[Mild head trauma: complications and acousticovestibular sequelae].

Head injury with fracture of the petrous bone is frequent and directly related to the development of human activity. Head trauma is associated with nearly 75% of traffic accidents; 5% of them with petrous bone fracture. Based on a clinical case, the acute injuries and secondary acoustic or vestibular sequelae after low-energy cranio-cerebral trauma are discussed here. The subject was a victim of a low-energy head trauma with brief loss of consciousness and translabyrinthine fracture of the petrous bone. This fracture caused invalidating equilibrium disorders related to a perilymphatic fistula.

Temporal bone findings on computed tomography imaging in branchio-oto-renal syndrome.

OBJECTIVES/HYPOTHESIS: To describe temporal bone findings using visual inspection and direct measurement on computerized tomography (CT) in individuals with branchio-oto-renal syndrome (BOR). We ask if it is possible for the untrained observer to use a battery of CT observations as a tool in the overall evaluation of the BOR phenotype. STUDY DESIGN: Retrospective evaluation of CT findings in individuals with a clinical diagnosis of BOR based on criteria derived from genotype-phenotype analyses. METHODS: Prospective measurement of temporal bone CT imaging in 21 individuals (42 ears) with BOR

Prevention of cerebrospinal fluid leakage and delayed loss of preserved hearing after vestibular schwannoma removal: reconstruction of the internal auditory canal in the suboccipital transmeatal approach--technical note.

The suboccipital transmeatal approach uses packing of a muscle or fat graft into the internal auditory canal (IAC) to prevent postoperative cerebrospinal fluid (CSF) leakage. However, preserved hearing after removal of vestibular schwannomas may decline over time because of the progressive constriction of cochlear vascular supply due to scarring of the IAC. We propose a surgical technique for IAC reconstruction, which separates the preserved cochlear nerve and vasculature from the graft, and regains the CSF space in the IAC. Prior to the drilling of the posterior wall of the IAC, the dura mate

A comparison of inter-aural attenuation with the Etymotic ER-3A insert earphone and the Telephonics TDH-39 supra-aural earphone.

One of the many reported advantages of the insert earphone over the supra-aural earphone is increased inter-aural attenuation (IA). Minimum values of IA determine the need for masking of the non-test ear in air-conduction audiometry. The aim of the present study was to measure inter-aural attenuation for the Etymotic Research ER-3A insert earphone (with deep and shallow insertion of the ear plug within the ear canal) and compare this with the supra-aural Telephonics TDH-39/MX41-AR earphone/cushion combination. Subjects were 18 adults ranging in age from 38 to 68 years (mean 50 years). Each sub

Complications and their management in tympanomastoid surgery.

Chronic diseases of the ear and their surgical treatment can endanger the delicate structures of the temporal bone in close relationship with the middle ear. The experience and expertise of the otologic surgeon, based on perfect knowledge of the complex anatomy of the whole temporal bone, are unfortunately built up partly through the management of complications encountered in tympanomastoid surgery and may require knowledge of basic neuro-otologic procedures. Paralysis and paresis of the facial nerve and labyrinthine, and dural and vascular injuries may have consequences not only in the final

ABR audiometry in evaluation of hearing organ in neonates from high-risk pregnancy.

In a group of 130 tested newborns, pathological brain stem responses derived from acoustic stimulus was achieved in 28 (21.5%) cases. 85.2% of newborns from this group of birthed babies comparised a high-risk pregnancy group. Based on the criteria of evaluating ABR parameters of the achieved responses, three categories of hearing dysfunctions were distinguished: conductive hearing loss in 7 cases, sensori-neural in 12 cases and neurological dysfunctions in 9 cases. Control testing which was carried out after three months following the initial evaluation, allowing for the elimination of 12 chil

Factors affecting the development of cerebrospinal fluid leak and meningitis after translabyrinthine acoustic tumor surgery.

Meningitis and cerebrospinal fluid (CSF) leak are serious complications of acoustic tumor surgery. Previous reports have varied in the incidence of and the predisposing factors to these complications. This study reviews a series of 723 acoustic tumors removed via the translabyrinthine approach at the House Ear Clinic in Los Angeles. The incidences of CSF leak and meningitis were 6.8% and 2.9%, respectively. The patients who developed these problems were compared to the remainder of the study population for differences in age at surgery, tumor size, operative time, and length of hospital stay.

