Cinchonidine Ameliorates Traumatic Brain Injury-Related Neurobehavioral Deficits via the SRC/RAF1/NF-κB Pathway to Attenuate Mitochondrial Dysfunction.

Cinchonidine (CD), a metabolite derived from gut microbiota, shows promise in protecting against traumatic brain injury (TBI), where mitochondrial dysfunction is a key driver of secondary damage. Using a mouse model of controlled cortical impact (CCI) and an in vitro primary neuron injury model, CD administration resulted in improved physiological status (weight recovery, stabilized heart rate) and behavioral outcomes (increased travel distance and speed) in TBI mice. Histological analysis showed reduced hippocampal neuronal damage, decreased pro-inflammatory cytokines (interleukin‑1β, interle

Efficiency and side effects of sorafenib therapy for advanced hepatocellular carcinoma: a retrospective study by the anatolian society of medical oncology.

BACKGROUND: Inoperable and metastatic hepatocellular carcinoma (HCC) is associated with a poor prognosis and low chemotherapeutic efficiency. Sorafenib is an oral multi-kinase inhibitor exerting its effects via the RAF/ MEK/ERK pathway, vascular endothelial growth factor receptor (VEGFR) and platelet derived growth factor receptor beta (PDGFR-β) tyrosine kinases. Randomized studies have shown a significant contribution of sorafenib to life expectancy and quality of life of cancer patients. The aim of the present study is to evaluate the efficacy and side effects of sorafenib therapy in Turkey.

Evaluation of Heterogeneity in the Coding Region of BRAF, MAP2K1, and MAP2K2 Genes in Primary and Metastatic Melanomas.

INTRODUCTION: The incidence of melanoma has been increasing in recent decades. BRAF mutations appear in 50%-70% of melanomas. The BRAF-targeted therapy increased the disease-free survival of patients with metastatic melanoma, but this response may be short, due to several resistance mechanisms, such as the presence of other subclones with mutations. Evaluation of mutations and heterogeneity in the coding region of the BRAF, MAP2K1, and MAP2K2 genes in primary and metastatic melanomas. PATIENTS AND METHODS: Twenty-seven samples of primary and metastatic superficial spreading melanoma (SSM) and

Molecular targeted treatment in infants with central conducting lymphatic anomalies.

UNLABELLED: Central conducting lymphatic anomaly (CCLA) is a rare and potentially life-threatening vascular malformation characterized by impaired central lymphatic flow. Hydrops fetalis and congenital hydro-/chylothorax are common neonatal presentations; however, diagnosing CCLA poses challenges and requires advanced imaging. Management typically includes supportive therapies with limited effect, such as medium-chain triglyceride (MCT) diet, octreotide or propranolol, and thoracic drainage. Upcoming treatment options with mammalian target of rapamycin (mTOR) and mitogen-activated protein kina

Ankylosing spondylitis and atrial fibrillation: a contemporary overview.

BACKGROUND: Bisphenol A (BPA), a widely distributed endocrine-disrupting chemical (EDC), has been associated with altered pubertal timing in children, including precocious puberty (PP). However, the multi-target molecular mechanisms potentially linking BPA exposure to PP remain incompletely understood. OBJECTIVE: To explore the potential multi-target mechanisms by which BPA may contribute to PP using an integrated computational framework. METHODS: We combined network toxicology, protein-protein interaction (PPI) network analysis, molecular docking, and molecular dynamics (MD) simulations. Sh

Efficacy and safety of anti-PD1 monotherapy or in combination with ipilimumab after BRAF/MEK inhibitors in patients with BRAF mutant metastatic melanoma.

BACKGROUND: Patients with V600BRAF mutant metastatic melanoma have higher rates of progression-free survival (PFS) and overall survival (OS) with first-line anti-PD1 (PD1]+anti-CTLA-4 (IPI) versus PD1. Whether this is also true after BRAF/MEKi therapy is unknown. We aimed to determine the efficacy and safety of PD1 versus IPI +PD1 after BRAF/MEK inhibitors (BRAF/MEKi). METHODS: Patients with V600BRAF mutant metastatic melanoma treated with BRAF/MEKi who had subsequent PD1 versus IPI+PD1 at eight centers were included. The endpoints were objective response rate (ORR), PFS, OS and safety in eac

Developments of MEK inhibitors as future cancer therapies: what have we learned from preclinical and clinical studies?

INTRODUCTION: The combination of MEK and BRAF inhibitors is effective in melanomas and other tumors with BRAF V600E mutations. MEK inhibitors enhance efficacy and delay the development of resistance to BRAF inhibitors. Recently, MEK inhibitors have demonstrated activity in plexiform neurofibromas in patients with type 1 neurofibromatosis and in histiocytic neoplasms. In the preclinical setting, MEK inhibitors are effective in tumors with RAS or receptor tyrosine kinase mutations. However, after the inhibition of MEK, regulatory feedback determines the rebound activation of ERK, thereby limitin

Phenolic Glycoside Monomer from Reed Rhizome Inhibits Melanin Production via PI3K-Akt and Ras-Raf-MEK-ERK Pathways.

