Radiation-Induced Soft Tissue Injuries in Patients With Advanced Mandibular Osteoradionecrosis: A Preliminary Evaluation and Management of Various Soft Tissue Problems Around Radiation-Induced Osteonecrosis Lesions.

OBJECTIVES: Radiation-induced soft-tissue injuries (STIs) in mandibular osteoradionecrosis (ORN) are not well studied regarding their correlations with nearby bone lesions. The aim of this study is to investigate the severity of radiation-induced STIs in advanced mandibular ORN and its relationship with hard-tissue damage and postoperative outcomes. METHODS: A retrospective study was performed in our institution from January 2017 to December 2019. Aside from demographic factors, the associations between the triad ORN variables (irradiation doses, ORN stages, ORN sizes) and radiation-related S

3-D titanium mesh reconstruction of defective skull after frontal craniectomy in traumatic brain injury.

INTRODUCTION: Decompressive craniectomy (DC) is a treatment strategy used to reduce intracranial pressure in patients with traumatic brain injuries. However, this procedure has a number of shortcomings, such as excessive sinking of the skin flap, which can lead to cerebral compromise and negatively affect the appearance of the patient. The reconstruction of skull defects has been proposed as a means to overcome these disadvantages. Few previous studies have reported the reconstruction of frontal skull defects using titanium mesh. The aim of this study was to provide a comprehensive review of a

Emotional artificial intelligence at the edge for tele-rehabilitation.

BACKGROUND AND OBJECTIVE: Tele-Rehabilitation (TR) has gained increasing attention in recent years due to its potential to reduce costs and overcome physical barriers compared with traditional in-person treatments. Within the framework of the Tele-Rehabilitation as a Service (TRaaS) research project, funded by the Italian Ministry of University and Research, previous work focused on developing a Cloud/Edge infrastructure to deliver remote rehabilitation to patients with Severe Acquired Brain Injuries (SABI), including cerebral stroke, Alzheimer's, and Parkinson's diseases. Building upon that w

Patterns of Third-Molar-Pericoronitis-Related Pain: A Morphometrical Observational Retrospective Study.

BACKGROUND: Mandibular third molar (M3M) removal and the management of postoperative complications represent a common matter of interest in oral and maxillofacial surgery. Pain represents a great symptom for patients affected by pericoronitis and it is the most common indication for third molar removal. The aim of the present article is to search for patterns of pre-operative pain in patients before undergoing third molar surgery and to test for a relation between some patterns of symptoms, such as pain intensity, site of symptomatic tooth, and referred area of pain. METHODS: This retrospecti

Long-Term Aesthetic Cicatrization Analysis of Lower Eyelid Incision for Orbital Floor Fracture Approach.

Orbital fractures are among the most frequent facial injuries. Of the 3 most widely described approaches in the literature, the lower eyelid approach is the authors' preference. This study retrospectively analyzed the patients treated at the Trauma Center of the Umberto I Hospital, Sapienza University of Rome from January 2010 to December 2020. Inclusion criteria were as follows: diagnosis of pure/impure orbital bone fracture, complete clinical and radiological documentation, and a minimum of 12 months follow-up. Sex, age, etiology, treatment, and associated complications were analyzed using I…

Association of clinical and social factors with risk of fracture in children with atopic dermatitis.

BACKGROUND: Children with atopic dermatitis (AD) have multiple risk factors for accidental fractures, injuries that can lead to significant morbidity and mortality. However, little is known about the factors that mediate the relationship between AD and fracture in children. OBJECTIVE: Our purpose was to examine the association of AD with fracture and to identify potential mediating factors. METHODS: This retrospective cohort study examined children with and without AD from a longitudinal matched cohort database of 353,040 children registered in the national health insurance service and parti

Will unilateral temporomandibular joint anterior disc displacement in teenagers lead to asymmetry of condyle and mandible? A longitudinal study.

PURPOSE: To evaluate the change of morphological symmetry of the temporomandibular joints (TMJ) during natural course of unilateral juvenile anterior disc displacement (UJADD) by comparing the ipsilateral joint with the contralateral side; and to analyse its effect over mandibular asymmetry (MA). PATIENTS AND METHODS: This study is a self-control longitudinal study. Consecutive UJADD cases with no history of TMJ infection, injuries to the jaws, or congenital, developmental and systematic disorders that may affect the craniofacial growth were collected and followed for at least 6 months. Combined pulsed dye laser and Q-switched Nd:YAG laser intraumatic facial tattoo removal: A case series.

Traumatic tattoos can be treated with several methods, including mechanical and chemical devices. However, they are rarely used due to the high risk of permanent side effects such as scarring and depigmentation. Recently, laser devices, especially the Q-switched (QS) laser and the pulsed dye laser (PDL), applied in combination, have achieved complete clearance of the lesions without any risk of side effects. Herein, we reported three cases of traumatic facial tattoos successfully treated with combined PDL and QS Nd:YAG laser.

