Global Implementation of the Nutrition-Friendly School Initiative (NFSI): A Scoping Review of Strategies, Challenges, Barriers, and Facilitators.

BACKGROUND: The Nutrition-Friendly School Initiative (NFSI) is a whole-school approach designed to address the triple burden of malnutrition among school-aged children. However, evidence on its implementation across diverse settings remains fragmented. METHODS: This scoping review followed Arksey and O'Malley's framework and searched PubMed, Web of Science, Embase, Scopus, and gray literature sources for studies explicitly implementing the NFSI framework. FINDINGS: Sixteen records were included. Evidence suggests that NFSI implementation improves nutrition knowledge, dietary behaviors, and s

Clinically accountable school health: a case for extending primary care into schools.

Rwanda's primary and secondary schools serve about 4.7 million learners, creating an important platform for child and adolescent health. Rwanda has school-health policies, referral arrangements, and strong primary health care (PHC) and community systems, but it lacks a standardized, funded, and clinically regulated function consistently responsible for school-linked first response, referral coordination, and follow-up. This Perspective argues that this continuity gap at the school PHC interface deserves explicit attention and proposes a School Health Officer (SHO) as one model to test within e

Implementation of Comprehensive Dyadic Perinatal Care During the Postnatal Year.

The United States is experiencing a high societal burden of pregnancy-related and infant morbidity and mortality during the postnatal year. These trends are inherently linked because the 2 members of the parent-infant dyad are interdependent for their well-being. Several systemic factors contribute to suboptimal health outcomes, including health care siloes under the medical specialization paradigm, multiple care transitions, and lack of care coordination during the postnatal year. Despite the clear positive impact of breastfeeding on parent and child health outcomes, as a dyadic specialty, la

Public Health Challenges in Somalia: Strengthening Health Systems for Resilient and Equitable Healthcare Delivery.

Somalia continues to face multidimensional public health challenges driven by prolonged conflict, political instability, fragmented governance, climate-related disasters, displacement, poverty, and a fragile health system. The country carries a dual burden of communicable and non-communicable diseases (NCDs), alongside persistently high maternal and child mortality and recurrent nutrition and WASH emergencies. These pressures are intensified by workforce shortages, limited laboratory and surveillance capacity, unequal access to care, and heavy dependence on externally financed programmes. In t

Small extracellular vesicles: connecting early life exposure outcomes to air pollution during pregnancy to early childhood health.

Exposure to air pollution poses a serious threat to maternal and child health, particularly during critical developmental periods. Extracellular vesicles (EVs), as key mediators of intercellular communication, have emerged as a novel mechanism through which environmental exposures, including air pollutants, exert systemic effects. This review synthesizes current evidence on the role of small EVs (sEVs) in mediating the biological impacts of prenatal air pollution exposure, with a focus on their molecular profile, including the presence of signaling molecules like miRNAs and proteins, and their

Impact of HIV treatment scale-up on women's reproductive health care and reproductive rights in Southern Africa.

The HIV epidemic has changed the face of women's reproductive health across southern Africa. In some circles, there have been calls for restrictions on women's reproductive rights, focusing particularly on the spread of HIV between sexual partners and from mother to child. However, during the past decade, public health attention and resources for the clinical care of HIV-infected individuals living in Africa have led to advances in women's reproductive health services. As many programs have recognized that effective HIV care and treatment services must link to other areas of primary care, key

[Assessment of management training for low-level community health workers providing care for children with acute respiratory infections in four districts of Senegal].

BACKGROUND: Acute respiratory infections are the leading cause of death in childhood and most child deaths in Senegal occur at home without qualified health care. Despite this situation, only qualified healthcare workers are authorized to prescribe antibiotics. A competency-based training program was developed to improve and assess management of acute respiratory infections in young children aged between 2 and 59 months by low-level educated community health workers (CHWs) in four districts of Senegal. METHODS: In accordance with the strategy developed by the World Health Organisation, educat

Health and cultural factors associated with enrolment in basic education: a study in rural Ghana.

