Oncology imaging in the abdomen and pelvis: where cancer hides.

As the incidence of cancer continues to increase, imaging will play an ever more important role in the detection, diagnosis, staging, surveillance, and therapeutic monitoring of cancer. Diagnostic errors in the initial discovery of cancer or at follow-up assessments can lead to missed opportunities for curative treatments or altering or reinitiating therapies, as well as adversely impact clinical trials. Radiologists must have an understanding of cancer biology, treatments, and imaging appearance of therapeutic effects and be mindful that metastatic disease can involve virtually any organ syst

The role of diffusion tensor imaging and fractional anisotropy in the evaluation of patients with idiopathic normal pressure hydrocephalus: a literature review.

OBJECTIVE Diffusion tensor imaging (DTI) for the assessment of fractional anisotropy (FA) and involving measurements of mean diffusivity (MD) and apparent diffusion coefficient (ADC) represents a novel, MRI-based, noninvasive technique that may delineate microstructural changes in cerebral white matter (WM). For example, DTI may be used for the diagnosis and differentiation of idiopathic normal pressure hydrocephalus (iNPH) from other neurodegenerative diseases with similar imaging findings and clinical symptoms and signs. The goal of the current study was to identify and analyze recently publ

Eosinophilic Cerebrospinal Fluid Pleocytosis in A Dog With Hypertrophic Pachymeningitis and Generalized Eosinophilic Disease.

An 8-year-old Whippet crossbreed was presented with a month's history of collapsing episodes and altered mentation. Clinical improvement was noted upon administration of prednisolone prior to referral. Neurologic examination was compatible with a multifocal intracranial disease. Magnetic resonance imaging revealed thickening of the dura mater around the left parietal and temporal cerebral cortex, left piriform lobe, and cerebellar hemispheres. Cerebrospinal fluid (CSF) analysis revealed marked eosinophilic pleocytosis with increased protein concentration. Infectious diseases were ruled out. Th

From door to diagnosis: A case report of a rare and life-threatening presentation of Takayasu arteritis.

Takayasu arteritis is a chronic, large vessel vasculitis that typically affects the aorta and its main branches. We present a case of a 33-year-old male patient who was diagnosed with Takayasu arteritis after being admitted for acute cardiogenic shock requiring inotropes. Initial transthoracic echocardiography demonstrated severe biventricular dysfunction. Computed tomography imaging of his chest showed pulmonary infarcts. Patient subsequently developed sudden lower extremity weakness and bowel incontinence, and urgent magnetic resonance imaging of his spine demonstrated thoracolumbar vascular

Intracerebral hemorrhage as a complication of reversible cerebral vasoconstriction syndrome in the postpartum periods: a case report.

Reversible cerebral vasoconstriction syndrome (RCVS) is a cerebrovascular condition marked by the diffuse, reversible narrowing of multiple segments of cerebral arteries. This syndrome predominantly affects women and most commonly presents around the age of 42. It can develop spontaneously or be induced by factors such as the postpartum period or exposure to vasoactive substances. This case report describes an uncommon presentation of RCVS in a 24-year-old woman who developed an intraparenchymal hemorrhage shortly after a spontaneous vaginal delivery, despite the absence of conventional stroke

Sparganosis of the Cauda Equina: A Rare Case of Lower Thoracic Cord Edema and Diagnostic Challenges.

Sparganosis is a rare parasitic infection caused by the larvae of Spirometra species, with spinal involvement being exceedingly uncommon. We present the case of a 46-year-old woman with progressive lower limb weakness, sensory deficits, and bowel and bladder dysfunction over a 3-month period. Initial magnetic resonance imaging revealed an abnormal intradural lesion from L2 to S2, along with thoracic spinal cord edema, raising suspicion for a spinal dural arteriovenous fistula (DAVF). After referral to our institution, further imaging ruled out DAVF, and findings suggested arachnoiditis and rad

Primary hepatic lymphoma in a patient with cirrhosis: a case report.

BACKGROUND: Primary hepatic lymphoma is a very uncommon disease. Due to its nonspecific clinical, laboratory, and imaging findings, it is often misdiagnosed. Liver biopsy is required to make a final diagnosis. Chemotherapy is the current gold standard of treatment. CASE PRESENTATION: An asymptomatic 65-year-old Caucasian man with Child-Pugh class A cirrhosis presented to our hospital with a nodular lesion seen on a routine surveillance abdominal ultrasound. His physical examination revealed hepatomegaly and no other significant findings. Magnetic resonance imaging of the abdomen showed a volu

Preterm white matter injury: ultrasound diagnosis and classification.

White matter injury (WMI) is the most frequent form of preterm brain injury. Cranial ultrasound (CUS) remains the preferred modality for initial and sequential neuroimaging in preterm infants, and is reliable for the diagnosis of cystic periventricular leukomalacia. Although magnetic resonance imaging is superior to CUS in detecting the diffuse and more subtle forms of WMI that prevail in very premature infants surviving nowadays, recent improvement in the quality of neonatal CUS imaging has broadened the spectrum of preterm white matter abnormalities that can be detected with this technique.

