Community sexual health providers' views on immediate postpartum provision of intrauterine contraception.

BACKGROUND: Increasing access to effective postpartum contraceptive methods can reduce the risk of unintended pregnancy and short inter-birth intervals. The need for an additional postpartum visit can be a barrier to women accessing intrauterine contraception after childbirth. Immediate postpartum intrauterine contraception (PPIUC) provision is known to be safe, but is not routinely available in the UK. Establishing this service requires multidisciplinary support, including from community and maternity stakeholders. The aim of this study was to determine the views of community sexual health pr

Use of telemental health for VA family services before and during the COVID-19 pandemic.

Military veterans experiencing relationship or family difficulties are able to access family services (i.e., couple and family therapy) through the Veterans Affairs (VA) Health Care System. Although family services have historically been provided face-to-face (F2F), the COVID-19 pandemic necessitated a sudden shift to the provision of care via telemental health, which includes videoconferencing (TMH-V) or audio-only phone appointments. This study demonstrated an unprecedented 16-fold increase in the number of TMH-V appointments for family services in VA during the first 9 months of the pandemi

Is economic inequality in family planning in India associated with the private sector?

This study examined the pattern of economic disparity in the modern contraceptive prevalence rate (mCPR) among women receiving contraceptives from the public and private health sectors in India, using data from all four rounds of the National Family Health Survey conducted between 1992-93 and 2015-16. The mCPR was measured for currently married women aged 15-49 years. A concentration index was calculated and a pooled binary logistic regression analysis conducted to assess economic disparity (by household wealth quintiles) in modern contraceptive use between the public and private health sector

The mental health of single homeless people in Northampton hostels.

Much research evidence about homelessness and health relates to large population centres with correspondingly large homeless populations. Such results may not necessarily apply to small towns such as Northampton. A local need was identified for information relating to health and access to health care for homeless people. Seventy five single, homeless people temporarily residing at open access hostels in Northampton were interviewed. The interviews covered: recent accommodation histories recent physical health mental health access to health care current social contacts, life style and aspiratio

On the compounding manifestations of racism shaping the US HIV/AIDS epidemic: why ending the HIV epidemic must address these factors for success.

OBJECTIVE: Growing racial/ethnic inequities in healthcare access and racially segregated sexual mixing contribute to persistent disparities in HIV incidence in the US. We aim to examine the extent to which eliminating racial/ethnic inequities in healthcare access could reduce disparities in HIV incidence and its interaction with assortative sexual mixing. DESIGN: A mathematical model. METHODS: We used two independently developed HIV transmission models to estimate HIV incidence among Black, Hispanic/Latino, and White/Other MSM and the corresponding incidence rate ratios (IRRs) comparing Blac

Healthcare providers' awareness, perceptions and experiences in integrating palliative care for women with HIV and cancer in Nigeria: a qualitative study on barriers and facilitators.

BACKGROUND: Women living with HIV face an increased risk of cancer due to compromised immune systems, necessitating integrated care approaches that include palliative care (PC). However, in resource-limited settings such as Nigeria, significant gaps in integrating PC into community healthcare persist, resulting in suboptimal outcomes for women managing HIV and cancer conditions. Understanding the barriers to and facilitators of PC integration from the perspective of healthcare providers is essential for improving care delivery. METHODS: A qualitative study was conducted across Nigeria’s six ge

Providers' assessment of barriers to effective management of hypertension and hyperlipidemia in community health centers.

We explored 251 providers' (47% licensed practical nurses, 27% registered nurses, 10% physicians, 10% physician assistants, 6% other) perceptions of barriers to effective management of hypertension and hyperlipidemia from 72 Midwest community health centers (CHCs). Optimal care for these diseases is difficult in any setting; little is known about the specific barriers CHCs face. Community health centers often have a multidisciplinary team that participates in patient care. Current models of quality improvement and chronic care management require virtually all CHC providers to know clinical gui

The age of trauma: the prevalence and psychological impact of potentially traumatic exposures in South Korea.

BACKGROUND: Exposure to potentially traumatic events (PTE) is associated with a range of negative mental health outcomes, including post-traumatic stress disorder (PTSD) and complex PTSD (cPTSD). Although exposure to PTEs is highly prevalent, and their impact on mental health is pervasive, research is mostly limited to diagnosed populations or specific trauma cohorts in the post-pandemic era. METHODS: We conducted an online survey of 1,000 Seoul residents from October 6-12, 2023, using a stratified sampling method. Participants were asked about their traumatic experiences, mental health outco

Exploring changes to financial protection and equity in Lithuania following 2017-2020 policies to improve access to outpatient medicines.

INTRODUCTION: Access to pharmaceuticals is a key area of health systems performance. Previous analyses showed medicines to be the largest contributor to catastrophic spending in Lithuania, particularly for people with lowest incomes. This paper examines changes to household spending on health following 2017-2020 pharmaceutical policies to improve access to outpatient medicines. METHODS: Household spending and catastrophic health spending were calculated using household budget survey data for 2016 and 2021. Pharmaceutical policies were obtained from the review of legislation and regulation, an

Expanding access to rehabilitation using mobile health to address musculoskeletal pain and disability.

INTRODUCTION: Musculoskeletal (MSK) disorders such as low back pain and osteoarthritis are a leading cause of disability and the leading contributor to the need for rehabilitation services globally. This need has surpassed the availability of trained clinicians; even in urban areas where services and providers are thought to be more abundant, access can be challenged by transportation options and financial costs associated with travel, care and lost time from work. However, continuing standard of fully in-person rehabilitation care for MSK-associated pain and disability may no longer be necess
