Correlation between the Antibiotic Resistance Genes and Susceptibility to Antibiotics among the Carbapenem-Resistant Gram-Negative Pathogens.

In this study, the correlation between the antibiotic resistance genes and antibiotic susceptibility among the carbapenem-resistant Gram-negative pathogens (CRGNPs) recovered from patients diagnosed with acute pneumonia in Egypt was found. A total of 194 isolates including Klebsiella pneumoniae (89; 46%), Escherichia coli (47; 24%) and Pseudomonas aeruginosa (58; 30%) were recovered. Of these, 34 (18%) isolates were multiple drug resistant (MDR) and carbapenem resistant. For the K. pneumoniae MDR isolates (n = 22), blaNDM (14; 64%) was the most prevalent carbapenemase, followed by blaOXA-48 (1

Draft genome sequence of carbapenem-resistant Shewanella algae strain AC isolated from small abalone (Haliotis diversicolor).

OBJECTIVES: Shewanella algae is an environmental marine bacterium and an emerging opportunistic human pathogen. Moreover, there are increasing reports of strains showing multidrug resistance, particularly carbapenem-resistant isolates. Although S. algae has been found in bivalve shellfish aquaculture, there is very little genome-wide data on resistance determinants in S. algae from shellfish. The aim of this study was to determine the whole genome sequence of carbapenem-resistant S. algae strain AC isolated from small abalone in Taiwan. METHODS: Bacterial genomic DNA was sequenced using an Il

Resistance to CCR5 antagonists.

PURPOSE OF REVIEW: CCR5 antagonists disrupt crucial interactions between HIV envelope glycoprotein gp120 and CCR5, preventing virus binding and entry. Current antiretroviral agents target viral proteins, whereas CCR5 antagonists bind to the host cell. This novel mechanism of action is posing new challenges to our understanding of drug resistance. RECENT FINDINGS: Possible mechanisms of resistance to CCR5 antagonists include the selection of CXCR4-tropic variants and the continued use of CCR5. Recent in-vitro data suggest that true co-receptor 'switch' may require the sequential accumulation o

Gastroduodenal mucosal defense.

PURPOSE OF REVIEW: The gastroduodenum has multiple means by which it resists injury from intrinsic and extrinsic factors, including gastric acid, nonsteroidal anti-inflammatory drugs, and Helicobacter pylori. We review recent insights into the mechanisms by which the gastroduodenum resists injury and discuss factors contributing to defensive failure. RECENT FINDINGS: Duodenal bicarbonate secretion, a primary defensive mechanism, is mediated by the downregulated in adenoma anion exchanger and is stimulated by estrogens. Nonsteroidal anti-inflammatory drug gastric damage is dependent on toll-li

Antimicrobial resistance programs in Canada 1995-2010: a critical evaluation.

BACKGROUND: In Canada, systematic efforts for controlling antibiotic resistance began in 1997 following a national Consensus Conference. The Canadian strategy produced 27 recommendations, one of which was the formation of the Canadian Committee on Antibiotic Resistance (CCAR). In addition several other organizations began working on a national or provincial basis over the ensuing years on one or more of the 3 identified core areas of the strategy. Critical evaluation of the major programs within Canada which focused on antimicrobial resistance and the identified core components has not been pr

Heat Shock Proteins as Targets for Cancer Therapeutics.

Introduction: Heat shock proteins (HSPs) are stress-responsive molecular chaperones that are frequently dysregulated in cancer and contribute to tumorigenesis, invasion, metastasis, immune interactions, and resistance to therapy. Distinct HSP families, including HSP27, HSP60, HSP70, HSP90, and HSP110, promote malignant progression through complementary effects on apoptosis regulation, mitochondrial function, proteostasis, and stabilization of oncogenic signaling pathways. This makes HSPs attractive therapeutic targets. Their coordinated roles within stress-adaptive chaperone networks further g

Comparative Analyses Suggest Genome Stability and Plasticity in Stenotrophomonas maltophilia.

Stenotrophomonas maltophilia (S. maltophilia) is a multidrug-resistant opportunistic pathogen. There are an increasing number of case reports on S. maltophilia infections in recent years, and the species is becoming a public health concern. Many studies have focused on profiling and pangenome of the species, particularly on their antibiotic resistance and virulence genes. However, there is a lack of studies on mobile genetic elements (MGEs), a subset of pangenome that significantly contributes to the diversity, stability, and plasticity of a population. In this study, 20 genomes of S. maltophi

Novelty and nuance in the intensive care unit: new options to combat multidrug resistant pneumonia.

