An experience with the treatment of 286 patients with acute cholecystitis is described. Cholecystectomy was performed in combination with one of the methods of internal drainage of bile ducts which made up 33.1% of the total number of operations performed for acute cholecystitis. Lethality in this group of patients was 4%. To decrease the amount of postoperative complications and lethality the external drainage of bile eliminating ducts is indicated in the nearest postoperative period.

Drainage of the biliary tree and removal of retained common bile duct stones can be achieved by performing a sphincterotomy of the ampulla of Vater using a diathermy sphincterotome directed through a fibre-optic endoscope. The procedure was successfully performed on two patients considered unfit for abdominal surgery. It is a useful method in selected patients to release stones retained in the common bile duct after cholecystectomy.

The literature data on acute cholecystitis occuring as a complication after operations on the abdominal organs are analysed. The author presents 3 own observations of cholecystitis in early postoperative period among 28,556 laparotomies. Taking into consideration the rarity of this complication, the author believes that the arguments adduced for simultaneous preventive cholecystectomy in operations for other abdominal organs diseases are dubious, especially in emergent abdominal surgery.

A case is described of abdominal trauma and rupture of a left-sided liver in a patient with Kartagener's syndrome (situs inversus, bronchiectasis, sinusits). Exploratory laparotomy, left thoractomy, cholecystectomy and hepatic artery ligation were necessary to control hemorrhage. Special attention was paid to the patient's respiratory condition postoperatively via physiotherapy and analgesia. The importance of identifying this three-part syndrome and especially the bronchiectasis is emphasized.

A study of the results of surgical treatment of patients with acute cholecystitis showed that cholecystectomy is a safe procedure for the majority of patients during their initial hospitalization and avoids the risk of recurrent attacks and readmissions. Cholecystostomy has a limited place in the treatment of older patients with systemic disease and advanced local disease. Early aggressive management of acute cholecystitis will probably reduce complications of cholecystitis and reduce the need for cholecystostomy.

A 25-year-old secundipara developed acute gangrenous cholecystitis coupled with suppurative cholangitis on the fourth pureperal day. The patient recovered after cholecystectomy and choledochotomy. It is pointed out that cholelithiasis is not rare in pregnancy but acute cholecystitis is very rare, especially in the puerperium. For this reason the diagnosis is difficult, yet the final issue depends on its early establishment and also on the early surgical intervention.

PURPOSE OF REVIEW: Low phospholipid-associated cholelithiasis (LPAC) syndrome is a rare genetic form of intrahepatic cholesterol lithiasis, … lelithiasis at an abnormally early age (before 40) and by the persistence of biliary symptoms after cholecystectomy. The diagnosis is usually confirmed by an ultrasound scan of the liver, which reveals the presence of intrahepatic microlithiasis, as evidenced by comet-tail images or microspots along the intrahepatic bile ducts. OBJECTIVES: To determine the clinicopathological features and prevalence of Eosinophilic cholecystitis (EC) with a single-center experience. Eosinophilic cholecystitis is an exceedingly uncommon pathologic issue caused by examining cholecystectomy specimens.
METHODS: A retrospective analysis was performed on 8,342 cholecystectomy specimens. Of these, 33 pathology results confirmed EC and were re-examined by pathologists. This study obtained demographic data, medical histories, comorbidities, clinical laboratory results, radiological findings, and pathological information from an electronic re…

We report a case of a patient with Dubin-Johnson syndrome confirmed by a genetic study. A 50-year-old woman who had symptoms of intermittent right upper quadrant abdominal pain was diagnosed with calculous cholecystitis at another institute and was presented to our hospital for a cholecystectomy. She had no history of liver disease, and her physical examination was normal. Abdominal computed tomography showed a gallbladder stone with chronic cholecystitis. During a laparoscopic cholecystectomy for cholecystitis, a smooth, black-colored liver was noted, and a liver biopsy was performed. The bio…

BACKGROUND: Increased intra-abdominal pressure causes hemodynamic changes that may affect renal biomarkers.
METHODS: This randomized, single-blind, single-center clinical trial recruited patients undergoing laparoscopic cholecystectomy at a tertiary care center in Brazil. They were randomly allocated to a standard intra-abdominal pressure group (P10-12, 10-12 mm Hg) and a low intra-abdominal pressure group (P6-8, 6-8 mm Hg). The primary outcome was the change in neutrophil gelatinase-associated lipocalin (NGAL) and cystatin C levels measured at the beginning of the procedure (T0), at the end …

Considering the recurrent biliary attacks, laparoscopic cholecystectomy was performed with intraoperative diagnosis of LBS. A subsequent endocrinological screening highlighted a normocalcemic hyperparathyroidism associated with Vitamin D deficiency, likely related to the recent pregnancy and not to LBS.
DISCUSSION: LBS …

In high-risk surgical patients with acute cholecystitis who are not candidates for early laparoscopic cholecystectomy, gallbladder (GB) drainage is an alternative treatment option. Percutaneous transhepatic gallbladder drainage (PTGBD) is a recommended first line intervention because of its high efficacy and feasibility in most centers. However, with the advent of endoscopic accessories and technology, endoscopic GB drainage has been chosen as a more favorable choice by endoscopists. Endoscopic transpapillary gallbladder drainage (ETGBD) can be performed under either fluoroscopic or peroral ch…

Cholecystocolic fistulas are uncommon, with rare cases of colonic obstruction described in the literature and even rarer cases of intestinal perforation due to gallstones. We describe a case of a 73-year-old man who presented to our ED with complaints of diffuse abdominal pain, vomiting, constipation, and fever for the past week. Abdomen CT showed signs of acute perforated appendicitis. An exploratory laparotomy was proposed which revealed cecal perforation caused by a 3 cm gallstone. A right colectomy was performed with primary anastomosis, without cholecystectomy or fistula repair. The posto…
