POEMS syndrome has been defined as an association of plasma cell dyscrasia with polyneuropathy, organomegaly, endocrinopathy, M protein and skin changes. Although certain authors do not distinguish this syndrome from osteosclerosing myeloma, syndromes comparable to POEMS have been observed without bone lesions. Others have described the clinical features involved under the terms of Crow-Fukase's syndrome, PEP syndrome (pigmentation, oedema, plasma cell dyscrasia, or Takatsuki's syndrome. Seen in men twice as often as in women, usually between the ages of 40-50 years, all five clinical features

BACKGROUND: Early detection of diabetic foot ulcerations (DFUs) can avoid or delay any progression into more severe stages, which may require limb amputation or lead to infectious sequelae and death. However, frequent clinical screening would be too intrusive and costly, and self-examination may be hampered by concomitant diseases and social disabilities. In addition, it requires professional knowledge and experience using specialized devices. Researchers reported that skin temperature monitoring could reduce the risk of DFUs in high-risk patients. The main research objects in this field are e

Diabetic patients are at increased risk for adverse outcomes of surgery. These adverse outcomes are related to pre-existing complications of diabetes, especially atherosclerotic disease, nephropathy (and perhaps increased susceptibility to other renal toxins), and peripheral and autonomic neuropathy. Hyperglycemia is associated with likely risks for poorer wound healing, increased susceptibility to infection, and probable loss of administered nutrients through glycosuria. Insulin use has the flexibility of timing and dose in the postoperative management of most diabetic patients. The combinati

BACKGROUND: Hereditary transthyretin amyloidosis (hATTR/ATTRv) results from the deposition of misfolded transthyretin (TTR) throughout the body, including peripheral nerves. Inotersen, an antisense oligonucleotide inhibitor of hepatic TTR production, demonstrated a favorable efficacy and safety profile in patients with the polyneuropathy associated with hATTR in the NEURO-TTR (NCT01737398) study. We report longer-term efficacy and safety data for inotersen, with a median treatment exposure of 3 years.
METHODS: Patients who satisfactorily completed NEURO-TTR were enrolled in its open-label ext

While certain autoimmune conditions are more common in the young (e.g. Type 1 Diabetes), others are more frequent in the aged. A striking example is chronic inflammatory demyelinating polyneuropathy (CIDP), a CD4+ T cell-mediated autoimmune disease of the peripheral nervous system, which has a peak decade of onset of 70-79 years. How aging predisposes to autoimmunity, however, remains unclear. CD4+ T cells are highly susceptible to age-associated changes, including acquisition of immunosenescent features such as enhanced SA-β-gal activity, increased Cdkn1a (p21) expression, and upregulation of

Obesity is becoming increasingly common worldwide. Glucagon-like peptide-1 receptor agonist (GLP-1 RA) type drugs have the potential to help patients achieve considerable weight loss. Several benefits have been described in the short term, especially relating to diseases associated with obesity, where rapid weight loss results in undoubted improvements. These conditions include type 2 diabetes mellitus, cardiovascular disease and metabolic dysfunction-associated steatotic liver disease. Known adverse effects include gastrointestinal (nausea, vomiting, acute pancreatitis, dehydration and malnut

Cardiovascular autonomic failure arises from lesions across central nuclei, peripheral nerves, and autonomic ganglia. This review examines whether dysfunction of the neurovascular unit and neural barriers contributes to that failure. The clearest evidence comes from rodent models of hypertension and heart failure. Increased permeability in the PVN, NTS, and RVLM develops alongside sympathetic activation and impaired baroreflex control. Angiotensin II and inflammation appear to affect endothelial transport and junctional integrity. Exercise and interventions directed at these pathways improve b

The nucleotide-binding domain leucine-rich repeat and pyrin domain-containing protein-3 (NLRP3) inflammasome plays a central role in inflammatory diseases, including cardiovascular, gastrointestinal, neurodegenerative, autoimmune, hepatic, renal, and pulmonary disorders. Although various cellular pathways tightly regulate its activation, the precise mechanisms remain unclear. Emerging evidence highlights adenosine monophosphate-activated protein kinase (AMPK) as a critical regulator of energy balance and cellular metabolism, suggesting its potential involvement in modulating NLRP3 inflammasome

This article analyzes the progress of animal experiments on the analgesic mechanism of electroacupuncture (EA) at the central level for neuropathic pain (NP) in the past 10 years, and summarizes the analgesic mechanism of EA at the central level for NP. EA, as a safe and reliable treatment, can treat NP by regulating the release of nociceptive neurotransmitters and receptors, upregulating the expression of non-coding RNA (ncRNA), inhibiting the activation of microglia, Ca2+/calmodulin kinase II (CaMKII) phosphorylation, dendritic spine remodeling, endoplasmic reticulum stress (ERS), and glucos

