxysmal vertigo. These children are older than 4 years and the vertigo is of peripheral origin. Vertigo due to atypical migraine is frequent in young children up to 4 years of age, shows a longer duration of vertigo (more than 1 min) and is of central origin.

The relatively high incidence of persistent post-traumatic headache and vertigo in children and adolescents presents a diagnostic and therapeutic challenge. It is often difficult to differentiate between functional complaints generated by psychological trauma or compensation-seeking and symptoms reflecting an organic etiology. The clinical and laboratory findings of 22 patients with post-traumatic headaches and vertigo were delineated into five major diagnostic categories: labyrinthine concussion, whiplash syndrome, basilar artery migraine, vertiginous seizures, and a non-specific post-traumat…

Two cases of inner ear injury caused by suppressed sneezing are described. One patient experienced vestibular symptoms in the form of reflexogenic vertigo that was relieved by surgical section of the tensor tympani tendon. The other patient had a sudden severe permanent sensorineural hearing loss. It is proposed that the aerodynamic pressure increase associated with suppressed sneezing is transmitted via the eustachian tube to cause an implosive fistula of either the round or oval window with injury to the membranous labyrinth.
