Sodium stibogluconate although potentially cardiotoxic is the drug of choice for Kalaazar and cutaneous leishmaniasis due to Leishmania braziliensis. Increasing use of this drug in the British Army has necessitated a formal evaluation of its cardiac side-effects. In a retrospective review of 697 postterm pregnancies we attempted to validate three tests used to identify the fetus at increased risk: the 24-hour urinary estrogen per gram creatinine (E/Cr), the nonstress test (NST) and the contraction stress test (CST). Sixty-eight patients (32 multigravidae, 36 primigravidae) with intact membranes admitted in early spontaneous labour were studied. Patients were randomly allocated to two groups: group I had an immediate amniotomy and group II were allowed to retain their membranes intact until full dilatation. Six male patients with narcolepsy for several years, were studied without and with amphetamine in order to evaluate possible abnormalities in autonomic control of the cardiovascular system. Studies were made of (1) heart rate and blood flow in the resting forearm during contralateral isometric handgrip, (2) respiratory sinus arrhythmia and (3) heart rate response to the Valsalva manoeuvre. A study of the fetal cardiac rate was made during uterine contractility induced by Pose's test in a group of 22 isoimmunized women carrying fetuses with variable degrees of hemolytic anemia. A high percentage of "positive tests" and other alterations in the fetal cardiac rate recordings were found. The risks of delivery and the need to examine the clinical value of different anomalies in the fetal cardiac rate are noted.

Four analgesic agents were studied during labour: pethidinesparine, ketamine hydrochloride, nitrous oxide-oxygen, and trichloroethylene in air (Trilene). Excellent analgesia was achieved with ketamine, however hallucinations were troublesome. Trichloroethylene analgesia was good without side-effects. Pethidine-sparine produced moderate analgesia but with nausea and/or vomiting. Nitrous oxide 50% presented poor analgesia. The reflex bradycardia occurring at the maximal isometric heart strain produced by clamping of aorta was shown to be less intense if the heart rate prior to the experiment had been low. Vagotomy prevented or decreased the bradycardia. The beta-adrenoreceptor blockade with inderal significantly reduced the bradycardia, whereas the alpha-adrenoreceptor blockade with tropaphen increased it. The important role of relationship between adrenergic and cholinergic regulation of the heart in development of the bradycardia was discussed.

Electrophysiologic evidence for dual pathways of conduction through the A-V node is presented in three patients without history of supraventricular tachycardia. In case 1, abrupt spontaneous changes in the PR interval from 0.17 to 0.42 second were seen.