PURPOSE OF REVIEW: To describe the increasing burden of multidrug resistant (MDR) Gram-negative pathogens in severe pneumonia and to examine the clinical trials supporting a role for novel agents for the treatment of this infection. RECENT FINDINGS: MDR Gram-negative bacteria cause an increasing proportion of severe pneumonias. Although the epidemiology of resistance varies across the globe, all regions have seen an evolution in resistance, especially among Enterobacterales spp, Pseudomonas aeruginosa, and Acinetobacter bumannii. Fortunately, several clinical trials have established the role

Using the Bacterial Ribosome as a Discovery Platform for Peptide-Based Antibiotics.

The threat of bacteria resistant to multiple antibiotics poses a major public health problem requiring immediate and coordinated action worldwide. While infectious pathogens have become increasingly resistant to commercially available drugs, antibiotic discovery programs in major pharmaceutical companies have produced no new antibiotic scaffolds in 40 years. As a result, new strategies must be sought to obtain a steady supply of novel scaffolds capable of countering the spread of resistance. The bacterial ribosome is a major target for antimicrobials and is inhibited by more than half of the a

Modeling HIV multiple infection.

Multiple infection of target cells by human immunodeficiency virus (HIV) may lead to viral escape from host immune responses and drug resistance to antiretroviral therapy, bringing more challenges to the control of infection. The mechanisms underlying HIV multiple infection and their relative contributions are not fully understood. In this paper, we develop and analyze a mathematical model that includes sequential cell-free virus infection (i.e.one virus is transmitted each time in a sequential infection of target cells by virus) and cell-to-cell transmission (i.e.multiple viral genomes are tr

Osteomyelitis related lameness across animal species: pathophysiology, etiology, epidemiology and experimental model approaches.

Osteomyelitis is an inflammatory disorder resulting from the invasion of opportunistic pathogens into bones and bone marrows, causing necrosis and affecting multiple animal groups. It represents one of the most prevalent musculoskeletal diseases, imposing substantial economic losses and raising significant animal welfare concerns across livestock and poultry industries. The pathophysiology of osteomyelitis is characterized by mechanisms of disease development, clinical age, the specific anatomic site in the bone, and structural differences between adults and juveniles. The inability of animals

Small-molecule RAS/RAF inhibitors target RAS-driven cancers via allosteric RAF disruption.

RAS, a frequently mutated cancer-driver gene, has been the focus of intensive research, with numerous inhibitors developed to target RAS and its signalling molecules. With the advent of approved subtype-specific RAS inhibitors targeting KRASG12C mutation, the development of RAS inhibitors that are effective regardless of RAS mutation status is the next major challenge to address clinically prevalent RAS mutations and further overcome RAS-driven acquired resistance to currently available drugs. With the goal of obtaining broad-spectrum aberrant RAS-signalling inhibitors, we conduct multimodule

The genomic profiling of high-risk smoldering myeloma patients treated with an intensive strategy unveils potential markers of resistance and progression.

Smoldering multiple myeloma (SMM) precedes multiple myeloma (MM). The risk of progression of SMM patients is not uniform, thus different progression-risk models have been developed, although they are mainly based on clinical parameters. Recently, genomic predictors of progression have been defined for untreated SMM. However, the usefulness of such markers in the context of clinical trials evaluating upfront treatment in high-risk SMM (HR SMM) has not been explored yet, precluding the identification of baseline genomic alterations leading to drug resistance. For this reason, we carried out next

Compound amino acid synergizes ceftazidime-avibactam to eradicate extracellular and facultative intracellular MDR pathogens.

With the rise in antibiotic-resistant infections, enhancing the efficacy of currently available antibiotics is crucial to address this crisis. In this study, we demonstrate that the clinically approved amino acid formulation 18AA (18 amino acids) synergistically enhances ceftazidime-avibactam (CZA) efficacy against extracellular and intracellular multidrug-resistant (MDR) pathogens, including carbapenem-resistant (CR) strains. The CZA+18AA combination eliminated CR Escherichia coli (CR-ECO) and MDR Edwardsiella tarda. Synergy was consistently demonstrated across all 10 tested clinical isolates

Amikacin-eravacycline combination mediates the synergistic elimination of carbapenem-resistant pathogens via in vitro and in vivo metabolic reprogramming.

Carbapenem-resistant (CR) organisms (CRO) have been identified as critical priority pathogens, emphasizing the urgent need for novel therapeutic strategies. Combination therapy emerges as a promising approach to address multidrug-resistant bacterial infections. Here we demonstrate that eravacycline (ERV), in combination with amikacin (AMK), effectively eliminates a panel of clinically isolated CR Escherichia coli, CR Klebsiella pneumoniae, and CR Acinetobacter baumannii. Mechanistically, the AMK-ERV combination enhances bacterial oxidative phosphorylation, leading to an accumulation of reactiv
