BACKGROUND & OBJECTIVES: As adult dengue fever increases it also affects women with pregnancy. Dengue fever is mainly treated conservatively. However, complications like pre - eclampsia, pre- term labour, increased risk of caesarean section and fetal transmission have been noted. During dengue epidemic in our region we noted many women with dengue fever and observed certain different problems.
METHODS: A retrospective analysis of all pregnant women admitted in obstetric ward with dengue fever over a period of 18 months was done.
RESULTS: The striking feature observed was the presence of seve…

BACKGROUND: there are no previous systematic reviews of published pediatric case series describing the etiology of fever of unknown origin (FUO). The purpose of collecting these data is to determine the etiologies for children with FUO in both developing and developed countries.
METHODS: the database Ovid Medline R (1950 to August 2009 week 4) and Ovid Embase (1980 to 2010 week 2) were used to conduct the search. Studies in any language were included if they provided the diagnosis in a series of 10 or more children with FUO. The diagnosis of each child at the time of publication of the study …

The febrile neutropenic episode (FNE) constitutes a medical emergency, primarily because of the attendant risks of overwhelming infection and death. Infection, particularly bacterial in nature, is the most important cause of FNE, although there are other potential causes that depend on host, disease, and therapy related factors. The spectrum of microbiological pathogens varies both temporally and geographically, rendering it difficult to provide universal recommendations for antimicrobial therapy in FNE. Nonetheless, there are unifying principles in the management of FNE that are helpful in gu

To identify genes associated with the clinical presentation of dengue, 50 cases of probable or possible dengue hemorrhagic fever (DHF), 236 dengue fever (DF), and 236 asymptomatic infections were genotyped for 593 single-nucleotide polymorphisms (SNPs) in 56 genes across the type 1 interferon (IFN) response pathway as well as other important candidate genes. By single locus analysis comparing DHF with DF, 11 of the 51 markers with P<0.05 were in the JAK1 gene. Five markers were significantly associated by false discovery rate criteria (q<0.20 when P<6 × 10(-4)). The JAK1 SNPs showed differenti…

Fever represents a normal physiological response as a result of the introduction of an infectious agent producing exogenous and endogenous pyrogenes influencing the central set point of body temperature. This response is an important immunological defence. Fever can be defined as any elevation of body temperature above 38 degrees C. Infancy body temperature should be measured rectal. Fever is mostly caused by a benign viral infection, but it can be an indicator of a major illness such as meningitis, septicaemia, pneumonia. The risk for severe evolution depends on the clinical context and the a…

The most common clinical presentation of chronic Q fever is endocarditis with infections of aneurysms or vascular prostheses being the second most common presentation. Here, the authors report a case of vascular chronic Q fever. In this patient, a renal artery aneurysm was discovered by abdominal and pelvic CT during a systematic investigation to identify predisposing factors to chronic Q fever because of high antibody titres in a patient with valve disease.

In the Netherlands, the number of notified human Q fever cases showed a steep increase over the last three years and is not expected to disappear in the next few years. Since vaccination might be an option to prevent Q fever cases in the general population, evidence is needed about its effectiveness. We therefore conducted a meta-analysis to determine the evidence base for effectiveness for Q fever vaccination in human populations. We calculated Mantel-Haenszel risk ratios and we used the following formula to calculate the vaccines effectiveness: (1-mhRR) × 100%. Although individual and the po…

Since 2007, the Netherlands is faced with the largest outbreak of Q fever ever reported. In the last four years, over 4000 cases have been reported. The course of the epidemic and possible factors associated with this sudden surge in cases of Q fever is described and the preventive measures in the veterinary sector and the outbreak management of this unique epidemic are summarised. Finally, the latest data on clinical presentation and diagnostic and therapeutic dilemmas of Q fever in the Netherlands are reviewed.

OBJECTIVE: To present a case of culture-proven typhoid fever in which granulomas were demonstrated histologically in the ileum and mesenteric lymph nodes.
CLINICAL PRESENTATION AND INTERVENTION: A 47-year-old Pakistani man underwent emergency hemicolectomy for severe bleeding per rectum associated with diarrhea. Two large ulcers in the ileum, near the ileocolic junction, as well as mesenteric lymph nodes showed both necrotizing and non-necrotizing granulomas in addition to mixed inflammatory infiltrate on histology.
CONCLUSION: Typhoid fever may be considered as one of the causes of the diff…

Crimea-Congo haemorrhagic fever is a tick-borne viral zoonosis with the potential of human-to-human transmission. The disease occurs in extensive areas in Asia, South-eastern Europe and Africa. Haemorrhagic manifestations constitute a prominent symptom of the late disease stage, with case fatality rates from 9 to 50%. The recent increase in the number of cases in Eastern Europe and the potential for nosocomial outbreaks indicate the advisability of diagnosis in every patient hospitalized in Italy with haemorrhagic fever.

Viral haemorrhagic fevers (VHF) caused by arenaviruses are among the most devastating emerging human diseases. The most important pathogen among the arenaviruses is Lassa virus (LASV), the causative agent of Lassa fever that is endemic to West Africa. On the South American continent, the New World arenavirus Junin virus (JUNV), Machupo (MACV), Guanarito (GTOV), and Sabia virus (SABV) have emerged as causative agents of severe VHFs. Clinical and experimental studies on arenavirus VHF have revealed a crucial role of the endothelium in their pathogenesis. However, in contrast to other VHFs, haemo…

Miliary tuberculosis is a progressive disease resulting from the massive hematogeneous dissemination of Mycobacterium tuberculosis. Immunocompromised and elderly patients are the most affected. A case of an immunocompetent 45 year old man, with miliary tuberculosis and a simultaneous infection by Coxiella burnetti is presented. The clinical presentation demands a high awareness for the presence of tuberculosis. The rarity of this association, only one case reported in literature, appears to be a fortuity coincidence.
