Neonatal separation of distal humeral epiphysis during Caesarean section: a case report.

We report a rare injury of distal humeral epiphyseal separation in a newborn during a Caesarean section. The diagnosis was made using magnetic resonance imaging. Emergency Caesarean deliveries with considerable traction applied to extract the baby can be the risk factor of such an injury.

[Surgical treatment of type I capitellar-humeral fractures using the Henry ventral approach approach. Short- and medium-term results].

Capitellar fractures are rare. Despite adequate surgical treatment, patients suffer frequently from limited range of motion, osteonecrosis, and neurological deficits. The purpose of this retrospective study was to evaluate short- and medium-term surgical results in seven patients who had type I capitellar fractures according to Bryan and Morrey. All patients were treated via an anterior approach, according to Henry, and open reduction and internal fixation with two screws. Seven patients were examined clinically, neurologically, and radiologically after 10-65 months. The fracture healed in all

Biomechanical models for the pathogenesis of specific distal upper extremity disorders.

BACKGROUND: Knowledge of the pathogenesis of most disorders that occur in the distal upper extremity is generally lacking. The individual roles of postulated etiologic factors, such as biomechanical or psychosocial exposures, are poorly understood and their potential interactions even less so. This article proposes biomechanical or physiological models of pathogenesis for specific distal upper extremity disorders. METHODS: Tendon entrapment of the dorsal wrist compartments (tenosynovitis), peritendinitis, lateral epicondylitis, and carpal tunnel syndrome are common specific neuromusculoskelet

Displaced supracondylar fractures of the humerus in children. Audit changes practice.

We performed an audit of 71 children with consecutive displaced, extension-type supracondylar fractures of the humerus over a period of 30 months. The fractures were classified according to the Wilkins modification of the Gartland system. There were 29 type IIA, 22 type IIB and 20 type III. We assessed the effectiveness of guidelines proposed after a previous four-year review of 83 supracondylar fractures. These recommended that: 1) an experienced surgeon should be responsible for the initial management; 2) closed or open reduction of type-IIB and type-III fractures must be supplemented by sta

Arthroscopic excision of type II capitellar fractures.

Type II capitellar fractures are uncommon. The current treatment recommendation for these fractures is open excision. The major complication of open excision is loss of motion. Two cases of the previously unreported technique of arthroscopic excision are presented with a review of the literature. Arthroscopic excision of displaced type II capitellar fractures is a safe, effective technique that causes minimal soft tissue trauma, allows thorough inspection of the joint for additional occult injuries, and optimizes postoperative range of motion.

[Complications of supracondylar fractures of the humerus in children].

The supracondylar fracture of the humerus is the commonest type of elbow fracture in children. The peak incidence for this injury is between 5 and 10 years of age. The complications of these displaced fractures remain challenging problems. The early complications are mostly the neuro-vascular injuries. The difficulty in maintaining the reduction and ultimately the cubitus varus is the most frequent long-term complication with an incidence ranging from 3 to 57%. Adequate reduction and pinning has dramatically decreased this incidence. This paper describes these complications and their treatment

[Fractures of the radius neck associated with ulna fractures in children].

The author has described one of the typical combinations of injuries in the elbow joint area in children, i.e. fractures of the neck of the radius with a simultaneous fracture of the ulna. The study is based on 37 cases observed by the author. Two types of the above-mentioned fractures have been distinguished: abduction (32) and extension (5) fractures. The methods of closed reposition of each type have been presented. These methods allow to reach the desired results even in major dislocations (60-90 degrees) of the head of the radius.

Preoperative Scaption and External Rotation Strength Deficits Are Predictive of Retear After Rotator Cuff Repair.

PURPOSE: To investigate the role of preoperative shoulder muscle strength as a predictive factor of retear after rotator cuff repair. METHODS: This retrospective study reviewed factors related to rotator cuff retears in patients who underwent arthroscopic rotator cuff repair. The patients included in the study were those who underwent surgery for full-thickness rotator cuff tears diagnosed on preoperative magnetic resonance imaging (MRI), or for partial-thickness tears involving more than 80% of the tendon thickness with an anterior-posterior tear diameter of at least 1.5 cm. The study subjec

Forearm Flexor Strain and Subsequent Ulnar Collateral Ligament Surgery Risk in Major League Baseball Pitchers: A Covariate-Matched Survival Analysis.

BACKGROUND: Ulnar collateral ligament (UCL) injury remains a significant source of time loss for baseball pitchers despite work identifying biomechanical, workload, and velocity risk factors. Because medial elbow stability also depends on the contribution of the forearm flexor-pronator complex, forearm flexor strains may be a marker of downstream UCL risk. PURPOSE: To quantify subsequent UCL surgery risk among Major League Baseball (MLB) pitchers with a nonoperatively managed forearm flexor strain who returned to play, and to evaluate whether return timing and pitching characteristics predict

Return to weightlifting following anatomic and reverse shoulder arthroplasty.

BACKGROUND: Weightlifting is a common form of recreational activity that can place higher levels of stress on the shoulder joint and is of particular interest to many patients undergoing shoulder arthroplasty. Despite the growing number of individuals receiving anatomic (aTSA) and reverse total shoulder arthroplasty (rTSA), recommendations regarding return to weightlifting remain unclear. The purpose of this study was to report the rate of return to weightlifting following primary shoulder arthroplasty and to evaluate postoperative function and performance outcomes. METHODS: A retrospective r
