Introducing medical abortion within the primary health system: comparison with other health interventions and commodities.

Over the years, a de-medicalisation strategy has been adopted for a range of public health interventions and commodities for the reduction o … he relationship between women and their health care providers. Measures for de-medicalising primary health services include adoption of simpler technology and service protocols, authorisation and training of less qualified providers, simplification or elimination of facility requirements, establishment of robust referral links to hospitals, increasing user contro …

User and provider perspectives on emergency obstetric care in a Tanzanian rural setting: a qualitative analysis of the three delays model in a field study.

The aim of this field study was to analyze the main dynamics and conflicts in attending and providing good quality delivery care in a local Tanzanian rural setting. The women and their relatives did not see the problems of pregnancy and birth in isolation but in relation to multiple other problems they were facing in the context of poverty. Local health professionals were aware of the poor quality of care at health facilities but were still blaming the community. The study describes the difficulties within the conceptual framework of the widely used "three delays model" to disentangle differen

Service-specific factors associated with unmet healthcare needs among people with disabilities in South Korea.

People with disabilities often experience unmet healthcare needs, and the factors associated with these needs may differ across healthcare service types. This study examined service-specific factors associated with unmet healthcare needs using Andersen's Behavioral Model. We conducted a cross-sectional online survey of 201 people with disabilities registered at a regional health and medical center in South Korea and their caregivers between November and December 2024. We assessed unmet needs for outpatient services, dental care, health screening, and home-visit rehabilitation, and classified i

Care coordination for persons with mental health challenges: a scoping review.

BACKGROUND: Care coordination is commonly employed to assist individuals with mental health challenges [MHCs]. However, its implementation in mental health contexts is inconsistent. The term, 'care coordination' is also used interchangeably with integrated care and case management. This review aims to (1) consolidate the literature on how, and in what contexts, care coordination has been used to help adults with MHCs access care from more than one service and (2) describe the challenges and benefits of implementing care coordination for adults with MHCs from the perspective of service provider

Migration to the Downtown Eastside neighbourhood of Vancouver and changes in service use in a cohort of mentally ill homeless adults: a 10-year retrospective study.

OBJECTIVES: Little research has investigated the role of migration as a potential contributor to the spatial concentration of homeless people with complex health and social needs. In addition, little is known concerning the relationship between possible migration and changes in levels of service use over time. We hypothesised that homeless, mentally ill individuals living in a concentrated urban setting had migrated from elsewhere over a 10-year period, in association with significant increases in the use of public services. SETTING: Recruitment was concentrated in the Downtown Eastside neigh

An interdisciplinary street outreach program to engage vulnerable neighbors in care.

Those experiencing poverty, homelessness, or behavioral health disorders are difficult to engage and maintain in health care services. These vulnerable populations experience unique barriers to engagement in health care services, which fuel health inequity. We need innovative approaches to address this need. This paper discusses the reciprocal relationship between a community center and a local university, which led to a street outreach program that provides a dynamic interprofessional experience for nursing, medical, and social work students and helps connect vulnerable populations to care. Estimated cost of treating myasthenia gravis in an insured U.S. population.

INTRODUCTION: In this study we estimated the costs paid by U.S. health plans for treating myasthenia gravis (MG) in 2009 and determined the … n per MG patient were $20,190 (SEM $4,763) and costs attributable to treating MG were $15,675. Home health services accounted for 23% of MG patient costs and represented almost exclusively IVIg infusion costs. Six MG patients had a total of 136 outpatient IVIg infusions at an average annual cost of $109,463 ± $57,303. CONCLUSIONS: The estimated annual health pla …

Pakistan's community health workers.

Pakistan's health characteristics are worse than those of other Asian countries at similar stages of development. Its mortality rate for children under five is 139 per 1,000, and its maternal mortality is 60 per 10,000. Malnutrition in women and children is widespread; 50 per cent of children under five are stunted. Pakistan's population growth rate of 3.1 per cent per year is among the highest in Asia. The high population growth rate and poor health status of many people call for extensive health care services, but, unfortunately, health services do not reach most of the people of Pakistan. P…

[Health policy of the Republic of Turkey in accordance with the Minutes of the Turkish Parliament (part I)].

Because of the invasion of the Ottoman territory after the World War I, the Turkish War of Liberty was initiated by Mustafa Kemal Pasha and his friends. While they aimed at a successful outcome of the battle, they at the same time tried to achieve a nation-wide organization. In order to look after the nation's medical and social service, on May 2nd 1920, a new ministry called the Ministry for Medical and Social Service was founded. The new ministry's task was not only to concern itself with the medical and social service, but also with immigration and of the immigrants. At the beginning the hi

Leveraging Policy Changes Made in Response to COVID-19 to Advance Telehealth and Novel Treatment Approaches for People Who Use Opioids: Qualitative Perspectives From Two Rural North Carolina Counties.

BACKGROUND: The COVID-19 pandemic has been a catalyst for telehealth services for people who use opioids; however, the data show dramatic increases in opioid overdose deaths and ongoing lack of treatment options in North Carolina, particularly among rural residents. METHODS: We conducted a listening session and qualitative interviews with 10 health care professionals, administrators, and community partners providing services for individuals who use opioids in Granville and Vance counties. Data were analyzed with an applied thematic approach. RESULTS: Participants described several benefits o…

Smoking cessation: health system challenges and opportunities.

The systematic integration of evidence-based tobacco treatment has yet to be broadly viewed as a standard-of-care. The Framework Convention on Tobacco Control recommends the provision of support for tobacco cessation. We argue that the provision of smoking cessation services in clinical settings is a fundamental clinical responsibility and permits the opportunity to more effectively assist with cessation. The role of clinicians in prioritising smoking cessation is essential in all settings. Clinical benefits of implementing cessation services in hospital settings have been recognised for three

The impact of PSTD on service access among people who use drugs in Vancouver, Canada.

BACKGROUND: Settings throughout the United States and Canada are contending with high rates of drug-related overdose. This in turn has prompted efforts to more effectively engage people who use drugs (PWUD) in treatment and care. However, while co-morbid mental disorders are prevalent among PWUD and can undermine access to services, the impact of post-traumatic stress disorder (PTSD) on service access is not known. Therefore, we sought to assess whether PTSD is associated with difficulties accessing health and social services among PWUD in Vancouver, Canada. METHODS: Survey data was derived f

Understanding Dysphagia Care in the Community Setting.

Factors including health policy reform and the aging population are increasing demand for quality healthcare in the community. People with dysphagia are supported by speech-language pathologists (SLPs) in hospital and community settings; however, little is known about the nature of dysphagia services offered by SLPs in the community. The aim of this study was to investigate SLP services and practices provided to community-based adults with dysphagia. A national cohort (n = 144) of SLPs working with community-based clients with dysphagia completed an online survey. Results revealed that clients

Occupational therapists' experiences in Australia's primary mental health funding scheme: Navigating complexity in a constrained system.

INTRODUCTION: Occupational therapists in the Australian context provide significant value to mental health, including through Better Access, a Medicare initiative that gives individuals access to up to 10 sessions with a psychologist, social worker, or occupational therapist following a referral from a medical practitioner. However, the experiences of occupational therapists working under the initiative are not widely understood, with no recent occupational therapy-specific research. This study aims to explore the experiences of occupational therapists working under Better Access, their distin