This inter-disciplinary study compares the health status of school-age children in Ghana, both enrolled and non-enrolled, and examines these results within a wider socio-economic and socio-cultural context including kinship and livelihood. Children matched for age and sex who were not enrolled in Primary School were significantly shorter and more stunted than enrolled children were, and 70% of all Primary school-age children were anaemic. Young children from farming communities were significantly more undernourished than children from fishing communities. Adolescent non-enrolled boys were more

INTRODUCTION: In France, care pathways in child mental health remain complex and difficult to navigate for families, despite political efforts to strengthen outpatient support. Often mobilized as “resources,” families are still rarely recognized as full partners in the care process. OBJECTIVES AND METHOD: This collaborative action research, conducted in a CMPP (centre médico-psycho-pédagogique; center for medical, psychological, and educational care) located in a rural area, aimed to better understand the obstacles experienced by families and to co-constr

Beyond the dental clinic: ending the global stagnation in pediatric caries with program science.

Globally, early childhood caries is the most prevalent disease in children, even after a century of development. This demonstrates an "effectiveness illusion" where clinical methods fail to deliver true equity. The reason for this inconsistency arises from "dental exceptionalism", which excludes oral health from the systemic medical care systems. Through this article, we advocate moving from a reactive, tools-focused Clinical Science model to a proactive, population-specific Program Science approach. By utilizing the principle of proportionate universalism, prevention can be integrated into th

The effects of social support on women's health during pregnancy, labor and delivery, and the postpartum period.

This review of the literature on social support and its relationship to maternal health indicates that emotional, tangible, and informational support are positively related to mothers' mental and physical health around the time of childbirth. The importance of various types of support changes with the changing needs of women as they move from pregnancy to labor and delivery, and then to the postpartum period. During pregnancy, emotional and tangible support provided by the spouse and others is related to the expectant mother's mental well-being. In addition, informational support in the form o

Climatic heterogeneity in meteorological associations with childhood acute respiratory symptoms across four South Asian countries.

BACKGROUND: Childhood acute respiratory symptoms (ARSs) remain an important public health concern in South Asia, but meteorological associations may not be transferable across climatically diverse settings. Studies that rely on pooled regional estimates or single exposure metrics may therefore obscure meaningful heterogeneity. We examined whether associations of childhood ARSs with temperature and vapour pressure deficit (VPD) differed across macroclimatic regimes in four South Asian countries. METHODS: We analysed Demographic and Health Survey data from India, Bangladesh, Nepal, and Timor-Le

The status and influencing factors of pressure on women of childbearing age to have a child again: A cross-sectional study in China.

BACKGROUND: Persistently low fertility and rapid population aging have increased concern regarding subsequent childbearing in China. Although fertility intentions have been widely examined, the multidimensional pressure experienced by parous women when considering another child remains insufficiently understood. METHODS: An online cross-sectional survey was conducted from January to June 2025 among 2097 parous women aged 18-49 years across 21 provinces and four municipalities in China. Subsequent fertility pressure was assessed using a validated 24-item scale. Guided by Becker's quantity-qual

The distribution and the relationship of HPV subtypes infection with pregnancy outcomes.

Human Papillomavirus (HPV) infection is a significant public health concern, particularly during pregnancy. The impact of HPV infection during pregnancy on maternal and child health is a growing concern. However, research on infection rate and pregnancy outcomes of nine-valent vaccine-covered and non-covered high-risk HPV subtypes during pregnancy remains limited. A retrospective cohort study involved 7110 pregnant women who underwent HPV testing in Fujian Provincial Maternity and Child Health Hospital, Affiliated Hospital of Fujian Medical University (FMCH) from January 2021 to December 2022.

Pre-Pregnancy Obesity and Infants' Motor Development within the First Twelve Months of Life: Who Is Expected to Be the Ultimate Carrier of the Obesity Burden?

INTRODUCTION: Pre-pregnancy obesity is a significant public health concern with profound implications for maternal and child health. The burgeoning evidence suggests that maternal obesity prior to conception is intricately linked with an increased risk of gestational complications, as well as with adverse neonatal outcomes. Furthermore, the long and short-term health of offspring, including the risk of early motor development impairment, obesity, and metabolic syndrome in childhood and adulthood, may be adversely affected as well. Addressing pre-pregnancy obesity is critical for improving over

Doula engagement and maternal opioid use disorder (OUD): Experiences of women in OUD recovery during the perinatal period.

BACKGROUND: Pregnant women who have substance use disorders (SUDs) are at increased risk of preterm birth, fetal mortality, and inadequate prenatal care and have higher rates of childhood trauma than their counterparts without SUDs. Doulas have been utilized with other vulnerable populations who experience trauma to increase perinatal healthcare utilization, provide emotional support, and improve birth outcomes. The objective of the current study was to examine, in women with opioid use disorder (OUD), perceptions of working with a doula in the perinatal period. METHODS: Eligible participants
