Spontaneous coronary artery dissection continues to pose a diagnostic dilemma in the evaluation of patients with chest pain. Our case discusses its manifestation in a male patient who visited the emergency department complaining of recent-onset chest pain. Evaluation of his chest pain through coronary angiography revealed luminal radiolucency corresponding to type 1 spontaneous coronary artery dissection (SCAD). The patient was promptly managed using medical interventions until stability was achieved and referred to cardiac rehabilitation care with close follow-up. In our literary contribution…

Kounis syndrome is an allergic acute coronary syndrome (ACS) characterised by coronary artery spasm, plaque erosion/rupture or stent thrombo … nce tomography (OCT). She had non-ST-segment elevation ACS as a result of anaphylaxis to cefazolin. Coronary angiography revealed severe stenosis in the left anterior descending artery; angiographically undetectable plaque rupture was detected using OCT. OCT also revealed intraplaque neovascularisation, suggesting that the culprit plaque had been vulnerable. OCT can a …

Since the first coronary angioplasty by Andreas Grüntzig in Zurich in 1977, the number of cardiac interventional procedures has steadily increased. The aim of this report is to summarise the state of catheter-based cardiac interventions in adults in Switzerland in 2018. Since 1987, the Working Group Interventional Cardiology of the Swiss Society of Cardiology has collected annually aggregate data from all facilities with cardiac catheterisation laboratories in the country, currently 36 institutions in 17 cantons of Switzerland. Over past years, the numbers of coronary angiography procedur…

We present a case of complete anterior papillary muscle rupture (PMR) due to localized papillary muscle infarction, without significant coronary artery disease. A 69-year-old woman was transferred to our hospital because of acute orthopnea and cardiogenic shock. Echocardiography showed severe mitral regurgitation due to anterior PMR, and emergency coronary angiography revealed no obstructive disease in the coronary arteries. Emergency mitral valve replacement was performed with intra-aortic balloon pumping support. Intraoperatively, the anterolateral papillary muscle was found to be completely…

Intraventricular septum-hematoma is a rare complication following percutaneous coronary intervention (PCI). This complication may represent … 9.2 cm sized end-diastolic septum-hematoma, which compromised right ventricular cavity. Emergent recoronary angiography ruled out further contrast extravasation from the RCA. Conservative treatment was intended after discussion in the "heart-team." The patient completely recovered with nearly complete resolution of the hematoma after 6 months.

The purpose of this study was to estimate patient doses during coronary angiography (CA) and different complex percutaneous transluminal coronary angioplasty (PTCA) procedures in China. Consecutive 290 patients (90 CA, 200 PTCA) with known or suspected coronary artery disease were enrolled prospectively into six groups. Differences in patient doses among the six groups were analysed, and the patient doses observed in this study were compared with the recent literature. The median dose area product values were 27.0 and 195.0 Gy cm(2) for CA and PTCA, respectively. The patient doses during CA we…

We treated a 77-year-old woman diagnosed with severe aortic stenosis, who had undergone catheter ablation for paroxysmal supraventricular tachycardia at the age of 62. Although the patient remained in sinus rhythm, she had been receiving Coumadin with a target INR level of 2.0 since that time. Preoperative coronary angiography revealed a coronary-left atrium (LA) fistula, while computed tomography and echocardiography findings did not detect thrombus formation. Intra-operative transesophageal echocardiography revealed a 2-cm solid mass, which unexpectedly appeared during deairing manipulation.…

BACKGROUND: A 51-year-old male was referred for consideration for heart transplantation because of recently diagnosed congestive heart failure refractory to medical therapy. Previous echocardiography demonstrated a left ventricular ejection fraction of approximately 15% with global hypokinesia. Coronary angiography did not reveal any clinically significant obstructive coronary artery disease and an electrocardiogram documented atrial fibrillation with a rapid ventricular rate of 130 beats/min. Other laboratory tests including thyroid function test were unremarkable. The patient's main complain…

Off-pump coronary artery bypass grafting is rarely applied to patients who have previously received a renal transplant in Japan. A 59-year-old male renal transplant recipient was admitted for unstable angina pectoris. Emergency coronary angiography revealed triple-vessel disease. Intraaortic balloon pumping was applied, followed by emergency off-pump coronary bypass grafting for complete revascularization. Intraaortic balloon pumping was ceased immediately after the operation because his hemodynamic status was stable. On the morning of the surgery, the patient was given his standard dose of im…

UNLABELLED: We evaluated the surgical results of postinfarction ventricular septal perforation by endocardial patch with infarction exclusion.
MATERIALS AND METHODS: We reviewed 8 patients complicating AMI who underwent surgical treatment at our institution from July 1997 to August 2000 (6 males, 2 females, mean age 73.9 +/- 9, range 57-87). The localization of AMI and VSP was anterior in 6 patients, inferior in 2. All patients had coronary angiography preoperatively. And 7 patients had the percutaneous transluminal coronary angioplasty of the infarct artery.
RESULTS: There were 2 hospital d…