Anterior interosseous nerve palsy mimicking rupture of the index flexor digitorum profundus after volar locking plate fixation of a distal radius fracture.

We describe the case of a patient with distal radius fracture who became unable to flex the distal interphalangeal joint of the index finger after internal fixation using a volar locking plate. There was palpable crepitus with active thumb motion, and wrist radiographs showed prominence of the volar plate at the watershed line. Therefore, our initial diagnosis was plate-induced closed rupture of the flexor digitorum profundus tendon of the index finger. However, upon surgical removal of the plate, no tendon rupture was found. Magnetic resonance imaging after plate removal showed diffuse increa

Primary neurolymphomatosis of the lower cranial nerves presenting as Dysphagia and hoarseness: a case report.

Primary neurolymphomatosis is an extremely rare tumor. We report the case of a 74-year-old patient presenting with dysphagia and hoarseness. Initial contrast-enhanced computed tomography of the head, neck, and chest did not reveal any lesions. His symptoms improved with short-term administration of prednisone but recurred and deteriorated. Magnetic resonance (MR) imaging revealed a tumor along the ninth and tenth cranial nerves across the jugular foramen. Fluorine-18 fluorodeoxyglucose positron emission tomography indicated this was a primary tumor. Repeated MR imaging after 2 months revealed

Diagnosis of adenomyosis: a review.

This article reviews the various preoperative diagnostic methods for uterine adenomyosis. MEDLINE and PubMed were searched using the keywords adenomyosis and adenomyosis diagnosis. Reviews, case-controlled studies and reports published from 1949 through March 2005 and written, or at least abstracted, in English were analyzed. Transvaginal sonography is superior to the transabdominal route, but its diagnostic sensitivity is still suboptimal, ranging from 50% to 87%. Magnetic resonance imaging is most effective for both diffuse and focal adenomyosis, with sensitivity and specificity that are com

White matter changes in elderly people: MR-pathologic correlations.

Magnetic resonance (MR) imaging features of white matter lesions, often seen in the elderly, are correlated with histologic findings. Dilatation of perivascular spaces is seen, especially in the frontal and/or parietal subcortical white matter; the spaces are less than 3 mm in diameter and have sharp margins with no perifocal abnormality. Old lacunar infarcts are larger than 3 mm in diameter and are irregularly shaped and accompanied by perifocal myelin pallor and gliosis. Periventricular hyperintensity, including cap and rim, histologically shows myelin pallor, dilatation of perivascular spac

Leptomeningeal malignant melanoma arising in neurocutaneous melanocytosis: a case report.

A rare case of histology-proved giant congenital melanocytic nevus (GCMN) with symptomatic leptomeningeal melanocytosis is reported. A 26-year-old man had had a large patch of pigmented nevus over his back and left arm since birth. He had begun to have seizures as well as symptoms and signs of increased intracranial pressure about six months before admission. Serial computed tomography of brain showed hydrocephalus, diffuse leptomeningeal enhancement and multiple well-enhanced, rapid-growing nodules on the surface of the cerebellum and left parietal lobe. Magnetic resonance imaging (MRI) revea

MR findings in human fascioliasis.

Fasciola hepatica is a trematode of herbivorous mammals, and man is infected by accidental ingestion of contaminated raw aquatic vegetables or water. There have been many reports on computed tomographic and cholangiographic features of human fascioliasis. However, findings of magnetic resonance (MR) imaging have not been reported. Hepatic fascioliasis produces three types of lesions in MR images arranged in tract-like fashion. The outermost area presents as an iso-signal area in T1WI, with slightly higher signal intensity in T2WI and diffuse enhancement after i.v. contrast. The second type pre

Association of Acute Ischemic Stroke Volume With Post-Stroke Epilepsy Development.

BACKGROUND AND PURPOSE: Stroke is a leading cause of epilepsy, especially in older adults. The SeLECT score remains the standard among post-stroke epilepsy (PSE) prediction tools. However, its broader validation is limited by the need to manually extract neuroimaging predictors (cortical and middle cerebral artery [MCA] involvement). Unlike the CAVE score, SeLECT did not evaluate acute stroke volume, which can now be quantified automatically. We aimed to determine whether stroke volume independently predicts PSE and compare its predictive contribution to SeLECT's neuroimaging variables. METHO

Exploring the link between intravoxel incoherent motion measured brain diffusivity during wakefulness and sleep macrostructure in the elderly.

Intravoxel incoherent motion (IVIM) is a diffusion-weighted magnetic resonance imaging (MRI) method that models slow (D, tissue diffusivity) and fast (D*, microvascular perfusion) signal components (f, signal fraction). This observational study explores the relationship between IVIM metrics and sleep macrostructure and physical activity levels in nineteen healthy elderly individuals (mean age 72.2 ± 5.0). Participants underwent two MRI sessions during wakefulness, four weeks apart, on a GE 3T Premier scanner. IVIM parameters (D, D*, f and their product fD*) were calculated using non-linear lea
