Oral amoxicillin-clavulanate: an alternative treatment to refractory Nocardia keratitis-a case report.

BACKGROUND: Bacterial keratitis (bacterial corneal infection) is a serious, vision-threatening disease caused by a wide range of bacterial species. The gram-positive bacterium Nocardia spp. is increasingly recognized in keratitis, yet is particularly challenging to treat due to delayed diagnosis and limited efficacy of current ophthalmic therapeutics. Antimicrobial resistance among clinical Nocardia isolates is also rising, further complicating patient outcomes. Herein, we report a case of Nocardia amikacinitolerans keratitis, resistant to amikacin, ciprofloxacin, moxifloxacin, clarithromycin,

Uncommon Factors Leading to Nephrotic Syndrome.

Nephrotic syndrome (NS) is characterized by proteinuria, hypoalbuminemia, edema, and hyperlipidemia. Apart from the traditional causes of NS, such as minimal change disease, focal segmental glomerulosclerosis, diabetes, infections, malignancies, autoimmune conditions, and nephrotoxic agents, there are also rare causes of NS, whose knowledge is of the utmost importance. The aim of this article was to highlight the less well-known causes that have a significant impact on diagnosis and treatment. Genetic syndromes such as Schimke immuno-osseous dysplasia, familial lecithin-cholesterol acyltransfe

Stevens Johnson Syndrome/Toxic Epidermal Necrolysis - Diagnostic Testing and Treatment for Ophthalmologists.

Epidermal necrolysis (EN) is a very rare, but life-threatening dermatological disease where the skin and various mucous membranes react to drugs (75%) or systemic infections (25%). If up to 10% of the body surface is affected, the disease is called Stevens Johnson syndrome (SJS); if more than 30% of the body surface is affected, it is called toxic epidermal necrolysis (TEN); if 10 - 30% of the body surface is affected, it is called SJS/TEN overlap. In the acute phase of the disease, 80% of cases show ophthalmic involvement (from mild conjunctivitis to total de-epithelialisation of the ocular s

Unilateral Proptosis as the Initial Manifestation of Acute Myeloid Leukemia in a Young Adult.

A 22-year-old male presented with progressive unilateral proptosis, conjunctival chemosis, and vision loss in the right eye over the course of one month, without associated systemic symptoms such as fever, fatigue, or infections. Initial imaging, including CT and MRI, suggested benign orbital conditions like cavernous hemangioma or orbital pseudotumor. Despite the absence of systemic signs, hematological investigations revealed anemia, thrombocytopenia, and leukocytosis. A peripheral blood smear raised suspicion of a hematologic malignancy, which was confirmed by bone marrow biopsy, diagnosing

Novel hydroxy naphthol blue-based recombinase polymerase amplification for the rapid detection of Staphylococcus pseudintermedius.

Staphylococcus pseudintermedius (S. pseudintermedius) is a significant cause of pyoderma, soft tissue, urinary tract, and ear infections in cats and dogs. Bacterial culture and biochemical phenotypic assays are the gold standards for clinical diagnosis of bacteria but molecular methods have now been developed to identify and differentiate S. pseudintermedius. However, these methods require complex and expensive equipment, restricting usage in many laboratories. This study developed a rapid, specific, and sensitive detection method for S. pseudintermedius based on recombinase polymerase amplifi

Polymer-Based Nanoparticles as Drug Delivery Systems for Purines of Established Importance in Medicine.

Many purine derivatives are active pharmaceutical ingredients of significant importance in the therapy of autoimmune diseases, cancers, and viral infections. In many cases, their medical use is limited due to unfavorable physicochemical and pharmacokinetic properties. These problems can be overcome by the preparation of the prodrugs of purines or by combining these compounds with nanoparticles. Herein, we aim to review the scientific progress and perspectives for polymer-based nanoparticles as drug delivery systems for purines. Polymeric nanoparticles turned out to have the potential to augmen

Is RPGR-related retinal dystrophy associated with systemic disease? A case series.

BACKGROUND: Ciliopathies responsible for retinitis pigmentosa can also cause systemic manifestations. RPGR is a ciliary gene and pathogenic variants in RPGR cause a retinal ciliopathy, the commonest cause of X-linked recessive retinitis pigmentosa. The RPGR protein interacts with numerous other ciliary proteins present in the transition zone of both motile and sensory cilia, and may play an important role in regulating ciliary protein transport. There has been a growing, putative association of RPGR variants with systemic ciliopathies: mainly sino-respiratory infections and primary ciliary dys

Aerobic Commensal Conjunctival Microflora in Healthy Donkeys.

From a clinical point of view, knowledge of the commensal microbial flora of the conjunctival sac in healthy individuals proves to be of great importance. The aim of this study was to assess the presence and the composition of normal ocular microflora of healthy donkeys. Fourteen clinically healthy donkeys (Equus asinus) participated in the study. After prior ophthalmological examination, which showed no abnormalities, a conjunctival swab was taken from each donkey from the right and left eye. Species-specific identification was based on a morphological assessment of bacterial colonies stained

Host Molecules That Promote Pathophysiology of Ocular Herpes.

Herpes simplex virus type-1 (HSV-1) is a human virus that causes lifelong infections in a large population worldwide. Recurrence of HSV-1 from latency in trigeminal ganglion (TG) is the trigger of the morbidities seen with this virus. In addition to causing fever blisters and cold sores, occasionally the virus can also cause corneal lesions resulting in blindness in untreated individuals. Several host cell proteins play important roles in HSV-1 infection of the eye. HSV-1 enters into the corneal epithelial cells via its interactions with cell surface receptors. In parallel, the Toll-like recep

Five new cases of syndromic intellectual disability due to KAT6A mutations: widening the molecular and clinical spectrum.

BACKGROUND: Pathogenic variants of the lysine acetyltransferase 6A or KAT6A gene are associated with a newly identified neurodevelopmental disorder characterized mainly by intellectual disability of variable severity and speech delay, hypotonia, and heart and eye malformations. Although loss of function (LoF) mutations were initially reported as causing this disorder, missense mutations, to date always involving serine residues, have recently been associated with a form of the disorder without cardiac involvement. RESULTS: In this study we present five new patients, four with truncating mutat