Diabetes mellitus (DM) is marked by prolonged elevated blood glucose levels, which lead to the formation of covalent adducts between glucose and plasma proteins through a non-enzymatic reaction called glycation. This biochemical process plays a crucial role in the development of DM complications, including retinopathy, nephropathy, neuropathy, and cardiomyopathy, while also impacting conditions such as rheumatoid arthritis, osteoporosis, and aging. Glycation alters the molecular structure, enzymatic activity, and receptor interactions of proteins, affecting their normal functions. Advanced gly

Glucose interacts with human serum albumin (HSA, the main protein responsible for the biodistribution of drugs in the bloodstream) and consequently affects the binding capacity of exogenous compounds. Thus, in this work, the interactive profile between HSA and the anti-inflammatory drug nimesulide (NMD, used mainly by patients with diabetic neuropathy to relieve acute or chronic pains) was characterized in nonglycemic, normoglycemic (80 mg/dL), and hyperglycemic (320 mg/dL) conditions by biophysics techniques. There is a spontaneous and ground-state association HSA:NMD under physiological cond

Autoantibodies against contactin-associated protein 2 (Caspr2) not only induce limbic autoimmune encephalitis but are also associated with pain conditions. Here, we analyzed clinical data on pain in a large cohort of patients included into the German Network for Research in Autoimmune Encephalitis. Out of 102 patients in our cohort, pain was a frequent symptom (36% of all patients), often severe (63.6% of the patients with pain) and/or even the major symptom (55.6% of the patients with pain). Pain phenotypes differed between patients. Cluster analysis revealed two major phenotypes including mo

Background: Pentraxin 3 (PTX3) is an acute-phase protein, which resembles C-reactive protein in both structure and function, and belongs to the same family. PTX3 is associated with cardiovascular diseases, obesity, and metabolic syndrome (MetS). This study evaluated the relationship between serum PTX3 levels, prediabetes, newly diagnosed type 2 diabetes mellitus (T2DM), and other biochemical and clinical parameters in obese patients with nonalcoholic fatty liver disease (NAFLD). Methods: A total of 77 obese patients with NAFLD were included. Forty-seven of them were with normal glucose levels

AIMS: To clarify the mechanisms of hypersensitivity and spontaneous pain in intraoral structures in rats with diabetes mellitus (DM) accompanied by reduced saliva.
METHODS: Adult male Sprague-Dawley rats received a single injection of streptozocin (50 mg/kg) to induce DM. Saliva volume, intraoral hypersensitivity to menthol and capsaicin solutions, and head-withdrawal thresholds (HWTs) to noxious heat and mechanical stimulation of the tongue and whisker pad were measured.
RESULTS: On day 7 after streptozocin injection, rats with DM had a significantly reduced spontaneous saliva volume, polyd

Fibromyalgia is a common and challenging chronic pain disorder with few, if any, highly effective and well-tolerated treatments. Alpha-lipoic acid (ALA) is a nonsedating antioxidant with evidence of efficacy in the treatment of symptomatic diabetic neuropathy that has not been evaluated in the setting of fibromyalgia treatment. Thus, we conducted a single-centre, proof-of-concept, randomized, placebo-controlled, crossover trial of ALA for the treatment of fibromyalgia. Twenty-seven participants were recruited, and 24 participants completed both treatment periods of the trial. The median maxima

To investigate the incidence and prognostic factors of ethambutol-related optic neuropathy (EON) in one medical center of southern Taiwan, a retrospective chart review study with 4803 newly diagnosed tuberculosis cases from January 2002 to July 2011 at one medical center hospital in southern Taiwan were reviewed. Of these patients, 1004 had ophthalmic records. Sixty-two cases (1.29%) experienced visual impairment and were diagnosed as EON with mean visual acuity of 0.86 ± 0.69 by logMAR. Sixteen of the 62 patients had a follow-up time > 6 months. Of these, eight patients (50%) showed visual im

Thiamine or vitamin B1 is a water soluble vitamin of the vitamin B complex. It is synthetized by bacteria, fungi and plants and is an essential component of multicellular living organisms. Humans are not able to synthetize this vitamin and have to obtain it from different foods. Thiamin has a vital role in the normal function of the human body. It functions as a coenzyme in the catabolism of carbohydrates and amino acids and has an antioxidant role. It has an essential function in a series of metabolic processes related to energy production and conversion of sugar to ATP, as a catalyst in the
