Practice Patterns in the Treatment of Patients With Severe Alcohol Withdrawal: A Multidisciplinary, Cross-Sectional Survey.

PURPOSE: To characterize physicians' stated practices in the treatment of patients with severe acute alcohol withdrawal syndrome (sAAWS) and to use intravenous (IV) phenobarbital as an adjuvant treatment for sAAWS. METHODS: A multidisciplinary, cross-sectional, self-administered survey at 2 large academic centers specializing in inner-city healthcare. RESULTS: We analyzed 105 of 195 questionnaires (53.8% response rate). On average, clinicians managed 32 cases of AAWS over a 6-month period, of which 7 (21.9%) were severe. Haloperidol (Haldol; 40 [39%]), clonidine (Catapres; 31 [30%]), phenoba

[Parkinson's disease: psychological and behavioral disorders].

Many patients with Parkinson's disease (PD) have clinically significant anxiety, depression, apathy, fatigue and sleep disturbances. These non motor symptoms of Parkinson's disease are still not well recognized in clinical practice, either in primary or in secondary care, and are frequently missed during routine examination, but often contribute to impair the functional abilities in PD patients. Awareness of behavioral and psychological symptoms in PD should generate new approaches to improve their diagnosis accuracy and facilitate timely therapeutic interventions.

[Acamprosate (Aotal): could adverse effects upset the treatment of alcohol dependence?].

Acamprosate, a stimulant of central inhibitory GABA neurotransmision and an antagonist of excitatory amino acids, is used in alcohol withdrawal and for the maintenance of abstinence. After identification of several cases of treatment discontinuation during alcohol abstinence because of acamprosate-induced adverse drug reactions (ADRs), a retrospective study was conducted in order to investigate and quantify acamprosate-induced ADRs. Up to July 2002, 472 patients were included for treatment of alcohol withdrawal: of these, 68% (n = 322) received acamprosate. At least one ADR occurred in 98 pati

Alcohol and suicide.

It is well established that alcoholics have a very high suicide rate. The evidence is of two kinds: a) Follow up studies of alcoholics consistently reveal high suicide rates. The proportion dying by suicide varies from 8% (Kessel & Grossman, 1961) to 21% (Gabriel, 1935) depending on the length of follow up. These figures represent a risk 5080 times that of the general population. Most such studies were based on clinically identified populations of alcoholics and might not be representative of alcoholics in general. However: b) Retrospective studies of suicides consistently find that a high pro

Alteration of thyroid hormone economy during alcohol withdrawal in alcoholics.

Alcohol and alcohol withdrawal may affect nutritional status in many different ways, including effects on thyroid activity. The influence of … se of T4, T3, rT3, or TSH, on the other. However, during severe ethanol abstinence (with or without delirium tremens), there was a significant increase in the serum levels of T4 and rT3, but not in those of T3 and TSH. These findings indicate that (severe) alcohol abstinence can interfere with thyroid hormone economy, either by enhancing thyroid hormone se …

Auditory brainstem response study of alcoholic patients.

Auditory brainstem responses were obtained from 52 alcoholic patients with or without neurological complications of alcoholism. Almost half of the patients had abnormal auditory brainstem responses manifested by a prolonged central conduction time (Wave I to V interval) without significant alterations in morphology. The incidence of abnormal auditory responses was related to the type and the number of neurological complications. Patients with Wernicke-Korsakoff syndrome, dementia or cerebellar degeneration had high incidence of brainstem abnormalities. All patients with 3 or more neurological

[Syndromologic independence of amentia].

On the basis of long-term clinical observations of patients with amentive states within the framework of somatogenic, infectious, intoxicational, vascular exogenous-organic, reactive and schizophrenic psychoses the author attempts to claim the nosological independence of the amentive syndrome. The paper contains data on the nosological specificity of the modified syndrome in various disorders. The independence of the amentive syndrome is also grounded by clinico-pathological data (EEG and biochemical).

The Role of Cytokines in Perioperative Neurocognitive Disorders: A Review in the Context of Anesthetic Care.