The primary antiphospholipid syndrome is a disorder which is characterized by: arterial and/or venous thrombosis, thrombocytopenia, recurrent fetal loss and high plasma levels of antiphospholipid antibodies. Valvular involvement is associated with arterial thrombosis and the most frequent manifestation is regurgitation. We report the case of a young male with primary antiphospholipid syndrome and previous cerebrovascular thrombosis hospitalized for subacute myocardial infarction. Coronary angiography revealed right and left anterior descendent coronary artery stenosis, the latter being success…

Five male patients, aged between 31 and 58 years, presented with anginal chest pain with nausea and sweating after the interruption of exercise. Prinzmetal variant angina was observed during the recovery phase of exercise tolerance testing. Coronary arteriography and selective left ventricular angiography were normal in all cases. Ergonovine, used in one case, induced coronary artery spasm. The angina was eased by Nifedipine in three patients and passed off with time in the other two patients. In one case attack occurred with amiodarone therapy and in another with glyceril trinitrate, after no

A 33 year old male with three years of increasingly frequent nocturnal angina pectoris was found on ECG tape monitoring to develop marked ST segment elevation followed by coarse ventricular tachycardia with subsequent spontaneous termination of the arrhythmia. Further evaluation included normal maximal exercise tolerance test, normal coronary arteriography and left ventricular angiography. Continuous nocturnal EEG, ECG, and blood pressure recordings on nitrate therapy revealed normal sleep patterns, development of sinus tachycardia with minimal muscular movement and a normal circadian decline

To evaluate coronary artery disease (CAD) with computed tomography coronary angiography (CTCA)-derived fractional flow reserve (FFR) in patients with atrial fibrillation (AF) requiring ablation. The study population consisted of 151 patients who underwent CTCA before AF ablation (AF group), and a control group of 151 patients from the outpatient clinic who underwent CTCA without any history of AF (non-AF group), matched for age, sex, BMI, and angina symptomatology. All study patients underwent CTCA with subdivision of coronary lesion type into severe (≥ 70% luminal narrowing), moderate (50% ≤ …

The presence of ST-segment elevation on the electrocardiogram alerts physicians in patients with chest pain. Emergency coronary angiography is usually performed in these patients. However, there are many conditions that cause ST-segment elevation on the electrocardiogram, such as pericarditis, hyperkalemia, Brugada syndrome, hypothermia, and early repolarization. Pneumothorax is a rare complication of tracheostomy and its symptoms are sudden chest pain and dyspnea. Also, it has been known that pneumothorax may cause ST-segment changes on the electrocardiogram. We presented a transient inferola…

The aVR-sign can indicate left main or multivessel coronary disease, but the sign is not STEMI eqvivalent and is not a sensitive sign for coronary disese. The following case is an example of this. An 89-year-old woman was admitted with chest pain, atrial fibrillation and multiple lead ST-segment depression but ST-segment elevation in lead aVR. The aVR-sign indicated urgent angiography with negative result. A spontaneous sinus conversion was observed with repolarization normalisation. Later the ECG demonstrated SA-blocks, and sinus arrest. Sick sinus syndrome was diagnosed and the patient was t

A 67 years old woman with history of surgical Mitral valve replacement underwent transoesophageal echocardiography for evaluation of mild dyspnoea. It revealed a large narrow-necked outpouching from medial portion of the inferior wall of the left atrium (LA) that was extended to the interatrial septum. A diagnosis of the LA pseudoaneurysm was made. It contained a fresh clot. Five days later, the patient presented with an acute inferior ST-elevation myocardial infarction. Coronary angiography revealed an occluded right coronary artery at its distal part by a large thrombus. Thromboaspiration wa…

We report about a 79 years old female patient which was admitted due to a symptomatic AV block 3rd degree. The coronary angiography excluded a coronary artery disease and the echocardiography revealed a normal left ventricular systolic function. Therefore a dual-chamber pacemaker was implanted. Following two micro-dislocations of the right ventricular lead, which required operative revisions, a computed tomography of the heart was performed. This detected an aneurysm of the ascending aorta (5 cm maximum diameter) with compression of the superior caval vein. This case shows that a possible caus…

Quadricuspid aortic valve is a rare congenital heart disease that is mostly detected incidentally during echocardiography or angiography, or at autopsy. This pathology is frequently associated with other congenital anomalies of the heart such as abnormalities of coronary ostia, coronary stenosis, aortic root dilation and coarctation of the aorta, as well as significant aortic valve dysfunction. Herein is presented the first documented case of an asymptomatic middle-aged male patient with incidentally diagnosed quadricuspid aortic valve and a preserved valve function associated with a patent fo