Bone marrow-derived mesenchymal stem cell transplantation does not improve quality of muscle reinnervation or recovery of motor function after facial nerve transection in rats.

Recently, we devised and validated a novel strategy in rats to improve the outcome of facial nerve reconstruction by daily manual stimulation of the target muscles. The treatment resulted in full recovery of facial movements (whisking), which was achieved by reducing the proportion of pathologically polyinnervated motor endplates. Here, we posed whether manual stimulation could also be beneficial after a surgical procedure potentially useful for treatment of large peripheral nerve defects, i.e., entubulation of the transected facial nerve in a conduit filled with suspension of isogeneic bone m

Superior orbital fissure syndrome after repair of maxillary and naso-orbito-ethmoid fractures: a case study.

OBJECTIVE: The superior orbital fissure syndrome results from damage to the nerves passing through the superior orbital fissure. In the present case, the superior orbital fissure syndrome developed after repair of facial bone fractures, although the symptoms were not observed before surgery and no obvious cause was found. To investigate the aetiology of this syndrome, we examined the superior orbital fissure anatomically. METHODS: We measured the width of superior orbital fissure on the horizontal plane including the optic canal using the computed tomography (CT) scans of other patients and c

Exposure of the cervical internal carotid artery: surgical steps to the cranial base and morphometric study.

OBJECTIVE: Several studies have reported on approaches to increase exposure of the distal cervical internal carotid artery (ICA), but these studies have neither systematically addressed the anatomic aspects nor quantified the additional exposure of each maneuver. We describe surgical steps to expose the ICA region, quantify the additional exposure of each operative step, and discuss ways to minimize surgical morbidity. METHODS: The ICA was exposed in 10 formalin-fixed cadaveric heads using the following four steps: 1) anterior sternocleidomastoid approach, 2) retroparotid dissection and divis

Predictive value of computed tomography in visual outcome in indirect traumatic optic neuropathy complicated with periorbital facial bone fracture.

The visual outcome of 39 patients who had unilateral indirect type traumatic optic neuropathy and periorbital facial bone fracture treated at the Chang Gung Memorial Hospital in Kaohsiung between June 1996 and June 2001 was retrospectively evaluated, based on the results of computed tomography studies that grouped the associated fractures as anterior orbital fracture, posterior orbital fracture, orbital blow-in or blow-out fracture. Pretreatment and posttreatment visual acuities were obtained from the medical records and measurements of visual improvement degree and percentage were calculated

Unmyelinated trigeminal pathways as assessed by laser stimuli in humans.

Laser pulses excite superficial free nerve endings innervated by small-myelinated (Adelta) and unmyelinated (C) fibres. Whereas laser-evoked scalp potentials (LEPs) are now reliably used to assess function of the Adelta-fibre nociceptive pathways in patients with peripheral or central lesions, the selective activation of C-fibre receptors and recording of the related brain potentials remain difficult. To investigate trigeminal C-fibre function, we directed laser pulses to the facial skin and studied sensory perception and scalp evoked potentials related to Adelta- or C-fibre activation in heal

Facial asymmetry in subjects with skeletal Class III deformity.

We investigated the frequency, site, amount, and direction of facial asymmetry in human adults with mandibular prognathism and examined if these characteristics were associated postnatally with cardinal clinical signs that may indicate a predisposition to facial asymmetry. Two hundred twenty young Japanese adults (69 men and 151 women) who exhibited skeletal Class III malocclusions were selected. The sample was divided into a Postnatal Factor Group and a Nonpostnatal Factor Group. The former group included those who had: (1) received orthodontic treatment using a chin cap; (2) exhibited clinic

[A possibility of central diffusion during stellate ganglion blockade: "the sheath of the spinal rachidian nerve"].

Stellate ganglion block is a common treatment for neuropathic pain. The technique is not without potentially severe complications when a paratracheal approach is used. A 33-year-old woman complained of atypical facial pain of 15 years' duration with pain intensity of 6 to 8 on a visual analog scale and no pain-free periods upon use of inadequate analgesia. One minute after performing a second stellate ganglion block the patient showed signs of apnea and paralysis of the upper extremities and face, with no involvement of oculomotor muscles or the lower extremities, and no loss of consciousness.

Altered mandibular function and prevention of skeletal asymmetries after unilateral condylectomy in rats.

Unilateral condylar injury is known to be a frequent cause of mandibular asymmetry. Whether this is due to the trauma itself or to the disturbed function that follows the injury is a very important question with ramifications for clinical complications related to facial asymmetries. The aim of this study was to test the hypothesis that mandibular function in a protruded position can compensate for the absence of one condyle and prevent potential growth asymmetries. Forty-eight 4-week-old rats were divided into two experimental and two control groups consisting of 12 animals each, as follows: (