INTRODUCTION: Melanogenesis, the process responsible for melanin production, is a critical determinant of skin pigmentation. Dysregulation of this process can lead to hyperpigmentation disorders. METHODS: In this study, we identified a novel Reed Rhizome extract, (1'S, 2'S)-syringyl glycerol 3'-O-β-D-glucopyranoside (compound 5), and evaluated its anti-melanogenic potential in zebrafish models and in vitro assays. Compound 5 inhibited melanin synthesis by 36.66% ± 14.00% and tyrosinase in vivo by 48.26% ± 6.94%, surpassing the inhibitory effects of arbutin. Network pharmacological analysis re

Activity and Resistance of a Brain-Permeable Paradox Breaker BRAF Inhibitor in Melanoma Brain Metastasis.

UNLABELLED: The therapeutic benefit of approved BRAF and MEK inhibitors (BRAFi/MEKi) in patients with brain metastatic BRAF V600E/K-mutated melanoma is limited and transient. Resistance largely occurs through the restoration of MAPK signaling via paradoxical BRAF activation, highlighting the need for more effective therapeutic options. Aiming to address this clinical challenge, we characterized the activity of a potent, brain-penetrant paradox breaker BRAFi (compound 1a, C1a) as first-line therapy and following progression upon treatment with approved BRAFi and BRAFi/MEKi therapies. C1a activi

Adjuvant Therapy Failure Patterns in the Modern Era of Melanoma Management.

BACKGROUND: The management of patients with resected stage 3 melanoma has changed significantly due to adoption of the Multicenter Selective Lymphadenectomy Trial (MSLT)-2 guidelines and to the survival benefit of adjuvant anti-PD-1 immunotherapy and BRAF/MEK-inhibitor (BRAF/MEKi) therapy. Data are scarce regarding recurrence patterns, adjuvant therapy responses, and therapy-associated adverse events (AEs) in the modern era. METHODS: This single-institution, retrospective study analyzed surgically resected stage 3 and oligometastatic stage 4 patients who received anti-PD-1, BRAF/MEKi, or surg

Acute motor and sensory axonal neuropathy related to treatment with MEK inhibitors in a patient with advanced melanoma.

Approximately one-half of advanced cutaneous melanomas have a V600 mutation in the BRAF gene that activates the mitogen-activated protein kinase pathway. The combination of BRAF plus MEK inhibitors is one of the most effective treatments for these patients. Severe neurological toxicities have been reported in the literature. However, these toxicities are very rare. Here, we present one patient with acute motor and sensory axonal neuropathy, which is a subtype of Guillain-Barre syndrome, secondary to treatment with MEK inhibitors. This side effect had never been described as related to these ag

Practical management of BRAF inhibitors in glioma: toxicity and resistance.

BRAF inhibitors have advanced treatment for patients with BRAF-altered high and low-grade glioma (HGG and LGG, respectively). Clinically-available therapies are effective but require selection by mutation type and careful, proactive toxicity management to maximize patient quality of life and treatment duration. While pediatric LGG patients often experience durable responses, adults with LGG and patients with HGG frequently develop treatment resistance and disease progression while on treatment. Strategies to prevent or overcome resistant disease are under active preclinical and clinical invest

How I treat refractory/relapsed hairy cell leukemia with BRAF inhibitors.

Hairy cell leukemia (HCL) responds very well to frontline chemotherapy with purine analogs (cladribine and pentostatine). However, approximately half of patients experience 1 or more relapses, which become progressively resistant to these myelotoxic and immunosuppressive agents. At progression, standard therapeutic options include a second course of purine analogs alone or in combination with rituximab and, upon second relapse, therapy with the anti-CD22 immunotoxin moxetumomab pasudotox. Furthermore, blockade of the mutant BRAF-V600E kinase (the pathogenetic hallmark of HCL) through orally av

New Potential Agents for Malignant Melanoma Treatment-Most Recent Studies 2020-2022.

Malignant melanoma (MM) is the most lethal skin cancer. Despite a 4% reduction in mortality over the past few years, an increasing number of new diagnosed cases appear each year. Long-term therapy and the development of resistance to the drugs used drive the search for more and more new agents with anti-melanoma activity. This review focuses on the most recent synthesized anti-melanoma agents from 2020-2022. For selected agents, apart from the analysis of biological activity, the structure-activity relationship (SAR) is also discussed. To the best of our knowledge, the following literature rev

Melanoma brain metastases: Biological basis and novel therapeutic strategies.

The development of brain metastases is the deadliest complication of advanced melanoma and has long been associated with a dismal prognosis. The recent years have seen incredible progress in the development of therapies for melanoma brain metastases (MBM), with both targeted therapies (the BRAF-MEK inhibitor combination) and immune checkpoint inhibitors (the anti-CTLA-4, anti-PD-1 combination) showing impressive levels of activity. Despite this, durations of response for these therapies remain lower at intracranial sites of metastasis compared to extracranial metastases and it has been suggest

Pharmacist tailored monitoring for patients initiating encorafenib and binimetinib combination therapy.

Introduction: Combination therapy with protein kinase B-raf (BRAF) inhibitor, encorafenib, and mitogen-activated extracellular kinase (MEK) inhibitor, binimetinib, is associated with adverse events (AEs) that can lead to therapy changes early in treatment. This study assessed the impact of pharmacist tailored monitoring on therapy changes during the first 90 days after initiation. Methods: This single center, pre-post intervention study included adult patients initiating encorafenib and binimetinib for unresectable or metastatic melanoma, with prescriptions filled through the center's specialt