Perioperative neurocognitive disorders (PNDs), including postoperative delirium, delayed neurocognitive recovery, and long-term postoperative neurocognitive disorders, present significant challenges for older patients undergoing surgery. Inflammation is a protective mechanism triggered in response to external pathogens or cellular damage. Historically, the central nervous system (CNS) was considered immunoprivileged due to the presence of the blood-brain barrier (BBB), which serves as a physical barrier preventing systemic inflammatory changes from influencing the CNS. However, aseptic surgica…

Diagnostic Challenges in an Elderly Patient With Cognitive Decline and an Incidental Brain Mass.

Cognitive impairment in the elderly is often attributed to neurodegenerative processes such as dementia or delirium. However, in some cases, structural brain lesions, including malignancies like glioblastoma and lymphomas, can present similarly. This case report is of a 79-year-old male with progressive cognitive decline who was initially suspected to have dementia but was later found to have a brain mass. The absence of focal neurological deficits and a normal initial CT scan delayed the recognition of an underlying malignancy. An MRI ultimately revealed a lesion concerning glioblastoma or pr…

Obstacles to Early Diagnosis of Acute Hepatic Porphyria: Current Perspectives on Improving Early Diagnosis and Clinical Management.

Porphyrias are, for the most part, inherited disorders of the heme biosynthetic pathway which lead to accumulation of specific intermediates responsible for most of the symptoms and signs of biochemically active disease. Acute hepatic porphyrias usually come to clinical attention primarily in women in their reproductive years who present with episodic, severe, generalized abdominal pain. Such acute attacks may also be associated with tachycardia, systemic arterial hypertension, hyponatremia, recent history of dark reddish to brownish urine, and anxiety, delirium, and sensory or motor neuropath…

[Geriatric traumatology-Principles of orthogeriatrics].

The number of patients with fragility fractures will continue to increase in the future. The multimorbidity of these patients places high demands on the treatment. The treatment of this complex group of patients in specialized centers is therefore recommended. The collaborative treatment of patients by trauma surgeons and geriatricians improves the results. The treatment is carried out according to a standardized protocol and includes rapid surgical stabilization of the fracture and an interdisciplinary care of the patients. During hospitalization many patients develop delirium. Due to the hig…

A Review of the Risk Factors and Approaches to Prevention of Post-Reperfusion Syndrome During Liver Transplantation.

Post-reperfusion syndrome (PRS) is a severe and highly lethal syndrome that occurs after declamping the portal vein forceps during liver tra … ndiscriminately. Considering the range of possible adverse prognoses including acute kidney injury, delirium and graft nonfunction, it is imperative that clinicians increase their awareness and prevention of PRS. The aim of this article is to review the current risk factors, pathophysiological mechanisms and prevention strategies for PRS.

[Research progress on moderate and deep sedation during wound dressing change in pediatric burn patients].

Moderate and deep sedation can effectively relieve or eliminate the pain and body discomfort during wound dressing change in pediatric burn patients, relieve anxiety, agitation, and even delirium of the children, reduce the metabolic rate of the children, make them in a quiet, comfortable, and cooperative state, which is conducive to the smooth completion of dressing change. This paper summarized the three aspects of moderate and deep sedation in pediatric burn patients, including the overview, main points of implementation, and effects, and further introduced the moderate and deep sedation me…

Anaesthesia, analgesia, and cancer outcomes: time to think like oncologists?

Cao and colleagues present a follow-up analysis of a previous RCT among >1200 older adults (mean age 72 yr) undergoing cancer surgery, originally designed to evaluate the effect of propofol or sevoflurane general anaesthesia on delirium, here to evaluate the effect of anaesthetic technique on overall survival and recurrence-free survival. Neither anaesthetic technique conferred an advantage on oncological outcomes. We suggest that although it is entirely plausible that the observed results are truly robust neutral findings, the present study could be limited, like most published studies in the…

Anti-N-methyl-D-aspartate receptor encephalitis in pregnancy associated with teratoma.

A 36-year-old pregnant woman with a prior history of depression and recent gunshot wounds presented with sudden deterioration in her mental status. Clinical examination revealed psychosis, hallucinations, and lack of orientation, with an otherwise normal neurological and cardiorespiratory examination. Computed tomographic scan of her head was normal, and she was diagnosed with acute psychosis and excited delirium. She did not respond to supraphysiologic dosages of antipsychotic therapy and needed physical restraints for combativeness and agitation. Her cerebrospinal fluid analysis was negative…
