Preliminary results of a liver allocation plan using a continuous medical severity score that de-emphasizes waiting time.

Liver allocation remains problematic because current policy prioritizes status 2B or 3 patients by waiting time rather than medical urgency. On February 21, 2000, we implemented a variance to the United Network for Organ Sharing liver allocation policy that redefined status 2A by much more rigid, definable criteria and prioritized status 2B patients by using a continuous medical urgency score based on the Child-Turcotte-Pugh score and other medical conditions. In this system, waiting time is used only to differentiate status 2B candidates with equal medical urgency scores. Comparing the 6-mont

Reappraisal of the role of vegetables in the vitamin A status of mothers in Central Java, Indonesia.

Food-based approaches for controlling vitamin A deficiency and its consequences, such as increased mortality, more severe morbidity, and anemia, have become increasingly important, thus prompting a reassessment of the relation between vitamin A intake and status. A nutrition surveillance system in Central Java, Indonesia, assessed the vitamin A intake and serum retinol concentration of women with a child < or =24 mo old with a semiquantitative 24-h recall method that categorized vitamin A-containing foods into 3 categories of plant foods and into 2 categories of animal foods and identified por

The utility of head computed tomographic scanning in pediatric patients with normal neurologic examination in the emergency department.

Head injury is a frequent cause of morbidity and mortality in pediatric trauma. Guidelines for obtaining computed tomographic (CT) scans in the child with mild head injury are poorly defined. This study investigated the utility of head CT scanning in the pediatric patient presenting with normal neurologic examination. All patients undergoing head CT scanning for trauma in the emergency department (ED) at a tertiary care pediatric trauma center during 1992 were identified (508). Charts were reviewed for historical and physical examination findings, CT results, and need for neurosurgical interve

Family Interviews Improve Health Service Recommendations in Mortality Review Process: A Mixed-Methods Assessment.

BACKGROUND: Within the United States, although there is strong motivation for incorporating family interviews into Fetal Infant Mortality Reviews (FIMR) and important potential for expanding it to other types of reviews, there is limited evidence that family interviews make a difference in review team outcomes. OBJECTIVE: This study aims to assess the impact of FIMR family interviews identifying health service gaps and generating actionable improvements. DESIGN: Mixed methods design with quantitative case-control analysis and qualitative semi-structured interviews. Quantitative data collecti

[Treatment of West syndrome].

PURPOSE: West syndrome (WS) is one of the catastrophic epileptic syndromes in infancy characterized by a triad of infantile spasms, psychomotor deterioration and hypsarrhythmic EEG pattern. WS is commonly associated with poor long-term outcome, especially in symptomatic cases, with development of other seizure types, impaired cognitive and psychosocial functioning. The aim of our study was to evaluate the efficacy of the control of infantile spasms using synthetic ACTH or vigabatrin in newly diagnosed cases and to correlate it with the underlyning causes, outcome and adverse effects. PATIENTS

The trends of mortality, aetiologies and risk factors of lower respiratory infections in Saudi Arabia from 1990 to 2021: results from the global burden of disease study 2021.

BACKGROUND: Lower respiratory tract infections (LRIs) are the fourth leading cause of death globally, affecting all age groups and leading to over 2 million deaths annually. Saudi Arabia faces a significant burden from LRIs, affecting more than 15% of the population each year. This study aims to provide an overview of LRI mortality, etiologies, and risk factors in Saudi Arabia from 1990 to 2021. METHOD: Data on LRI mortality in Saudi Arabia from 1990 to 2021 were extracted from the 2021 edition of the Global Burden of Disease (GBD) Result Tool. The analysis encompassed mortality rates across

Predictive analytics characterize cardiovascular function and outcomes following cardiac surgery.

BACKGROUND: Using supervised machine learning, predictive models of cardiovascular and respiratory instability were previously created and display risk scores (CoMET score), characterizing the risk of clinical deterioration in the ensuing 12 h. We assessed the relationship of CoMET scores with vasoactive-inotropic scores (VIS) and short-term clinical outcomes in pediatric patients following cardiac surgery. METHODS: Retrospective cohort study of all patients admitted to the pediatric cardiac intensive care unit following cardiac surgery at a single center over an 18-month period. We performed

Center Volume and Failure to Rescue in Pediatric Cardiac Surgery: Is There a Relationship?

BACKGROUND: Failure to rescue (FTR) is a validated quality metric for evaluating outcomes in pediatric cardiac surgery. Using a national administrative database, we sought to assess the relationship between annual hospital case volume and FTR. METHODS: Cardiopulmonary bypass (CPB) cases were extracted from the Kids' Inpatient Database for 2016 and 2019. FTR was defined as inpatient mortality of patients with at least 1 postoperative complication. Hospitals were divided into high-, mid-, and low-volume hospitals on the basis of annual CPB case volume terciles. A mixed effects multivariable log

Association Between Changes in Racial Residential and School Segregation and Trends in Racial Health Disparities, 2000-2020: A Life Course Perspective.

INTRODUCTION: Most studies of the relationship between racial segregation and racial health disparities have focused on residential segregation. School-based racial segregation is an additional form of segregation that may be associated with racial disparities in health. This study examines the relationship between both residential segregation and school segregation and racial health disparities among non-Hispanic Black compared to non-Hispanic White persons at the county level in the United States. It also examines the relationship between changes in residential and school segregation and sub

Trends in injury-related mortality among residents of Jiangsu Province from 2012 to 2021: an age-period-cohort analysis.

OBJECTIVE: We investigated the temporal trends and examined age-, period-, and cohort-specific effects of injury-related deaths among residents in Jiangsu to provide evidence for future injury prevention. METHODS: This study included 406,936 injury deaths from the Jiangsu provincial population death registration system. The average annual percent change (AAPC) in age-standardized mortality rates (ASMRs) was analyzed using joinpoint regression. Age-period-cohort models were generated to explore the effects of age, period, and birth cohort effects on mortality risk. RESULTS: ASMRs for all inju

State-level estimates of excess hospitalizations and deaths associated with influenza.

BACKGROUND: National estimates of influenza burden may not reflect state-level influenza activity, and local surveillance may not capture the full burden of influenza. METHODS: To provide state-level information about influenza burden, we estimated excess pneumonia and influenza (P&I) and respiratory and circulatory (R&C) hospitalizations and deaths in Colorado from local hospital discharge records, death certificates, and influenza virus surveillance using negative binomial models. RESULTS: From July 2007 to June 2016, influenza was associated with an excess of 17 911 P&I hospitalizations (

Indications and complications of inpatient parenteral nutrition prescribed to children in a large tertiary referral hospital.

BACKGROUND: Parenteral Nutrition (PN) is prescribed to children with intestinal failure. Although life saving, complications are common. Recommendations for indications and constituents of PN are made in the 2005 guidelines by the European Society of Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN). The aim of this study was to establish if the indications for prescribing PN in a tertiary children's hospital were appropriate, and to identify complications encountered. Data were compared to those published by the National Confidential Enquiry into patient outcome and death (NCEPO

Health transition in Brazil: regional variations and divergence/convergence in mortality.

This study analyzes the main characteristics of the health transition in Brazil and its five major regions, using a framework that accounts for regional inequalities in mortality trends. The regional mortality divergence/convergence process is described and discussed by considering the specific contributions of age groups and causes of death in life expectancy variations. Results show that mortality change in Brazil has follow the epidemiologic transition theory to some extent during the period under analysis - for instance, the sharp decline in infant mortality in all regions (first from infe