Apparent travelling wave velocity changes in cases of endolymphatic hydrops.

This study attempted to develop a test which would be specific for endolymphatic hydrops. It is based upon the hypothesis that increased pressure in the scala media will alter the stiffness of the basilar membrane and hence increase the speed of the travelling wave. Auditory brainstem-derived responses were used to measure the basilar membrane travelling wave velocity in normals and to take the equivalent measures in Meniere's patients. Ten normally hearing subjects and 20 Meniere's patients were tested. The data for the Meniere's group showed that the travelling wave velocities were within no

Investigations into the cause of vertigo in sudden sensorineural hearing loss.

In this light microscopic study of the temporal bone, an attempt has been made to find a morphologic correlate of vertigo associated with idiopathic sudden sensori-neural hearing loss (ISSHL). Hair cell densities of the three cristae and both maculae, as well as vestibular ganglion cell (neuronal) count estimation, was done in nine ears that had documented histories of ISSHL. There were five ears with vertigo and four without. These quantitative data--i.e., hair cell densities and neuronal counts, of the vertiginous ears (group I) and nonvertiginous ears (group II)--was compared by histograms

Pathology of endolymphatic hydrops.

The term 'endolymphatic hydrops' is not a definitive diagnosis since the condition occurs in a variety of otological disorders. Endolymphatic hydrops was found in 57 ears (9 per cent) of the 703 temporal bones in the collection of the Ear Research Institute, Los Angeles. These 57 ears included normal ears of two newborns, post-stapedectomy ears, and ears with the primary diagnosis of Menière's disease. Paget's disease, congenital syphilis, and chronic otitis media. The typical clinical picture of fluctuant hearing loss, pressure, tinnitus, and episodic vertigo occurred in about one-third of th

Continuous vestibular implant stimulation partially restores eye-stabilizing reflexes.

BACKGROUNDBilateral loss of vestibular (inner ear inertial) sensation causes chronically blurred vision during head movement, postural instability, and increased fall risk. Individuals who fail to compensate despite rehabilitation therapy have no adequate treatment options. Analogous to hearing restoration via cochlear implants, prosthetic electrical stimulation of vestibular nerve branches to encode head motion has garnered interest as a potential treatment, but prior studies in humans have not included continuous long-term stimulation or 3D binocular vestibulo-ocular reflex (VOR) oculography

Check-list for the assessment of functional impairment in children with congenital aural atresia.

OBJECTIVES: Congenital Aural Atresia (CAA) is a deformity of the external ear and it is commonly associated with malformations of middle and inner ear and, in some cases, with other facial deformities. Very few assessment measures exist for evaluating the functional impairment in children with CAA. Purpose of this study is to introduce and describe an assessment Checklist, (nominated FOS Checklist) that covers feeding abilities (F), oralmotor skills (O), communication/language development (S) in children with CAA. FOS wants to offer a range of assessment providing a profile of the child in com

Histopathological evaluation and long-term results of soft tissue preservation technique in cholesteatoma surgery.

The goal of cholesteatoma surgery is total removal of the cholesteatoma matrix and prevention of recurrence. Preservation of soft tissue in the attic is reported to improve post-operative middle ear aeration, and thus prevents recurrence. However, the histology and nature of the preserved tissue have rarely been reported. The aim of this study is to clarify the histology of the preserved soft tissue in cholesteatoma surgery, and to show its relationship to the clinical course. Surgical specimens were obtained from ten patients with pars flaccida-type cholesteatoma. In these patients, cholestea

Long-term survival of LGR5 expressing supporting cells after severe ototoxic trauma in the adult mouse cochlea.

INTRODUCTION: The leucine-rich repeat-containing G-protein coupled receptor 5 (LGR5) is a tissue resident stem cell marker, which it is expressed in supporting cells (SCs) in the organ of Corti in the mammalian inner ear. These LGR5+ SCs can be used as an endogenous source of progenitor cells for regeneration of hair cells (HCs) to treat hearing loss and deafness. We have recently reported that LGR5+ SCs survive 1 week after ototoxic trauma. Here, we evaluated Lgr5 expression in the adult cochlea and long-term survival of LGR5+ SCs following severe hearing loss. METHODS: Lgr5GFP transgenic mi
